Why does pain occur? The mechanism of pain explained

Pain is an unpleasant and distressing physical sensation caused by disease or injury, which induces hurt and anxiety in those suffering from it.
There are many different manifestations of pain, ranging from short term (acute pain) to long term (chronic pain). Other classifications include pain in the internal organs (visceral pain), injured tissue (inflammatory pain), nerves (neuropathic pain), etc.

The reason for the occurrence of pain depends on the cause and kind of pain being experienced. In most cases, pain acts as a warning sign that the body has been afflicted by a disorder of some sort, which may have arisen due to internal sickness or external wounding. Acute pains are generally cured on their own through rest or simple medication. Chronic pains, on the other hand, are more complicated in nature and the treatment requires more elaborate diagnosis.

The mechanism of pain

Pain is an extremely personal and subjective experience and affects each individual to varying degrees. The pain signals, which can arise in any part of the body, travel through the spinal cord to the brain along thousands of specialized nerves and nerve fibres. In the brain, it is processed in the centres associated with anxiety, emotions, memory, appetite, etc. Signals and pain inputs are then returned from the brain to the spinal cord, which may heighten or diminish pain.

In some cases, pain may also be induced by damage to the brain and spinal cord, which happens after a stroke.
A constant barrage of pain signals may cause the cells at the end of nerve fibres to become over-sensitized. This is known as ‘wind-up’ and is one of the most common causes of chronic pain that occurs even though the root of the problem has been identified and treated.

The causes and effects of pain differ from case to case depending on the signals received by the brain and its interpretation. It affects the individual’s day to day activities and if persistent, can also have a harmful impact on mental health and psychosis. Pain is always handled, diagnosed and treated differently in all patients by drawing out the best possible solution to the problem.

My father has back bone pain problem. When he sit for a long time his backbone at lower side pains too much. When we go to x-ray and other report got swelling in lower backbone at the end. Suggest treatment. It is about 4 months. A little treatment he takes but there is no relief. Please suggest treatment.

Hello Avoid sitting on hard surfaces. Take hot water formentation or hot packs twice a day. It will take time to recover.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Pain, paresthesia, numbness etc in thigh especially in outer aspect can be due to compression of a nerve which is called as meralgia, also it could be due to compression and symptoms from the back region. Initially you can start with: - physical therapy like tens - medication like pregabalin or gabapentin in small doses can be started and we can monitor the response - can also take an anti-inflammatory medication for some time especially if there is any pain associated. Let me know in around 1 week -10 days how are you feeling, we can get some tests to establish the diagnosis and also step up the medications as per the response. Apply ice on painful area for 5-6 mins then ointments for 2-3 days. Still pain persist then go for ultrasound therapy by a physiotherapist.

Pain in musclesis a common condition. Do hot fomentation keep a pillow under the legs at night &/when resting. Any way take ibuprofen 200mg od & sos x 5day caldikind plus (mankind) 1 tab odx10days you will need other supportive medicines also. Do knee exercises it may have to be further investigated make sure you are not allergic to any of the medicines you are going to take. Do ask for detailed treatment plan. Do not ignore. It could be beginning of a serious problem.

Take rest for few days. Kindly show me photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Kindly take biod3 max 1 tab dailyx10 paracetamol 250mg od & sos x5days do back (spine)/shoulder/knee exercises make sure you are not allergic to any of the medicines you are going to take do not ignore it. I am 48 year male. Stiffness in my fingures started around one year ago. It is not continous. Some time it pains also during sudden jerks like situation. My knees feel more colder then other parts of body during winter. Kindly suggest.

If you want treatment by your choice, then you can go to any road side bone setter. He will give you n number of exercises and manipulations, creams and lot many things. If you want proper treatment for your problem, then ask for treatment of choice. Locking of knee is mainly because of meniscus tear. Now ho ahead and get treatment from knee specialist or zola chhap. It's your choice.

spine extension with against pressure. but do gradually in prone position on the bed bridging exercise, do thrice a day with 20 movement cycling, (start 10 to 15 minutes daily) quadriceps exercise 20 times twice a day. with 1 kg weight cuff

For pinched nerve/compressed nerve treatment varies, depending on the severity and cause of the nerve compression. You may find that you benefit greatly from simply resting the injured area and by avoiding any activities that tend to worsen your symptoms. You should see a doctor if you feel constant numbness or pain in the area despite minimal use or if the muscles in the affected area feel weaker over time. Seek immediate treatment if the symptoms are severe or the area feels cold or appears very pale or blue. You might consider seeing a physiotherapist. She/he can give you specialized stretches and exercises that aid in healing a pinched nerve. Certain exercises relieve the pressure on the pinched nerve, helping to reduce pain. Many of the stretches used in this healing process must be done with a trained professional, so don't do them on your own.

Spring beds, folding beds or thick matress are harmful Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again. Do not ignore. It could be beginning of a serious problem.

Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold). Advice. Avoid sitting cross legged. Ankle stretching. Ankle strengthening exercises. Avoid squatting. Quadriceps exercises. Core strenthening exercises.

First of all check your posture when you are lifting any material like heavy bucket . And try to lift the heavy bucket without bending your back . you may lift the bucket after bending your knees only. This will definitely help you. * Gentle massage with lukewarm mustard oil of your waist area will also give you relief. * Surya Namaskar is only exercise which you need to do. * In diet vegetable protein of cereal , B complex supplements and half teaspoon of haldi in a cup of milk for seven days will give you enough relief.
