I'm 28 years old male having pain in just at the lower portion of knee. I made x-ray but there is nothing abnormal in that particular area. Identify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. Inform after 10 days for further guidance.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned backache as your problem. If you have only localized backache with no other associated symptoms like radiating leg pain, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied 3-4 times in a day - analgesic spray for local application like dynapar qps spray can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc backache can be due to multiple reasons like degenerative disc issue, weak back muscles, mechanical back pains, spinal stenosis etc. If you do not improve in 2-3 weeks with this conservative management, we will need to evaluate you further in form of dynamic x rays to look for any instability as a cause for pain and some blood investigations to rule out any deficiencies etc. Do not hesitate to contact me if you need any further assistance. All About Knee Replacement

Knee replacement is a surgery wherein an artificial joint is used to replace a diseased, damaged or worn out knee. This surgery is common among people who fall in the age group of 60-80, but recent trends seem to suggest that younger people are opting for this surgery as well. The lifetime of the artificial knee joint is around 20 years, provided the knee is well cared for.

Why do you need Knee Replacement?

Most common reason is “high grade osteoarthritis” due to wear and tear of the knee joint. The pre-hospital study of the Knee joint is mandatory and would decide what kind of Replacement is suitable to the patient. If there is diabetes or hypertension associated with this, then it should be controlled well before undergoing surgery. Hemoglobin of at least 10 gm% is required.

If the mobility in your knee joint is reduced leading to impaired functioning of the knee joint, then you might need a knee replacement surgery. You may experience pain while walking, sitting and, in some cases, resting as well.

Some of the common reasons why you may opt for this particular surgery are:

  1. Gout, where, small crystals are formed inside the joint.
  2. Rheumatoid Arthritis, an autoimmune disorder, wherein the immune system of the body attacks the body’s healthy tissues.
  3. Hemophilia, wherein, the blood ceases to clot normally.
  4. Injuries to the knee
  5. Disorders that cause unusual bone growth (bone dysplasias)
  6. Death of bone in the knee joint following blood supply problems (avascular necrosis)
  7. Knee deformity with pain and loss of cartilage
  8. Unusual growth of bones in the knee joint

Knee replacement surgery is classified into:

1. Partial Knee Replacement: In this surgery, only one part of the joint is replaced.

2. Total Knee Replacement: Total knee replacement surgery involves replacement of both sides of the knee joint.

The usual hospital stay period is around 2-3 days after the surgery is completed. Initially, you will require the help of crutches to walk for at least 2 months. You may also be asked to do gentle knee strengthening exercises. It may take up to two years to recover completely from a knee replacement surgery. If you wish to discuss about any specific problem, you can consult a orthopedist.

After a good sleep getting up in the morning I get usually pains in shoulder joints & in the back for about a minute or so till I walk a few steps which will become almost normal. Regularly I am going for walk 4 kms. And busy always, never have any sleep during the day. Why is it due to?

It is very likely to be a deficiency of calcium & vitamin d in the body. Avoid direct exposure to cold air / cold water. Massage gently the affected parts with any good ayurvedic oil on a routine basis.

I'm 33 year old. Since last 1-2 years neither i'm able to sit or stand for even few minutes. I feel my legs and back have become too weak. If I walk, my legs start paining & when I sit on the ground, it takes some time to get up and straighten my back. I'm unable to sit upright on the chair as well. My job for last 10 years has been operating the computer, in night shifts. Please let me know at the earliest what my medical condition is & what I need to do to overcome this problem. Thank you.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Because of your previous history and lifestyle I will advise you conservative management initially failing which we will have to investigate you in a greater detail. For back pain: if you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. For leg pain: actually pain at calf/leg region can be due to many reasons, commonly it is muscular in nature but it can also be due to tendons, nerves, blood vessels etc. My advise to you for initial 2-3 weeks will be: - ice packs application intensively with elevation of your limbs at night - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning once pain has subsided - slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - we will get some blood tests for you - serum vitamin d and serum potassium levels, esr and crp once you have all these reports then you can contact me as I can advise you for proper supplementation do not hesitate to contact me if you need any further assistance. Hello,

I constantly try to educate patients on proper posture, but because people (myself included) spend so much time driving and sitting for prolonged periods of time. Driving can mean different for different people. It may be a way of leisure for some or as an everyday activity to commute to office. Driving for long time has always accompanied with some or the other musculoskeletal problems. It become a leading cause of back pain to our fast pace life.The problem is treated but the root cause of it is never looked after or we just don't bother about it much..

