I have been diagnosed with spiking of tibial spine bilaterlly, tiny marginal ostophytes formation seen at right patellar polar region. I feel some hard object while lying down at night and also burning sensation in my anus region. Pl advise?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Spiking of tibial spine with marginal osteophytes are indication of degenerative changes in your knee region. Treatment of osteoarthrosis depends upon what grade of degeneration has occurred, what is your knee range of movements and what medications like analgesics do you use on a daily basis? recommendations for early oa are: - start doing daily brisk walks for at least 2-3 km in a day in mornings. - intensive knee - quadriceps and hamstring strengthening exercises should be initiated initially in presence of a trained physical therapist and then at home on your own - avoid sitting cross legged, not to use indian style washrooms, do not do anything which eccentrically loads the knee and increases cartilage breakdown. - adequate calcium and vitamin d metabolism in terms of appropriate intake is important to keep bone metabolism ok - you can take short course of anti inflammatory medication like tab etoshine on sos basis especially when there is an episode of inflammatory pain, swelling etc. Do not hesitate to contact me if you need any further assistance.

Sit ups, pushups, crunches, weight lifting exercise will help you in gaining muscle mass. Ideal body weight: 68-70 kg. Drink lots of water. Avoid stressing over your hair fall since dopamine released can decrease hair further. Stay away from chemicals like hair dyes, bleaches. Avoid straighteners and hot combs. Overwashing and overshampooing can make your hair fall. Wash your hair twice a week and shampoo on the same day. Use mild shampoos. Massage scalp thoroughly to increase blood circulation. Towel dry hair, do not use blow dryers. Make sure not to sleep with wet damp hair. Comb hair regularly before going out. Protect your hair from sun. Stay away from alcohol and caffeinated drinks like coffee and cola since they produce antioxidants which damage free radicals. Exercise regularly for it promotes blood flow and enhances healthy, shiny hair growth. Yoga and meditation help destress.

Whey protein is LIKELY SAFE for most children and adults when taken by mouth appropriately. High doses can cause some side effects such as increased bowel movements, nausea, thirst, bloating, cramps, reduced appetite, tiredness (fatigue), and headache. Protein is good for hair . For hair growth eat healthy diet.
 Include protein rich diet. Soya food, protein shakes, beans, pulses are good for hair. Nuts are good source of omega 3 and fiber. Its also good for hair. Drink lots of water.

Go get it operated, Laproscopic Hernia repair is best ,as you ll recover fast and will be able to perform normal sexual activity after your marriage, as one month is more than enough to recover! No other option left for you. As the hernia sac is touching testes, this ll definitely hamper your sexual activity!

M going through stress due to a new project abd mild depression as suggested by psychiatrist. He has advise to take flunil-10, fluanxol0. 5, zapiz 0.25, plse advise on medication m also having high bp which general physician as advised medicine. After first counselling with physiatrist I hvent felt good abd seems it was too time bound and effective solution was not suggested. Not aure on medication provided or should I continue with the counselling. Plse advise.

Hello I would suggest that you continue the medicines. It takes some time however after that you will be able to experience significant improvement. Also most likely your doctor will decrease and stop fee medicines in due course. As far as counselling is concerned, it's usually quite helpful. If you felt that you didn't benefit from it, you should share this thought with your doctor who would make changes to make it better for you. Finally I would advise you to complete the course of treatment so that any chances of relapse becomes minimal. Good luck.

The development of urinary stones is most commonly related to:
Decreased urine volume
Increased excretion of stone-forming components
Inadequate urine drainage, which may lead to stasis
Decrease in urinary citrate levels leading to deposition of calcium
Deficiency of vitamins a or c - these conditions can also lead to the" hyper triad: hyperparathyroidism, hypercalcaemia, and hyperuricosuria.

Treatment
When a stone causes no symptoms, watchful waiting is a valid option. For symptomatic stones, pain control is usually the first measure, using medications such as nonsteroidal anti-inflammatory drugs or opioids. More severe cases may require surgical intervention. For example, some stones can be shattered into smaller fragments using extracorporeal shock wave lithotripsy. Some cases require moreinvasive forms of surgery. Examples of these are cystoscopic procedures such as laser lithotripsy or percutaneous techniques such as percutaneous nephrolithotomy. Sometimes, a tube (ureteral stent) may be placed in the ureter to bypass the obstruction and alleviate the symptoms, as well as to prevent ureteral stricture after ureteroscopic stone removal.

