Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You will have to specify which part of the hand are you talking about, are there any associated symptoms like numbness, paresthesias etc present as well? do you have any neck pain and do you feel pain actually radiating from your neck region to whole upper limb? it will be useful if you can send me a clinical picture highlighting the involved region. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

You have borderline high bp, reduce salt but I am not suggesting a salt free food. Avoid tinned foods as salt is a preservative. Avoid other packed foods also like chips, namkeem, street foods. Use minimum oils for cooking. Eat lots of seasonal vegetables and fruits. Reduce stress by exercise and yoga and meditation. Take a tablet of safe betablocker like metoprolol 25 mg twice daily. Get back here with progress reports only after observations, after 2 weeks. Get back here with reports. You will get more guidance from any of us here on the forum.

I am having neck pain radiating to my right thumb which started around oct'2014. I am working in it sector and prolonged hours I have to sit infront of computers. I took some treatment in ayurveda for almost one month. Then later shifted to idd therapy which showed hood results and I was able to get back into my job after 1-2 months treatment. There's a disc prolapse in c5 c6 region and that's the reason. So, now the pain again started to come back although not severe and I am afraid I have to quit my job and go for surgery. Can any body help me send a suggest a solution without surgery.?

DNB (Radiodiagnosis), DMRD, MBBS
Radiologist, Bangalore
Hi, thanks for writing in to us. Your mri scan shows disc problem at c 5 - c 6 level. This requires rest, physical therapy and pain relief as first line of treatment. Maxgalin er is a good medicine and will treat nerve related pain. The condition you have might be due to trauma or degeneration. It is unlikely to heal completely. You can try a rehabilitation and treatment with nerve pain reliever like maxgalin er. Please do not stress your neck for a long duration and avoid sudden neck movement. Go slow on your job. Non surgical pain relief options are best. They might be steroid shots or rf ablation. Regards,

OSTEOARTHRITIS

BPTh/BPT
Physiotherapist, Gurgaon
OSTEOARTHRITIS
Degenerative joint disease, osteoarthrosis, and osteoarthritis are all the same disease characterized by the joint cartilage breakdown causing painful and stiff joints. Literally translated as inflammation of the joints, osteoarthritis is part of the family of diseases known as arthritis which refers to 100 various diseases. Osteoarthritis commonly causes disability. There is no cure for osteoarthritis, but there are effective treatment options.
Osteoarthritis often only strikes one joint, usually a joint that bear a lot of stress such as the hip, back, knee, or hand joints. Symptoms include pain, painful use of the joint, painful periods after the joint has been used, joint discomfort during significant changes in the weather, swelling in a joint, stiffness of the joint, bony lumps in the hand and finger joints, a decreased amount of joint flexibility.

The cartilage between joints is supposed to be smooth to allow joints to easily slide over it while in use. Over time some people lose this smoothness of the cartilage and there is a rough, sandpaper like appearance to the joints. This means that the joints can no longer slide gently over the cartilage. This is osteoarthritis. The cause is time and age and the eventual wearing down of the joint’s cartilage, but it is not necessarily easy to indicate who will suffer from osteoarthritis and who won’t. Joint stress, injury, and heredity tend to play a role in these factors, but it is not an absolute.

CLINICAL MANIFESTATIONS are -

Joint Pain aggravated by use and relieved by rest.
Night Pains.Stiffness of the Joints.Swelling of the Joints.
Tenderness of the Joints.Limited Movement of the Joints.
Wasting of Muscles around the Joints.
Movements of the Joints make crepitus.
There is Deformity of the Joint.
Risk factors for osteoarthritis include gender, as osteoarthritis strikes more females than male, age, as osteoarthritis is typically found in people over 45 years of age, joint injuries especially those related to sports, obesity, diseases that cause malformations or disfigurement of the bone structure, weakened quadriceps, and other forms of arthritis

Factors Leading To Arthritis are -

Overweight.
Lack of Exercises.Excessively rich diet and fatty substance and overeating.
Modern Life (Sitting in chairs and working too long).
Mental Factors like TENSIONS, SUPRESSIONS OF EMOTIONS, FEAR, HYPERSENSITIVITY.
Physical examinations can often determine the presence of osteoarthritis. In some cases a physician may take a list of physical complaints, a physical examination, blood tests which rule out other varieties of arthritis, X-rays, MRI, and other various imaging techniques to determine the presence of osteoarthritis. Imaging can reveal the cartilage loss and the development of boney growth which is indicative of osteoarthritis.
The greatest complication related to osteoarthritis is pain, as often the pain can be so detrimental that it is crippling and prevents mobility or the use of the joint. It is not uncommon for a patient to experience acute and intense pain for the first year of living with osteoarthritis only to have it diminish to reasonable levels. Pain medications are often more intense for the first year of living with osteoarthritis but can cause complications of their own if used for too long. Thus, pain medications are only used when absolutely necessary.

