Hip Replacement Surgery - What To Expect?

MBBS, MS - Orthopaedics, DNB - Orthopedics

Hip Replacement Surgery - What To Expect?

Hip replacement is a common surgical procedure that you may need if your hip has been damaged due to arthritis, a fracture, or other serious conditions. All of these can make common activities like walking or getting in and out of a chair very painful and difficult. If you are facing the same difficulties despite medications, changes in lifestyle and physiotherapy, it is time to go in for a hip replacement.

Causes of Hip Pain
The most common cause of long-term hip pain and disability is arthritis.

  1. Osteoarthritis- This is an age-related problem that causes wear and tear of the hip joint. Usually, older people get this arthritis. What happens in osteoarthritis is that the cartilage cushioning the bones of the hip wears away. The bones then rub against each other leading to hip pain and stiffness.
  2. Rheumatoid arthritis- RA is an autoimmune disease in which the synovial membrane in the hip joint becomes inflamed. This damages the cartilage causing pain and stiffness.
  3. Post-traumatic arthritis- This arthritis follows a hip injury or fracture. In this the cartilage may become damaged leading to hip pain and stiffness.
  4. Avascular necrosis- The lack of blood to the femoral head can cause the surface of the bone to collapse resulting in arthritis.
  5. Childhood hip disease may also require this surgery.

Hip replacement- How it’s done?
Hip replacement surgery is one of the safest operations in medicine. The hip is a ball –and –socket joint and in a total hip replacement the damaged bone and cartilage is removed and replaced with metal or plastic prosthetics. Hip replacement surgery is usually performed in a traditional manner or using a minimally-invasive technique. The main difference between the two lies in the size of the incision for surgery.

During standard hip replacement surgery, a patient is given general anesthesia. This is followed by a cut along the side of the hip after which the surgeon moves the muscles connected to the tip of the thighbone to expose the hip joint. Post this the ball portion of the hip joint is removed by sawing off the thigh bone. After this, the artificial joint is attached to the thighbone by using either cement or a special material. The surgeon then removes any damaged cartilage from the surface of the hipbone and attaches the replacement socket portion to the hip bone. Most hip replacement surgeries today are performed using the standard technique however; the minimally-invasive technique, is getting more and more popular. In this, the surgeon makes one to two cuts from 2 to 5 inches long and then carries out the same steps like in the standard hip replacement surgery. The patient may have to stay in hospital for 3-6 days post a hip replacement surgery and has to undergo long-term physiotherapy to attain normal use of the hip joint.

I was a sports man I had a operation in my leg my ligament has broken now can I start playing again?

MS - Orthopaedics, MBBS
Orthopedist, Pune
Hello lybrate-user, according to your history and as a sportsperson that operation would most likely be an acl reconstruction. Playing again will depend upon your rehabilitation protocol and the strength of the fixation of ligament which your operating surgeon will most likely know the best. Generally after around one month of physiotherapy low impact activities and around six months for starting of high impact activity like running, sprinting, playing.

Arthritis(Joint Pain) ? Ayurvedic View
According to ayurveda there are three doshas- vata, pitta, kapha. When these are in a balanced state we are healthy. Any imbalance in these three doshas leads to diseases in the body.

Vata resides in the colon and colon is therefore primarily responsible for all the vata disorders.

Vata imbalance in the skeletal system (asthi dhatu) causes osteoarthritis.
Vata imbalance with toxins (ama) causes rheumatoid arthritis.
Vata imbalance in blood (rakta dhatu) entering into bony tissue causes gout.

Massage with sesame oil to reduce the vata.

Digestion must be improved so that no further ama is produced. Fasting is very beneficial for digesting the ama. Duration of fasting depends upon the strength of the person, place and season.

Diet-
Consume the foods which are easily digestible and do not make wind or gas.
Vegetable juices and juice of carrot, beetroot and cucumber mixed together is also beneficial.
Green salad, fruits like apples, oranges, grapes and papaya can be taken.
Use of cumin, coriander, ginger, asfoetida, garlic, fennel and turmeric is also helpful.

Dont's-
Avoid eating hot, spicy and fried foods, sweets, wind forming foods like cabbage, cauliflower, spinach and potatoes should be prohibited.
Avoid taking too much tea, coffee, alcohol, white sugar, yogurt, chocolate and cocoa, excessive smoking.
Sleeping during day time, staying up late in the night and mental tensions like worry, anxiety, fear, stress and grief etc, should be given up.

Here are some tips for how to handle your back pain: -while sleeping, try lying in a curled-up, fetal position with a pillow between your legs. If you usually sleep on your back, place a pillow or rolled towel under your knees to relieve pressure. -find a mattress with back support. A good mattress should provide support for the natural curves and alignment of the spine. Medium-firm mattresses usually provide more back pain relief than firm mattresses. -do not perform activities that involve heavy lifting or twisting of your back -avoid bending forward and sitting in a place for long time -stretching and strengthening exercises are important. -a physiotherapist can tell you when to begin stretching and strengthening exercises and how to do them. Best wishes.

Arthritis - How Acupuncture is used to Treat it

Bachelor in TCM, Diploma in Acupuncture, Diploma in Naturopathy & Yogic Science (DNYS)
Acupuncturist, Mumbai
Arthritis - How Acupuncture is used to Treat it

Acupuncture is a Chinese method of treatment which aims at maintaining the energy balance in the body. The procedure involves stimulating the energy channels of the body by using fine steel needles. It enhances the production of endorphins (pain controlling hormones) in the body, therefore less pain.

Signs you are suffering from arthritis

Arthritis is a disorder that is characterized by inflammation and swelling in the joints. The symptoms of arthritis are tenderness in the joints, fatigue, fever and stiffness in the joints that worsen in the morning. Initially, the smaller joints in the body are affected; it then progresses to joints in the elbows, wrists and the knees.

Causes behind it

Arthritis is caused when the immune system starts attacking the membrane surrounding the joints. Inflammation occurs, resulting in destruction of the bone and cartilage. It causes the ligaments and the tendons holding the joint to weaken. The joints then starts to lose their original alignment and shape.

Dietary guidelines you must follow

You need to follow dietary guidelines such as avoiding refined and processed foods as they can contribute to the inflammation of the joints. You also need to steer clear of foods which may trigger a reaction from your immune system. Mild exercises to strengthen the joints may be performed to improve mobility.

Acupuncture and arthritis

The procedure begins with the practitioner applying alcohol to the affected spot and tapping the area with a needle. The disorder determines the number of needles required, the spots of insertion and the depth to which they have to be inserted. The duration for which the needles are kept in the body ranges from a few minutes to an hour. In order to enhance the effect of the needles, they may be heated or energized electrically. You may experience a tingling sensation in case electricity is used.

Acupuncture is a holistic method of treatment that has no side effects. It does not involve chemicals; so it is safe to use. It will not cure the disorder, but can reduce the pain to a large extent. One of the added benefits of this procedure is that no matter, which part of the body is subjected to this process, it does help calm your mind down. If you wish to discuss about any specific problem, you can consult a acupuncturist and ask a free question.
