Respected lybrate-user tokas hi I don't recommend medicines for weight loss at all. Just avoid more oily spicy stuffs, do regular walking if 3-4 kms a day, do some exercises by your own have more salads monitor weight on regular basis sure you will get results. So far as breast discharge is concerned its hormone related but you can primarily go for ultrasonography or mammography of breast definately. Thanks regards.

Effective Management of Rheumatoid Arthritis

MBBS, MD - Medicine, ACLS, Post Graduate Course In Rheumatology, Masters in Psychotherapy and Counselling
General Physician, Jaipur
Effective Management of Rheumatoid Arthritis

Over time, our joints, connective tissues and muscles go through a lot of wear and tear. Additionally, certain conditions may also cause problems, including inflammation and conditions that create persistent pain. These conditions usually fall under the broad category of arthritis, which can be of many types. Rheumatoid arthritis (RA) happens when your immune system targets your joints and causes inflammation. RA influences the joints on both sides of the body, for example, both hands, wrists and knees. This symmetry separates it from other kinds of arthritis. It can affect the skin, eyes, lungs, heart, blood, or nerves.

Various signs of inflamed joints are as follows:

  1. Stiffness: Functioning of the joints becomes rigid and doesn't move properly. It is particularly severe in the morning. While those with different types of arthritis have stiff and immobile joints in the morning, people with rheumatoid arthritis take over 60 minutes before their joints feel free.

  2. Swelling or Inflammation: Liquid enters the joint and gives rise to inflammation.

  3. Extreme pain: Irritation inside a joint makes it painful and delicate even to touch. In a chronic condition, this may also lead to fractures.

  4. Redness and warmth: The joints might appear hotter and more pink or red than the skin around it.

Needless to say, your lifestyle choices affect all aspects of your physical wellbeing, and that also include your joints- especially if you are suffering from arthritis.  It’s a common theory, that joints which are strained and stressed by bearing the burden of extra pounds show more inflammation, wear and tear and stiffness which lead to aggravation of arthritic pain. Here are a few holistic approaches to consider; certain changes in your existing lifestyle that can help you to reduce your arthritic pains:

  1. First and foremost, it’s important that you must indulge in plenty of exercises every single day, to keep pounds off and thus help yourself handle this degenerative disease.
  2. In case of creating a healthy diet plan, stick to low calorie and low carb foods. Also be sure that you are consuming lots of vegetables and fruits in your diet. This also helps you to keep fit and put a break on the obesity factors that are detrimental to your health.
  3.  As even a few extra pounds can worsen arthritic pain, keep track of your weight and talk to your orthopedic doctor to determine your optimal and healthiest weight. Focus on this prescribed weight coefficient and keep tracking your weight loss patterns unless you have reached your goal.
  4. Cut down on alcohol. There has never been a finer alternative to that.
  5. Feeling depressed or stressed by arthritic pain is a common syndrome among arthritic patients. Nevertheless, on the flipside, such reactions actually worsens the agony of this disease and inhibit you from doing what you need to do, such as taking medications and exercising to keep yourself fit at the end of the day.

Although admittedly, making drastic modifications in your lifestyle can be painful and difficult at first, these are the best remedies for easing arthritic pain in a natural way.
Don’t just rely on medications, take control of your disease in a holistic manner and you’ll soon find yourself in control of your life again.

Lifestyle Changes

Being overweight is a large risk factor for osteoarthritis because it puts a large amount of stress on weight-bearing joints. This damage is largely preventable by losing excess weight and increasing safe activity. Exercise (as long as it does not put dangerous strain on the joints) will improve your muscle strength. Strong muscles will support your weight-bearing joints and ultimately lower the chances of the negative symptoms of OA. Regular exercise can help prevent and reverse these effects. Several different kinds of exercise can be beneficial, including range-of-motion exercises to preserve and restore joint motion, exercises to increase strength (isometric, isotonic, and isokinetic exercises), and exercises to increase endurance (walking, swimming, and cycling). Exercise programs for people with rheumatoid arthritis should be designed by a physical therapist and tailored to the severity of your condition, your build, and your former activity level. Getting a sufficient amount of rest is also necessary for management of OA symptoms. If you wish to discuss about any specific problem, you can consult a General Physician.

