PCOS

Polycystic ovary syndrome (PCOS), also called hyperandrogenic anovulation (HA), or Stein?Leventhal syndrome, is one of the most common endocrinedisorders among women. PCOS has a diverse range of causes that are not entirely understood, but there is evidence that it is largely a genetic disease.

PCOS produces symptoms in approximately 5% to 10% of women of reproductive age (approximately 12 to 45 years old). It is thought to be one of the leading causes offemale subfertility and the most frequent endocrine problem in women of reproductive age. Finding that the ovaries appear polycystic on ultrasound is common, but it is not an absolute requirement in all definitions of the disorder.

The most common immediate symptoms are anovulation, excess androgenic hormones, and insulin resistance. Anovulation results in irregular menstruation,amenorrhea, and ovulation-related infertility. Hormone imbalance generally causes acne and hirsutism. Insulin resistance is associated with obesity, type 2 diabetes, andhigh cholesterol levels.The symptoms and severity of the syndrome vary greatly among those affected.

CAUSES:

PCOS is a heterogeneous disorder of uncertain cause.

The severity of PCOS symptoms appears to be largely determined by factors such as obesity.

PCOS has some aspects of a metabolic disorder, since its symptoms are partly reversible. Even though considered as a gynecological problem, PCOS consists of 28 clinical symptoms.

Even though the name suggests that the ovaries are the cornerstone of disease pathology, cysts are a symptom instead of the cause of the disease. Some symptoms of PCOS will persist even if both ovaries are removed; the disease can appear even if cysts are absent. Since its first description by Stein and Leventhal in 1935, the criteria of diagnosis, symptoms, and causative factors are subject to debate. Gynecologists often see it as a gynecological problem, with the ovaries being the primary organ affected. However, recent insights show a multisystem disorder, with the primary problem lying in hormonal regulation in the hypothalamus, with the involvement of many organs. The name PCOD is used when there is ultrasonographic evidence. The term PCOS is used since there is a wide spectrum of symptoms possible, and cysts in the ovaries are seen only in 15% of people.

PCOS may be related to or exacerbated by exposures during the prenatal period, epigenetic factors, environmental impacts (especially industrial endocrine disruptors such as bisphenol A and certain drugs) and the increasing rates of obesity.

Not everyone with PCOS has polycystic ovaries (PCO), nor does everyone with ovarian cysts have PCOS; although a pelvic ultrasound is a major diagnostic tool, it is not the only one.

DIET AND MANAGEMENT:

The primary treatments for PCOS include: lifestyle changes, medications and surgery.

Goals of treatment may be considered under four categories:

Lowering of insulin resistance levels
Restoration of fertility
Treatment of hirsutism or acne
Restoration of regular menstruation, and prevention of endometrial hyperplasia and endometrial cancer
General interventions that help to reduce weight or insulin resistance can be beneficial for all these aims, because they address what is believed to be the underlying cause.

As PCOS appears to cause significant emotional distress.

LIFESTYLE CHANGES:

If you are overweight, weightloss may be all the treatment you need. A small amount of weight loss is likely to help balance your hormones and start up your menstrual cycle and ovulation.
Eat a balanced diet that includes lots of fruits, vegetables, whole grains, and low-fat dairy products.
Get regular exercise to help you control or lose weight and feel better.
If you smoke, consider quitting.

I am 46 years and diabetic for the past 10 years with reasonable control and insulin dependent. I would like to know how does alcohol affect me since I consume two drinks every day.

Fever is not considered medically significant until body temperature is above 100.4 F (38 C). Fever serves as body's natural defences against bacteria and viruses which cannot live at higher temperatures. Also, the body's defence mechanisms seem to work more efficiently at a higher temperature -Fevers of 104 F (40 C) or higher may be dangerous and demand immediate treatments like: -Acetaminophen (dolo-650 mg) twice or thrice can be used to lower a fever . Aspirin should not be used for fever in children/ adolescents or during a viral illness. -Wear loose fitting clothes and don’t overdress. Overdressing can cause temperature to rise further. -Tepid water (85 F [30 C]) baths may help bring down a fever  -The body should be cooled with a wet sponge. -A fever can cause severe dehydration. Drink plenty of fluids.. Have citrus fruits, particularly Vitamin C rich ones like mausami, Amla juice etc.  -Take light and easily digestible home cooked meals like sprouts.avoid outside food -Wash hands frequently with soap and water especially after returning from public places or public transport.  -Further therapy depends on the cause of fever like urinary infection/typhoid or malaria/dengue infection and the accompanying symptoms that need to be investigated and treated accordingly

My mother has frequent urination problem from last 4-5 yrs. Urine comes in very less quantity in drops with burning pain. USG and urine test gives no symptoms of problem. She is not diabetic. Blood found in urine which is not seen by eyes directly. Doctor says that it may be shrinkage of the tube (narrow urethra. Please advice some medicine to cure.

For headache take tablet paracetamol 650 mg eight hourly and few things related to headache so that we can plan the treatment accordingly - if pain is associated with blurred or decreased vision than check your eye sight for refractive error - if the pain is associated with cold and fever than we have to rule out sinusitis as the cause and if the pain is pulsatile in nature and with a throbbing pain usually with an aura than we have to rule out migrane as the cause - if the pain is associated with neck problems than we have to rule out cervical spondylosis as the cause - if the pain is associated with either low or high bp or due to hyperacidity than we have to treat the cause after examination.

