Cancer and Future Fertility - How To Preserve?

MD - Obstetrtics & Gynaecology, MBBS, FNB Reproductive Medicine, MRCOG
IVF Specialist, Mumbai
Cancer and Future Fertility - How To Preserve?

Cancer is increasing in prevalence with improved detection and diagnostic techniques so prolonging the life and improving the quality of life.It has been observed to be affecting more of younger age groups so can have a lethal effect of the future fertility by destruction of their gametes (sperm or eggs). As doctor we feel need of spreading awareness at multidisciplinary level regarding preservation of future fertility which needs counseling and guidance at preliminary stages only before the damage happens.

Various Methods-

  • Oocyte/Egg cryopreservation
  • Sperm Cryopreservation
  • Embryo freezing

Method will vary as per the clinical status

Oocyte  cryopreservation.

Egg freezing means removing mature eggs and freezing them for later. This process may also be called egg banking . Its done in cancer patients preferably before exposure to chemo and radiotherapy. Its same like an IVF cycle except that embryos are not formed. Here  medications are given  to stimulate the development of multiple follicles in the ovaries.

Type of cancer can also decide the protocols.

Besides there are now more patient friendly mild stimulation protocol especially in estrogen sensitive cancers

The eggs are retrieved from the patient using a transvaginal technique under anesthesia and are kept frozen for future use by a method called vitrification.

Frozen eggs can be kept frozen for years(5 years as per ICMR but not specified for cancer cases) till the woman decides to use them for conception  and once the patient is ready to get pregnant, the eggs are thawed and then fertilized using ICSI, an assisted reproductive technology that injects one sperm directly into the egg. The embryos (fertilized eggs) will then be transferred into the woman’s uterus (womb) in order to achieve a pregnancy.

Embryo Freezing

It has the highest success rate compared to any other treatment.  Its like normal IVF when eggs are retrieved and mixed with sperms. Once fertilized, the embryos (fertilized eggs) grow in the laboratory for 3-5 days until they are suitable for freezing.

Sperm Freezing
Sperm Freezing is an effective and simplest way for men who have gone through puberty to store sperm for future use. Many men can store sperm (either from natural ejaculate or from a TESA sample

Causes;-different types of cancers may be having ------- different types of multiple causes, not any one, in some cancers--viruses have been detected. Adultrated foods, allopathic salts, hormonal drugs, chemical salts, paints, alcohol, smoking, constipation, unhealthy water, foods, uncompatible foods, excessive sex-loss of semen, industrial hazards, polluted air, acidity, arthritis, stones, unknown reasons etc. Are other causes. Prevention;;-- you can prevent cancers by avoiding its possible causes. ---------------------------- - hi dear, you can identify cancer by its symptoms/ findings etc. Symptoms;--generally pain is main symptom but mainly --------- depend upon the organs involved. Other symptoms can be -bleeding, obstruction, ulceration, fever, weakness, loss of appetite, indigestion etc. Etc. Early detection;;- is generally difficult. But ---------------- lumps may be detected, palpable. Suddenly found when ever pain or some problem is there.

Prostate enlargement requires a full clinical evaluation for the cause of enlargement. It is important to assess ultrasound status and post void residual urine along with serum PSA before starting any medications. If benign hypertrophy is present you can start tablet Urimax D daily once at bedtime.

Good morning, I am Dr Dinesh Singh I am director of Radiation Oncology at Max Hospital, Vaishali. Today we are going to discuss about cancer, the cancer is a group of about 100 disease different diseases and it is basically the cells they start developing, multiplying and then they do not stop first they grow locally and there and then they spread out to different parts and what are the different symptoms of the cancers, well it depends upon which part of the body the cancer is developing like we are discussing about the brain tumors that the patient might have difficulty in vision, he may be having headache, he may be having vomiting or he may be having fits. Then to further investigate we need do a contrast study of the MRI then we diagnosed and then we keep recording it. For head and neck cancers like they can wear ulcer or a soar which doesn't heal, there can be a change in the voice the person might have difficulty in swallowing or so these are the early symptoms of cancers originating into the head and neck area. Then we further investigate we take a biopsy and then we diagnose the patient. For breast cancer, the earliest symptoms is a painless lump usually in Cancer in early stages pain is not there, so whenever there is a lump in a breast it should be investigated to allowed cancer. More than 90% of the lump in the breast on cancerous but it should be evaluated and cure, when we come to the lung cancer the common symptom is a mild cough. So any cough which is not getting the cure for more than two weeks it should be taken seriously it can be tuberculosis, it can be cancerous, so it has to be evaluated and investigative. Talking about the GI cancers of the intestinal cancers, well it present like indigestion or the patient is not getting well some might discomfort in the abdomen. So any problem persisting more than 2 weeks should be investigated by ultrasound, by endoscopy and then we do it. Carcinoma of the rectum and anal canal usually present with a bleeding associated with the stools or without stool should not be confused with the piles because they have the same presentation. So any problem persisting for more than 2 weeks is the thumb rule has to be investigated diagnosed and they have to be treated. Once we see that there is a problem then what we do we have to do an investigation and the first investigation is the biopsy many times a general people they have a false notion that if a biopsy is done then cancer will spread, well it is absolutely wrong. The biopsy is a must and it does not spread cancer. Thank you.

My grandmother is suffering from breast cancer. Is there is medicine without chemo?

Fellow Breast Surgery (MUHS), DNB (General Surgery), MBBS
Oncologist, Aurangabad
Hello. Whether a patient receives chemotherapy or not in case of breast cancer will totally depend on the stage of disease at presentation to the doctor. If your doctor has prescribed chemotherapy, there are very good chances that it is mandatory. There are very few but well identified instances when chemotherapy is either not required or can be avoided safely or avoided in view of risk of side effects being more possible than the benefit. Other than stage, the things that influence this decision, are age, any other diseases like diabetes or high blood pressure, etc. The biology of cancer e. G. If er, pr positive disease and whether the disease has already spread but not causing any functional harm. Such a decision has to be taken by a well learned and well experienced doctor, along with an informed decision. From patient. I can guide more only with more of patients details.

I am 20 years old. I have a lump inside my breast can you tell me the symptoms of breast cancer.

BHMS
Homeopath, Kolkata
Symptoms and Signs of Breast cancer: A Change In How The Breast Or Nipple Feels Nipple tenderness or a lump or thickening in or near the breast or underarm area A change in the skin texture or an enlargement of pores in the skin of the breast (some describe this as similar to an orange peel’s texture) A lump in the breast (It’s important to remember that all lumps should be investigated by a healthcare professional, but not all lumps are cancerous.) A Change In The Breast Or Nipple Appearance Any unexplained change in the size or shape of the breast Dimpling anywhere on the breast Unexplained swelling of the breast (especially if on one side only) Unexplained shrinkage of the breast (especially if on one side only) Recent asymmetry of the breasts (Although it is common for women to have one breast that is slightly larger than the other, if the onset of asymmetry is recent, it should be checked.) Nipple that is turned slightly inward or inverted Skin of the breast, areola, or nipple that becomes scaly, red, or swollen or may have ridges or pitting resembling the skin of an orange Any Nipple Discharge—Particularly Clear Discharge Or Bloody Discharge It is also important to note that a milky discharge that is present when a woman is not breastfeeding should be checked by her doctor, although it is not linked with breast cancer.
