My father is a paralyzed men. And he is 45 years old. His mouth smells bed even he Brushes 3 time a day. What should he do. Please suggest.

General Physician (AM)

Persistent bad breath or a bad taste in the mouth may be a warning sign of gum (periodontal) disease. Gum disease is caused by the buildup of plaque on teeth. Bacteria cause the formation of toxins to form, which irritate the gums. If gum disease continues untreated, it can damage the gums and jawbone. Other dental causes of bad breath include poorly fitting dental appliances, yeast infections of the mouth, and dental caries (cavities). The medical condition dry mouth (also called xerostomia) also can cause bad breath. Saliva is necessary to moisten the mouth, neutralize acids produced by plaque, and wash away dead cells that accumulate on the tongue, gums, and cheeks. If not removed, these cells decompose and can cause bad breath. Dry mouth may be a side effect of various medications, salivary gland problems, or continuous breathing through the mouth. Many other diseases and illnesses may cause bad breath. Here are some to be aware of: respiratory tract infections such as pneumonia or bronchitis, chronic sinus infections, postnasal drip, diabetes, chronic acid reflux, and liver or kidney problems. You can buy a number of mouthwashes that claim to eliminate bad breath. However, many of these generally provide only a temporary way to mask unpleasant mouth odor. There are, however, several antiseptic mouth-rinse products that kill the germs that cause bad breath. Ask your dentist about which product is best for you.

Hi Sir, My brother had cough & fever from Sept-Nov 2015 he was on Antibiotics, Paracetamol and cough syrup for same time but didn't got any relief finally done Chest CT scan in Nov and found Pleural Effusion, pleural fluid aspirate done on 24th Nov but whole fluid didn't not aspirated because patient get vertigo problem during aspiration so doctor stop aspiration, from 24th Nov TB treatment started. He got mild relief from cough and fever But after 15-20 days he again faced severe cough and mild fever, again CT scan and Chest sonography done and found some pleural fluid. Again, pleural fluid aspiration done and TB treatment continue, from that day he started getting relief day by day. Now is feeling absolutely fine. TB treatment is continue from Nov till today. From April he started feeling Hip shoulder back knee and Feet pain, some blood test done and found Vitamin B12 & Vitamin D is very much deficient. Both vitamin B12 and D injections and capsules Tablets supplement started and got felling better but didn't get any relief in Feet pain. Since last 40 days he having pain, pin needles and burning sensation in both feet.As per doctor prescription all types pain killer reliever like Dynapar SR, Zobid Zerodol SP etc taken but till today didn't got any relief in feet pain, even pain killer didn't work for 1 hour. We took opinion from Orthopedic and Neuro Surgeon and done MRI of Spine and Feet, MRI is normal as per MRI report but Feet pain and burning is still persist Now we consult with Neurologist, he said it look that it's Neuropathy due to TB medicine Isonizad. He said to consult TB physician and stop taking Isonizad and he prescribed Tab Pregabalin, Zen 100, Ultracet and Deep Senz ointment. We again consult with our TB physician, he said TB drug Isonizad can't be withdrawal because if we stop it TB May relapse later. As per doctor TB treatment will continue for more 3 months i. E. Till the end August Feet pain burning pin needles sensation is still persist. So now we are very confused and can't understand what to do. We have following questions 1 Please advise us what to do 2 Is Neuropathy is permanent? 3 Can neuropathy can be cured? 4 Can we stop Isonizad? Thanks & regards.

DNB(Respiratory Diseases), MBBS
Pulmonologist, Jabalpur
Neuropathy is usually irreversible, but can be benefited after some time and taking few drugs with tb drugs to prevent further neuropathy. Isoniazid can be stopped but it may weaken the course of treatment and the chances of getting tb again may increase with a weak course.

I am addicted to masturbation. I do it daily. Is there any way to get rid of that. I am loosing my weight. How to overcome this problem.

MBBS, Diploma in Child Health (DCH), DNB (Pediatrics), MNAMS (Membership of the National Academy), MRCPCH, Certificate in GI Endoscopy
Pediatrician, Delhi
I understand that it is a difficult situation for you and you are quite stressed. Let me assure you that there is no scientific evidence suggesting any correlation between weight loss or masturbation. There is no need to'get rid' of it or to'overcome' it. If you want to gain weight, you need to have a nutritious diet and exercise adequately. Please consult an adolescent health specialist in person.

What are the first aids proffered for migraine when it sees several times in a day time?

PGP In Diabetologist, Fellowship in non-invasive Cardiology, MD - Medicine, MBBS
Endocrinologist, Delhi
headache ,which is occuring several times in a day may not be migraine at all. typically migraine attacks are longer in duration ,lasts at least 2-4 hours but in some of my patients they lasted for 2-3 days. it is also affecting only one side of head, and most of my patients know beforehand that they are going to get an attack( aura). so, please get properly investigated for your headache starting from eye check up etc. and visit a good physician

I am a 27 years old male, I have migraine for the past 7 years. What should I do?

MBBS, MD - Consultant Physician
General Physician, Ghaziabad
Welcome to lybrate. We are here to help you. Such pattern of headache is seen in migraine. Migraine headaches are usually unilateral, associated with throbbing, vomiting sensations, photophobia and phonophobia. They aggravate on stress. They also aggravate on watching tv, loud noises, sleep deprivation. A clinical examination specially neurological examination, fundus examination are must to rule out any other neurological problem which might also require neuroimaging. I would also advise you to get your eyes tested. You should see your doctor. He will classify the headache and if it is migranous, treatment would be with the drugs like flunarizine, propanolol, amitryptilline. The drugs are decided on the basis of severity, frequency and duration of headache. I hope it helps. Stay healthy.

This is regarding my brother in law. He has been experiencing seizures since childhood whenever he has a lack of sleep the previous night. Recently we got his EEG, MRI (FOR EPILEPSY PROTOCOL) and other blood tests done which was referred by Dr., Mumbai. All the tests were normal and the doctor prescribed Valprol CR 200 thrice daily for 2 years and also said that if the medicines are taken on time for the said period then the condition would be cured forever. So could you please confirm me is it true if the medications are taken on time then he would lead a seizure free life forever? Please let me know.

DM - Neurology
Neurologist, Rajkot
Your brother might be suffering from idiopathic generalized epilespy. If he has additional myoclonus then it can jme. Valproate is a good choice for such epilepsy. 2 years of seizure control is required but it is not necessary that after that he will be seizure free for life. Because sometimes seizures recur if the precipitating factors are not taken care of. Like sleep deprivation, stress, fasting, etc.