Prevention
Recurrence of renal stones is common and therefore patients who have had a renal stone should be advised to adapt and adopt several lifestyle measures which will help to prevent or delay recurrence:
Increase fluid intake to maintain urine output at 2-3 litres per day.
Reduce salt intake.
Reduce the amount of meat and animal protein eaten.
Reduce oxalate intake (foods rich in oxalate include chocolate, rhubarb, nuts) and urate-rich foods (eg, offal and certain fish).
Drink regular cranberry juice: increases citrate excretion and reduces oxalate and phosphate excretion.
Maintain calcium intake at normal levels (lowering intake increases excretion of calcium oxalate).
Depending on the composition of the stone, medication to prevent further stone formation is sometimes given - eg, thiazide diuretics (for calcium stones), allopurinol (for uric acid stones) and calcium citrate (for oxalate stones).

Hello- urine gets its yellow color from a pigment called urochrome. That color normally varies from pale yellow to deep amber, depending on the concentration of the urine. Darker urine is usually a sign that you're not drinking enough fluid. Some times some medicines also cause the urine to become yellow.

You are having vit. D3 deficiency as well. Take vit. D3 60, 000unit tabs. 1 tab. Odx12tabs gem cal ds I tab odx15 then repeat vitd3 test. Why not discuss with me on video conference (facility provided by lybrate.)

She is a great human being and emotional person as other people are but with few exceptions. When someone (me or anyone) speaks to her and her point of view is declined or rejected, she gets hyper - high voice pitch with aggressive body language. I feel she feels that to defend her point till she fights will make everyone agree. In restaurant or public place, if I don't agree with her, she will raise her voice. While watching little but not highly emotional scene, she will have rolling tears. In friends, while having discussion, if debate happens that ends in crying or shouting. I can try two ways - changing myself and trying to control the situations or treating her somehow. But what about her reactions with outside people! Her hands start pointing or banging doors and sitting in the room. Also, when she is happy - she claps and laughs in front of strangers or people we just met. I am planning for a baby and want to know what are solutions without medications This is cultural for her to speak loud in her native place but not in our society and how I put across the point to make her agree. On personal front ,I have some problems like I smoke but outside balcony or toilet after her restrictions, but I feel I also add up by putting clothes anywhere, plates not back in kitchen which makes her angry. During her menstrual cycle, she is in severe pain and emotional (my5 days red status)

I appreciate your honest and elaborate putting forth of the problems. I believe your wife has PMDD which is premenstrual​ dysphoric disorder which is present in about 5-8 out of 100 women of menstruating age group. I think she does need to undergo a detailed investigation for the same along with a consultation with a psychiatrist for help with medications. With regards to her behavior, I think she does fit into a personality spectrum if not a disorder and certainly needs therapy in the form of counseling. If the counseling psychologist thinks that she needs to get treated for a psychiatric condition, I believe​ we can help her with medications as psychiatrists. I am sure if it is a problem in all situations and not just with you, it certainly calls for treatment medically. A combination of therapy including medications and counseling is probably more beneficial than medications alone. Look forward to hearing from you. Hi. Kindly follow the below points: Home-made remedies: - Take 1 tbsp of fuller's earth (multani mitti) + a dash of lime + bit of rosewater – apply this on your affected area till it gets dried up, then rinse it off. - Take 1 tsp of Alum (or fitkari), make it slightly wet with water and apply this on your wet area, spread it well; let it dry and rinse it off. Alum remove your excessive moisture or oil and it also tightens the skin's tonicity. Medication: You can take homoeopathic medicines (for acne) – Berberis Aquefolium Q/ 10 drops in 1/2 cup of water/ thrice a day (and for local application as well) and Kali Brom. 200/ thrice a day – for 20 days. Medicines (for hairlfall) - Weisbaden 200/ thrice daily for 1month. Use Schwabe's Arnica shampoo and Hair growth oil.

Wash your face with Bakson's Facewash (wash your face with warm water only) and followed by CTMP, i. E. Cleansing, toning, moisturising and protection (apply liberally Sunscreen Gel with SPF-30) after washing your face daily. Prevention: -Take healthy food only and avoid unhealthy ones (although sebaceous glands are not excretory glands then as well!) as what you eat is reflected on your face! -Avoid taking unnecessary stress/ tension as it encourages secretion of stress hormone in the body as thus affects skin or any other tissue of the body. -Drink atleast 8-10 glasses of water in a day. -Take 6-8 hrs. Of sound sleep everyday. -Don't use others towels, handkerchief on your face as well as never share yours. -Never ever rub your face as it damages the skin (even towel should be dab on skin softly) and break outs of acne get worsened! -Maintain hygiene, wash your face 2-3 times in a day, changes the pillow covers, bed sheet daily, use cotton bed sheet only (try to sleep on your back).
