Dear Sir/Madam, Since last 1 week at tip of my middle finger near nail, used to pain severely whenever I stretch or move the hand either to pick anything or just as. The pain is so much as if someone had pluck the nail or feels as electric shocks. Please suggest what to do or if it is sign of any disease Thanks.

It can be paronychia !! It is fungal or bacterial infection of nail fold. Go to a surgeon, If so he will prescribe antibiotics or if there is pus in nail bed, he will put a small incision and drain the pus. From your data I can infer this !

We are planning to buy a weight loss exercise machine for reducing the fat. Please help us to choose the right machine considering my mother (the knee pain patient). She is about 5 ft height and 97 kg weight. My choice is electronic treadmill. Will it be a right choice? Please share your suggestions.

To lose weight with osteoarthritic knee or injured knee is difficult and should be done keeping in mind that injuries to knee should not be aggravated by exercises. Treadmill, squats, jogging and running are high impact workouts on knee and should be avoided. Following exercises can be done with low impact on knees. 1. Swimming doesnt exert stress on knee joint and burns fat if done at a stretch for 20 mins. 2. Strength training in form of bench press, shoulder press and abdominal exercises, triceps and biceps exercises can be done. 3. Walking on soft ground or soil as in garden can be done. In fact warm up before exercises in such patients can be done by a gentle walk. Avoid treadmill walk. 4. For home exercise machines, you can consider options like rowing machines, stationary cycle, elliptical machines. When using a cycle do not use on steep slopes or modes. 5. Along with exercise also use dietary modification in form of low calorie, protein rich diet to lose weight.

I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias ( electrical shock like abnormal sensations) or any weakness in limbs. It is not osteoporosis . What you have is most probably osteoarthritis. The diagnosis needs to confirmed. If in fact you have osteoarthritis then you need to be assessed for comorbidities and needs to be put on an intensive exercise program with control of pain using safe pain modfiying medications instead of pain killers. You appear to have torn a ligament (glenoid labrum) in your shoulder. You need to get an MRI done for your shoulder. Considering your young age, you need key hole surgery for your shoulder (Arthroscopic Bankart repair) to prevent further dislocations and further damage.

I got my ankle injured in February last year n in first MRI it said grade 1 tear and in the 2nd MRI after six months. Everything came normal but my ankle still pains. It's been over an year to my injury now n I am not getting any respite from pain so I also took two tricort injection in a month the pain still persist can you help.

Neck pain may be because of cervical spondylosis, Ankylosing spondylitis, slip disc ,Myofascial syndrome etc. I need some investigation like TLC, DLC,ESR, CRP, HLA B-27, X_Ray Cervical spine (AP/Lat) and latter on MRI Cervical spine. I am 30 Year old women having spine (disc) issue. According to my MRI report “Posterior and left paracentral disc extrusion at L5-S1 level superior migration causing compression on left existing nerve”. Due to this the toes of my left foot has lost power restricting dorsiflexion. There is no back pain and I am able to move and walk freely without any issue. The numbness and sensation have shown recovery after 15 days of physiotherapy. My doctor has advised for surgery but I am reluctant to go for it. My two questions are 1) Whether it is possible to get recovery without surgery? If yes what precautions should be taken? 2) I fear further damage due to this nerve compression. Whether there are chances of further damage due to this if I don’t go for surgery?

Hello madam, The symptoms described by you and the MRI report corelate. It means that you have significant disc problem. It is considered as a strong indicator of surgery if you have weakness of muscle. Generally for disc related problems conservative treatment by medication and physiotherapy will work, but when muscle weakness is present, then there is high risk of progression and chances of recovery of muscle power cannot be predicted. If you recover from this problem without surgery it will be great, but have to be lucky. But if it gets worse then you may have to be only sorry. Unfortunately no test or no Surgeon can predict if someone in your position will recover or not. Ifeel the doctor who has seen you in person has more information about you than me, so my advice would be to get your self treated as per his decision. Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Your injury was around 6 months old and since then all tissues must have healed by now. Did you do a x ray after your initial injury to rule out a bony issue? my advise is: - do intensive ice packs 3-4 times in a day - take a short course of anti inflammatory medication for 5-7 days - start physical therapy like stretching exercises for your plantar fascia initially under supervision of a physical therapist - modify your footwear and try to wear well cushioned sports shoes or leather shoes with insoles to give extra protection to your heels. Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are many causes of pain in lower back region. It can be due to faulty posture, related to weak muscles, issues with intervertebral disc, narrowing of spinal canal, any bony instability besides many other, so accurate diagnosis is the key to proper management of the condition. You have mentioned early osteoarthritic changes which is surprising at this stage but I would like to see the investigations which you have done which mentions these changes. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation.
