Hi doctor, I'm very fearful while picking up the client call and strange people around me. My voice tremble like anything. Though, I know the answer to the client call, I couldn't explain to them comfortably. I feel ashamed when I finished call as I had a bad call. And, when strangers aren't around, my confident and way of speaking is really good. I'm not sure where this fear goes. Please help me. This is my career related issue and I have to come out of this problem. Currently, I'm taking medicines for hand tremor Betacap TR 40, Benfomet plus and Lonazep 0.25 which is suggested by neurologist. Please advise.

You are just afraid of people and their judgments of you. This is closely related to the fear of public speaking, which everyone has, including me. But you will not notice it because we use the fear t prepare and deliver the talk properly. You can imagine that nobody is there and just speak naturally. You are obviously good but have a low self-confidence and esteem. It is all about trusting people. Do you know why you do not trust people? Have you had a bad experience? If not, you will still need to meet people and live with them. The ultimate test of a personality is in the social context. The greater test is when we meet with the opposite sex. And the final test is when we have to bed and board with a member of the opposite sex in marriage. If you wait for that time to resolve it, it might be too late and traumatizing. Even if you are afraid of people, face your fear and see what happens. They are certainly not going to kill you. So what is the worst that can happen? Really nothing but it will feel great to have friends and lots of companions in life. We are after all social animals, gregarious by nature and need each other. So don’t handicap yourself on this front. Attend a personality development class, personal grooming class, public speaking class, and conversation classes. It will help to do a lot of this with a close friend, if you have one. If not, find one. We are into relationships all the time. It is near impossible to live without people. All research has shown that living with someone gives us good health and long life too.

I have been suffering from severe migraine and vomiting since last 1 month. Is it a symptom of migraine or something else?

I am giving you idea of Symptom of migraine Headache eye pain. (most commonly one-sided pain; less frequently both sides of the head are affected) pain located near the eye on affected side. pain that worsens with exertion or physical activity. Nausea/ Vomiting sensitivity to light and/or sound consult me for further management

Headache

Headache is one of the most frequently encountered problem at a clinic. Even questions at lybrate are mostly studded with this symptom.
More often than not, headaches are self limiting and without a serious medical problem. But any chronic headache needs proper evaluation for definitive therapy.
Grossly, headaches can be divided into primary (no definite structural lesion in brain) or secondary (associated with changes).
Any headache, if associated with vomiting without nausea, visual disturbance or nerve deficit (new onset squint, double vision, loss of sensation anywhere, facial deviation amongst many) may indicate secondary headache, but exceptions are there. These require immediate attention to rule out life threatening disease.
Commonest primary headache is tension type headache all of us have felt at certain times. Often it is like a band pressing around forehead. Common analgesics manage them well but for recurrent problem, we might prescribe prophylactic drug.
Migraine is something very common presenting as pain in one half of head with vomiting, nausea, visual or auditory aura. They require immediate analgesic like acetaminophen and prophylaxis with propranolol or amitriptyline.
Cluster headache, as the name suggests, comes in cluster for few days and more common in male. Oxygen has been proven as effective treatment.
Trigeminal neuralgia is more sharp, lanceolating pain, but responds satisfactorily to carbamazepine.
Sinusitis, in frontal bone presents as headache more around 10am in the morning that requires therapy with antihistamines and antibiotics occasionally.
Therefore' headache' carries little value unless it is described in detail to ensure proper therapy.

. 1. It could be due to essential tremor. The cause of the tremor is unknown. In some cases it is hereditary and runs in families. There is no definite treatment unless it is severe 2 other causes of tremor could be stress, anxiety, panic, nervousness, inadequate sleep, physical exhaustion, strong emotion, hyperthyroidism, hypoglycemia, smoking/tobacco/alcohol abuse/withdrawl, excess tea/coffee, drugs like steroid/amphetamine, neurological disorder, 3. Investigate yourself to exclude the above causes of tremor after consulting neurologist 4. Avoid stress/anxiety/nervousness/ panic etc 5. Avoid excess tea/coffee/ smoking/alcohol if you have 6. Take good nourishing diet 7. Have adequate sleep. 8. Do a thyroid function test to exclude thyroid problem 9. Consult neurologist for further advice.

Insomnia is a mental condition, which is characterized by a difficulty in falling asleep, even when you have the chance to do so. Hie, my mom is 40 years old. She feels vibration under her pelvic area since a month. It doesn't pain her but annoys her a lot. Numbness in bottom area could be due to pressure on the nerve. This could be due to lumbar spondylosis with nerve root compression. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Any way take paracetamol 250mg od & sos you will need other supportive medicines also. Do back (spine) exercises make sure you are not allergic to any of the medicines you are going to take you may have to use lumbosacral support for some time. Don't ignore.

My son , now 19 yrs underwent treatment for viral encephalitis when he was 12 yrs. Now under anti epileptic medicines like gardinal , topomac,frisium,levipil,lacosom . At your age you can identify the particular words that pose a problem to you and then write those words down and articulate the letters and the words they form just as a young kid would do; and you will register an audio-visual memory of those particular words to gradually develop an audio-visual impression and memory of the same. It will be even more useful to write the meanings of those words and relate to them as well. You could sing the words and meanings to make a lasting impression. But you should not rush all the methods at once to learn quicker: it will only confuse you and make it even more difficult, and will lead to frustration. In a more advanced situation you can print index cards of those words and learn to memorize them in the same fashion as mentioned above. As time goes by you may get audio books and listen to them often but always in an atmosphere conducive to good listening. If you wish you could also get the help of reading teachers who will professionally take you from simple material to higher material as you advance in the learning program. The audio mode of learning is slower than the visual mode but speed is not an immediate objective, comprehension and word recognition is. It will be a long process but if you are determined, it will be rewarding. You could have benefited from an earlier intervention, like in childhood, if they had noticed and acted then. Please remember that your intelligence is not in question. So it is up to your commitment, sincerity and above all perseverance.
