Know Everything About Sinusitis Surgery

What is a Sinusitis Surgery?

The traditional sinus surgery involves the removal of the diseased sinus tissue with improvements in the drainage channels by creating a definite pathway so as to drain out the infection from the cavities.

Usually, endoscopic techniques allow for better access and visualization as compared to making external incisions. This furthers the recovery process from this sort of a surgery.

Unlike other surgeries, a sinusitis surgery aims at re-routing the infected sinus pathways in addition to clearing the damage pathways. Post-operative care is as important as the surgery itself. In fact, insufficient follow-up and poor post-op care are two of the most important reasons behind the failure of this surgery.

Few essential things you need to know before such a surgery:

  1. Balloon Sinuplasty is a comparatively newer method to treat and expand the natural drainage mechanisms of one’s sinus passages.
  2. The patient should refrain from eating or drinking any beverage at least eight hours prior to the surgery.
  3. Once the surgery is carried out, the patient can go home and rest with his/her head elevated for faster recovery. Also, applying a towel wrapped in ice can also help stop the bleeding that might so commonly occur.
  4. Recovery from the surgery can take about 5 days and the patient should not carry out any strenuous activity until cleared by the surgeon.
  5. Post-operative care is of the essence in this sort of a surgery.
  6. Follow up with the doctor if you happen to have fever that refuses to subside even with medications, headaches that do not respond to medications or bleeding that doesn’t stop.
  7. Any swelling in and around the nose or fluid draining from the nose should not be left ignored and untreated.
  8. Identify and stay away from allergens, moisturize your nose frequently with the help of nose moisturizers and avoid the common cold and flu.

Surgery choices

Endoscopic surgery is preferred over traditional surgery for most cases of chronic sinusitis that require surgery. It is less invasive, less expensive, and has a lower rate of complications.

  1. Endoscopic surgery may be done to remove small amounts of bone or other material blocking the sinus openings or to remove growths (polyps). Normally, a thin, lighted tool called an endoscope is inserted through the nose so the doctor can see and remove whatever is blocking the sinuses.
  2. Sinus surgery may be done when complications of sinusitis, such as the development of pus in a sinus, infection of the facial bones, or brain abscess have occurred. In this type of surgery, the doctor makes an opening into the sinus from inside the mouth or through the skin of the face.

What are the complications and risks?

  • Recurrence of polyps or other nasal problems.
  • Excessive bleeding post-surgery.
  • Chronic drainage from the nasal cavities, crusting of one’s nose and excessive dryness.
  • Further aggressive surgery might be required.
  • Damage to one’s eyes.
  • Damage to one’s skull, including drainage of spinal fluid.
  • Facial or upper teeth numbness.
  • Polyps which result in nasal obstruction.

Here are some suggestions for taking care of your nose and sinuses after surgery:

  1. Keep your head elevated to help reduce bleeding and swelling after your operation. The first night after surgery, elevate your head with extra pillows or sleep in a recliner.
  2. If you have packing material and splints in your nose, make sure they stay in place. If the packing gets clogged, breathe through your mouth. Do not remove the packing or splints.
  3. Some bleeding is normal for 2 to 3 days after your operation. If you think you are bleeding a lot, be sure to call your doctor.
  4. Don’t blow your nose for at least a week after surgery. Don’t do any heavy lifting, straining, or strenuous exercise. This increases the likelihood of bleeding in your nose.
  5. If you have to sneeze, try doing it with your mouth open.
  6. Don’t take aspirin. It slows clotting and increases bleeding.

I have tonsil problem for last 30 years, what should I do?

MS - ENT(Gold Medalist), MBBS
Ear-Nose-Throat (ENT) Specialist,
U need to do warm saline gargles daily and avoid cold and sour food. If tonsils are enlarged and causing problem in swallowing or breathing or causing recurrent sore throat, surgery (tonsillectomy) will be a better option instead of medical treatment.

Ethmoid sinus problem, like sinusitis, can cause a pain which makes you believe it is coming from the eye. Actually, the origin of the pain is behind the eyes. Medications prescribed for sinusitis will also relieve your eye pain within 1 or 2 days. Otherwise, if pain in the eye persists, it needs an ophthalmologist consultation immediately.

I do not want to go for sinus operation. Can I use otrivin nasal drop whole life because in night I need it to have a sound sleep.

MS - ENT(Gold Medalist), MBBS
Ear-Nose-Throat (ENT) Specialist, Delhi
U should not use otrivin nasal drops bcz on long term use causes side effect in form of rebound phenomenon called rhinitis medicamentosa that means persistent nasal block for which it is used. Instead of that use flomist nasal spray twice a day for 2 weeks and steam inhalation also twice a day.

Dear Dr. I have continuous snoring and insomnia during my sleep and I feel more sedative when I wake up in the morning. Can you please guide me how to recover from this.

PDDM, MHA, MBBS

Clear nasal passages. If you have a stuffy nose, rinse sinuses with saline before bed. Using a Neti pot, nasal decongestant, or nasal strips can also help you breathe more easily while sleeping. If you have allergies, reduce dust mites and pet dander in your bedroom or use an allergy medication. Keep bedroom air moist. Dry air can irritate membranes in the nose and throat, so if swollen nasal tissues are the problem, a humidifier may help. Change your sleeping position. Elevating your head four inches may ease breathing and encourage your tongue and jaw to move forward. There are specially designed pillows available to help prevent snoring by making sure your neck muscles are not crimped. Sleep on your side instead of your back.

Sinus problem occurs bcz of inadequate drainage of sinus tha may be due to some mass in nose, deviated septum, allergic polypoidal mucosa. You need to take antihistaminics and steroid nasal spray. Avoid exposure to dust. Get a ct scan done and review with report.

Hi If you have never experienced  this problem, then you need to start  *an antibiotic  immediately.  *A decongestant  medicine *Avoid water entry in the ear. *Avoid shampooing for atleast 10 days till the ear discharge dries up. *Get a Pure tone audiometry and tympanometry done. These tests can detect ear drum perforation. Otoendoscopy  may also be done Ear discharge may occur due to acute or chronic otitis media. Your symptoms  appear to be of Acute otitis media. In this case ear discharge occurs after a small perforation  in the ear drum due to pus buildup.  But if you have had the problem previously  too it may be due to Chronic otitis media. In that case you have to also start an ear drop and may need surgery.

First thing you should go for ct scan of nose and sinus than treatment is based on grade of the sinus, if it is grade iv than no treatment is available or effective unless you get this corrected by surgery, surgery will give you nearly 80-90%. Than remaining you can treat with homeopathy than it will help. The bone of he nose gets deviated this leads to nose breathing problem if a person suffering from nasal obstruction along with sinusitis and mild to moderate nasal allergies it means most probably he is suffering from dns (deviated nasal septum. Nasal obstruction is because of the deviated portion of the nasal septum (this is the bony partition of the nose cavity. Allergies are because of the hampered muco-ciliary mechanism (normally mucus is produced in the nose to moisten our nose cavity, oral cavity, and help food bolus to move downward, this mucus is basically moves on the cilia of the nasal mucosa) this mechanism gets hampered and mucus start accumulating in the nose and sinus cavity which is why thick mucus comes from nose in patients of nose obstruction and person develops sinusitis. When a surgeon removes the nasal septum deviation has not been removed properly the symptoms persists, and patient thinks that the nose bone has again regrown. It is not because of regrowth but because of half surgery done or due to the non removal of the deviation (reason can be many) nowadays nasal septal deviation can be removed endoscopic ally. But if the sinusitis is grade ii or iii than there are possibility that you may get relief from symptoms.