There is no actual cure for osteoarthritis, although there are treatment options available. When the pain is intolerable for more than a year, most physicians recommend replacing the joint. Joint replacement technology has improved dramatically over the past few years and most joint replacements have been done with wonderful success. Other pain relieving options include topical pain relievers, some of which are prescription strength, NSAIDS to help with both the pain and the inflammation, as well as other over the counter pain relievers. Corticosteroid injections can often relieve joint pain for a reasonable amount of time, but are not typically used as a regular therapy for more than a period of one or two years. Low dose antidepressants have been proven to relieve pain without the side effects of narcotics. These medications are prescribed in doses low enough to prevent the effects of antidepressants while still providing pain relief. There are some anticonvulsants that will provide the same type of pain relief even if the patient has no signs of depression or seizures.

YOGIC APPROACH for ARTHRITIS -

Asana – VAJARASANA, PADMASANA, AKARNA DHANURASANA, MARJARASANA.
Hatha Yoga Kriyas – PURNA and LAGHU SHANKHAPRAKSHALANA, KUNJAL and NETI.
Pranayamas – NADISHODHAN and BHASTRIKA.
Meditation – Releases mental and emotional tensions, reduces arthritic rigidity.
DIET –

Cooked light grains and cereals.
Boiled pulses.Boiled and baked vegetables.
Green Salads (TOMATO, BEET, CARROTS).
Fresh Fruits.Reduce intake milk and dairy products

Hi sir!
My mother is suffering from knee pain from 5 years
When she take ayurvedic medicine then she is fine for some...
From her symptoms it seems like she has developed osteoarthritis (oa) of knee. A heating pad or cold pack can feel really good on achy joints. Use whichever one feels better unless you have an injury, in which case cold is best for the first couple of days. You can apply cold or heat several times a day. Just remember to cover the cold pack with a towel and keep your heating pad at a low setting to avoid burning your skin. Whether you use heat or cold, remove it after 20 to 30 minutes. Physical activity is essential for people with osteoarthritis, whether it means walking around your apartment. Activities that cause pain should be avoided temporarily. -if the knee is swollen or sore, the following positions and activities you should avoid until knee pain and swelling resolve: squatting kneeling twisting and pivoting try to take the dietary supplements glucosamine for knee pain. Try using a cane to help her walk can take some of the weight off a painful hip or knee. It might even slow the damage from knee osteoarthritis. Please see a physiotherapist for detail assessment and management of knee pain. Best wishes.

A very practical approach I have been using for most chronic knee pain sufferers planning surgical intervention!!!! It gives amazing post op results!!
An interesting blog by David Buttler ( from whom I learnt it)!!!

" Knee replacements in the brain ? the patient?s side
Posted by David buttler.

John Barbis, aging Philadelphian PT had a bilateral knee replacement 4 months ago. I have just watched him scurry though the snow like a squirrel and run up stairs like a five year old. He can sit on his haunches and he has been doing all this for months.

John says ???I felt immediate ownership of my new knees after surgery. But I had prepared well ? more my mind than the rest of my body. I started two months before the surgery. I did left/right knee discrimination exercises 3 times a day, I visualised the surgery, I visualised the IV going in, then going under and waking up. I did lots of mindfulness , the UCLA mindfulness project. I visualised my knees moving after the surgery and I imagined how they would move much more than they did before. I imagined that I would have pain, but I reasoned that it would not be pain related to any danger to my knees. I knew medication would be available if I needed it.?

?Having seen knee replacement surgeries before was helpful. I knew they would be bending my knees full range in the operating theatre and I knew it would only be swelling and sensitivity that would limit me post-surgery and this would be temporary and a part of the healing process. I knew the only way I could hurt myself was if I fell. I visualised walking normally as soon as I woke. I only used a walker at night for a few days, but I didn?t want use it as I wanted a normal walking pattern using my arms as soon as possible?.

?The new knees were all mine as soon as I woke up and I was glad to say goodbye to the old ones. I wanted to be in total control. I didn?t want anyone pushing on my knees. There is no reason why someone else should have to push on someone?s knee to get range of motion?.

?I knew there would be nociception, I knew there would be neural flares but I knew they could be made safe. I avoided the total knee education class offered ? it was scary?

?Overall, I worked on my brain more than working on my knees?

Hello,Dandruff causes hair loss so its necessary to treat dandruff first. Take Thuja 200, 5 drops in the morning, once in a week. Take Arnica Q and Cochleria arom. Q in equal amount, mix with equal amount of coconut oil, and apply this on your scalp, thrice a day. Wash your hair with B&T Anti- Dandruff Shampoo. Revert me after 1 month. Home remedies are as follows:- Coconut oil helps eliminate dandruff due to its antifungal properties. It also moisturises dry scalp and provides relief from itching. Take some coconut oil and mix half the amount of lemon juice in it. 1. Rub it on your scalp and massage for a few minutes. 2. Wash your hair after at least 20 minutes. 3.