Do You Have Atrial Fibrillation - All You Need To Know?

Do You Have Atrial Fibrillation - All You Need To Know?

Atrial fibrillation is a condition that results in rapid heartbeat leading to heart failure, stroke, and other cardiac complications. In this condition, the upper valves of the heart beat irregularly. Some of the common symptoms of this condition include weakness, heart palpitations and shortness of breath. There could be several episodes of Atrial fibrillation. Although it is not life threatening, there could be serious complications owing to the nature of the condition. Atrial fibrillation can also lead to a condition known as the ischemia.

What are the different types of Atrial fibrillation?

  1. Occasional- This condition is also known as paroxysmal fibrillation. Here the symptoms come and go. For few patients, the symptoms might stay for a few minutes, for others, it can stay up to hours before everything becomes normal again.
  2. Persistent- The beat of the heart does not go back to normal. Medication along with electric shocks might be required to amend the condition.
  3. Long-standing persistent- This is the kind of Atrial fibrillation that can last up to 12 months at stretch.
  4. Permanent- This is the type where the erratic rhythm of the heartbeat can’t be restored. Long term medication is required to fix the condition.

What are the possible causes of Atrial fibrillation?
Some of the possible causes of Atrial fibrillation include Coronary artery disease, sleep apnea, viral infection, lung diseases, exposure to tobacco, caffeine, etc, metabolic imbalance, hyperactive thyroid gland, viral infections, congenital heart defects, stress due to illness, pneumonia and surgery, history of heart surgeries etc.

What are the risk factors for Atrial fibrillation?
Here are some of the common risk factors for this condition:

  1. Age plays a critical role in any cardiac related disorder. The older a person is, the greater are the chances of Atrial fibrillation
  2. High blood pressure along with unhealthy lifestyle can increase the risk of Atrial fibrillation
  3. People with a history of obesity can get this disease
  4. Chronic conditions such as kidney disease, metabolic syndrome, thyroid problems can lead to Atrial fibrillation
  5. People with an existing heart condition such as heart problem, heart palpitation, valve failure, coronary artery disease etc have an increased risk of suffering from Atrial fibrillation.

How is Atrial fibrillation diagnosed?
A doctor might use one of the many tests or diagnostic technique that are available to diagnose this condition:

  1. ECG- This is first but an important test to fathom whether there are any irregularities in the heart beat. The sensor presents in the ECG machine throws heart bit diagram in the form of graphs.
  2. Echocardiogram- This is a technique wherein sound waves are passed through the heart and an image is taken to understand about any possible irregularities. This is a fine test that can readily detect underlying and structural heart diseases.
  3. Blood tests- Blood tests can help to identify any thyroid irregularity or other condition that might lead to Atrial fibrillation. If you wish to discuss about any specific problem, you can consult a Cardiologist.

Rinse your face with water and pat it dry. Crush up mint leaves thoroughly, you can use a blender but a mortar and pestle works quite well if you happen to have one lying around. Rub the juice and crushed leaves on your face and leave for 5-10 minutes before rinsing off thoroughly with cold water. Do cleansing and tonning of face daily before sleep.

Mr. lybrate-user, Thanks for the query. It appears you are on a fairly high dose of Glimepiride. Metformin (2000 mg) is also fairly high. Usually Glimepiride should not be used more than 4 mg in a day. Even though recommended upper limit is 8 mg, conventionally using more than 4 mg is not advised. Since there is no mention of actual glucose levels (fasting, PP and HbA1c%), there is no way to know how good is the control. Ideally in a well controlled diabetes fasting should be < 100 mg, PP 150 to 160 mg and HbA1c% <6.8. Another aspect looking at your weight and height it is clear that with a BMI of 31.25 kgs /M2 you are already in grade 1 obesity. This can be one of the major impediments in proper glucose control. As regards continuing the same medications for long time, yes you can do that. These drugs are safe. Millions of people are using them for years together. Further whether in future glucose level will remain in control with the same drugs can not be predicted. Most important is to look at your diet control and regular exercise of over 1 hour a day. That will ensure better control. Thanks.