Glycemic Index in Diabetes

The glycemic index is a measure of food containing carbohydrates and how it can raise the level of glucose in blood. This is essentially a process where every food is ranked based on a reference food such as white bread. A food with a high GI value increases the level of glucose in the blood compared to a food with a GI that is medium or low. Some of the common examples of food with a low GI include legumes and dried beans. Fats and meats are not included in the index due to the non-availability of carbohydrate in them.

Some common foods with a GI of 55 or less include bread made up of pumpernickel and whole wheat, muesli, oatmeal, bulgur, barley, pasta, yam, lentils, sweet potato, converted rice, corn, non-starchy fruits, and vegetables. A food set with medium GI in the range of 56 to 69 includes quick oats, basmati rice, pita bread and couscous. Some example of foods with high GI value in the range of 70 includes pineapple, russet potato, white bread, macaroni, puffed rice, corn flakes, rice cakes and melons.

What influences GI?

Since GI has nothing to do with fibre and fat, some general findings of the GI count are as follows:

  1. When a food is processed or cooked, the GI tends to increase

  2. More the storage time of the food, the higher is the GI. It is applicable for ready to cook food and frozen food as well.

  3. The higher is the GI count, more the ripeness of a fruit or vegetable.

  4. Converted food items tend to have a lower GI as compared to the original version of the food

What are the other considerations?

While the GI value gives first-hand knowledge about the type of carbohydrate a person is consuming, it is hard of any help when it comes to the amount of carbohydrate intake. Portion size, therefore, still plays an important role for patients suffering from diabetes. The GI count of a food item greatly varies when combined with a food which has higher GI or lower GI for that matter. Nutritious food that is extremely beneficial for the body, tends to have a higher GI count. For instance, the GI count of oatmeal is greater than that of a chocolate.

Which one is more effective?

While there is no hard and fast rule for maintaining the carbohydrate count, an approach that is equally balanced between a GI count and carbohydrate count works best for a patient. Both the type of carbohydrate as well as the count of carbohydrate plays a crucial role in keeping the blood sugar level under control. If you wish to discuss about any specific problem, you can consult a endocrinologist.

Ideally masturbation should not be more than 2-3 times a week for any adult. It will keep you sexually active. Too much of everything is bad as always. Along with this schedule of masturbation you need proper diet which should consist of fresh fruits, fresh vegetables and one hour of jogging early morning every day. This will keep you fit for future activity also. Do not keep negative thoughts. They will hinder in future psychologically.

1. Take one tablet of crocin pain relief as and when required upto a maximum of three tablets daily after food 2. Cold sponging sos 3. Drink plenty of water 4. Take rest 5. If no relief do blood examination like esr, cbc, widal/ typhidot test, blood for mp/malaria antigen, dengue antigen (ns1) test, urine re/me after consulting physician and follow the advice and treatment.

I am 30 years old n having no medical issues. My height is174 n weight is 74 kg, last five years I am not feeling morning erection, and when I am unable to do healthy intercourse, before 1 min I ejaculate, almost six months I have taken treatment under a qualified sexologist, but after stopping medicine, results is as usual. Dear lybrate user, kindly note that if you are unmarried & indulge in sexual relation after an interval of minimum 7 days then you are bound to discharge quickly during intercourse but that does not imply that you have any disease. It occurs due to loss of control of the muscles of penis. Normally it is seen that when a person does sexual intercourse daily he gains control over the muscles of penis & can stay longer during sex. You need to take homoeopathic mother tincture salix nigra q, 30 drops, thrice daily, after meals, in a cup of water to get rid of premature ejaculation. But before taking any medicine kindly notice whether you are having a second erection within 15 minutes of ejaculation for the first time. If you are not experiencing any erection within 15 minutes of ejaculation then you are surely suffering from erectile dysfunction. There will be a change of medication in this case. To get rid of erectile dysfunction you need to take homoeopathic mother tincture agnus castus q q, 30 drops, thrice daily, after meals, in a cup of water. Along with medicines you can also try a home therapy to boost your sexual performance. Soak 4-5 dry dates (khurma khajur) in a cup of lukewarm milk before going to bed daily. Kindly note the following steps to perform the kegel's exercise:- 1) start urinating until you feel that you have voided almost half amount of urine from bladder. 2) when you feel you have voided almost half of the urine try to stop the flow of urine by squeezing the internal muscles of penis. 3) you should not use your hands to stop the flow. Your hands should be resting on your waist. You have to stop the urine only by your self control over the internal muscles of penis. 4) try to stop the urine flow by the control of internal muscles up to 20 seconds. On the very first day you won't be able to hold the urine for more than 3 seconds. However, practicing the exercise daily will improve your control over the muscles of penis & after 15-20 days you will be able to stop the urine for 20 seconds by the control of your pelvic muscles. This control of pelvic muscles will help you to get rid of premature ejaculation while you are performing sexual intercourse 5) after 20 seconds is over, void the rest of the urine. Do not stop the flow of urine for more than 30 seconds. N. P.- kegel's exercise is effective only in the case of premature ejaculation & it is not helpful for patients suffering from erectile dysfunction.