Why we get the pain or associated problems when we drive for long time?

Poor posture while driving can produce a repetitive load to the tissues that causes sustained stress. Daily prolonged drives apply loads to the spine that can cause viscoelastic tissues to slowly deform and creep. Simply staying in the car seat with poor posture long enough will eventually ensure damage. Sustained load and resultant deformation/creep causes a progressive reduction in tissue strength.With your hands in the proper position on the steering wheel, the thoracic spine, scapula and shoulder joint have a better chance of remaining in neutral. usually, we hold steering wheel with one hand and lean to other side.due to leaning hip bone( iliac crest) and 12th rib approximate; this will contribute to shortness (tightness) of the quadratus lumborum or latissimus dorsi

If the pelvis is rotated, it will contribute to lumbar or SI pathology and overactivity of the TFL(muscle). If the knees are even slightly superior to the hips, then there will be hip flexors muscles over activity or shortness, and hip extensor inhibition or weakness. Prolonged sitting on the glutes can cause poor circulation and lack of oxygen to the tissue, contributing to inhibition.

A person driving with a clutch who has poor or limited ankle dorsiflexion (like after ankle fracture) can perpetuate overly active hip flexors because they will pull the foot off the clutch using the hip flexors, instead of dorsiflexion at the ankle.

Some Do’s:
1) Select a car that matches your height and suits your work.
2) Change posture as much as possible take frequent Microbreaks ,recommended atleast 15 minutes, every 2 hours
3) Place hand on steering wheels so that elbows are close to your side and not so high that you have to reach up for the wheel.
4) Place hands at 3 o’clock or 9 o’clock position this will reduce the amount of strain on shoulders and put hands in safer position.
5) It is advisable to keep two hands in steering wheel except while shifting gears. Change hand posture frequently to reduce fatigue and improve circulation.
6) Learn Microbreaks: An example of a "microbreak" for driving in the car is squeezing the shoulder blades together for 20-30 seconds and repeating 2-3 times. Repeating anterior and posterior pelvic tilts and lateral tilts for a minute or two is sometimes helpful. Other microbreaks for driving include cervical range of motion, and shoulder retraction with elevation and depression.
Some Dont’s:
1) Avoid lifting after driving long hours because your muscles are tired your ligaments are stretched and disc are at risk of injury, give yourself some time to stretch and relax.
2) The death grip posture, the one arm cool dude” the Wrist resting at 12 o’clock position on steering wheel and fingers flop over the top, arm straight out in front and one arm propped on your window.


The longest journey begins with a right sitting posture, not with a turn of the ignition key !!!! dive safe

Dear you must be already taking some medicines for diabetes and hypertension which probably keeps these under control. For the knee pain the temporary measure is painkillers followed by knee replacement. It is a conventional treatment. But there is a natural treatment which may possibly cure these problems.

Sweating after jogging is normal. If worried about heart get checked and have proofs. Do not go by hearsay of google which is generalized. Get ecg & halter check up done, blood tests for cbc, sugar & total lipid profile. If reports are normal, carry on with your life otherwise do get back here on forum.

Kindly show me photograph of your foot. Anyway. You may sleep on a hard bed with soft bedding on it. Kindly take biod3 max 1 tab dailyx10 paracetamol 250mg od & sos x5days make sure you are not allergic to any of the medicines you are going to take do not ignore it. It could be beginning of a serious problem. If no relief in 4_5 days, then contact me again.

Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily.

It's a progressive disease but we can slow it down by doing normal streatching exercises do not go for though exercises with weights, need to keep an eye on the posture walking and other ADLs. Do let me know if you need any help or want more detail.

Keep your leg raised while sitting or lying quadriceps strengthening exercises hamstring stretching calf muscles stretching contrast bath.
