Pediatrician,
Studies of the rotavirus vaccine have shown that it can prevent about 74% of rotavirus infections. More importantly, it can prevent approximately 98% of severe infections and 96% of hospitalizations from rotavirus. Please there are two brands of the rotavirus vaccine -- rotateq (rv5) and rotarix (rv1). Both vaccines are given orally, not as a shot. The only difference is the number of doses that need to be given. Please with rotateq, three doses are required. They should be given at ages 2 months, 4 months, and 6 months. Rotarix only requires two doses -- at 2 months and 4 months. Please the vaccine may be given at the same time as other vaccines, and the american academy of pediatrics recommends that the rotavirus vaccine be included as part of the routine immunizations given to infants. Please so if you give your child is protected against rotaviral diarrhoea or he may or may not get infected. So it is not given as compulsorily or routinely in our national immunisation schedule. It is an optional vaccine.

My 3 months baby hasn't sleeping properly at night.

Sometimes a 3 month baby is still yet to develop a regular day and night pattern. Most often they should settle by the time they are four months old . If you are breast feeding, the baby settles down after a feed and a burp. Try to create a semblance of darkness and quiteness during the night and use diapers so that he does not awaken every time he passes urine. Remember try to get sleep when the baby sleeps, to keep your prolactin levels high to help breast milk secretion. A calm mother usually has a calm baby, and so try not to reflect your anxiety on to the baby. Kangaroo care of the baby will also help which even men can assist.

HEALTHY FOOD INTAKE FOR STRENUOUS STUDY DAYS

Exams are here again. As always we tend to neglect our food in the process of concentrating on studies. But we forget that to concentrate on studies we have to concentrate on taking a healthy diet.

Here are a few diet tips that will help you balance your study as well as diet.

1. Eat small and nourishing meals every three hours, instead of just 2-3 heavy meals. The brain needs a constant supply of energy and a heavy meal will divert the blood supply to digest the heavy meal.

2. Your daily diet must include 2-3 fresh, seasonal fruits. Start your day with a boost of fruit energy. Have a banana with a tablespoon of honey as the first meal of the day. Fruits provide you not only with fiber, vitamins and minerals but also simple and complex carbohydrates for short and long duration of energy supply when studying. The fiber will also help to keep the bowel movement clear.

3. A good breakfast is a must for anyone. The breakfast must contain foods with complex carbohydrates like flakes, oats, rawa (semolina), daliya (wheat cracks), poha (rice flakes) or pav with vegetables and a protein source such as eggs, milk, pulse, sprouts or paneer.

4. Nuts and dry fruits like almonds, walnuts, and dates provide nutrients which enhance brain power and concentration and also provide an extended source of energy supply.

5. Include potassium-rich coconut water, citrus fruits/ juices and soups in your diet.

6. Green leafy vegetables, pulses, and nuts will also provide vitamins and minerals to help avoid anemia. (Anemia is a common problem causing difficulty in concentration.)

7. Including milk as a bedtime meal and curd/lassi/buttermilk after lunch or as a mid-meal assures a supply of Vitamin B12 too.

8. Drink plenty of water to keep your body hydrated. Always keep a glass of water beside you to remember to take sips of water every 15 minutes.

9. Basic exercise like stretching, walking or yoga with meditation will help to reduce negative stress and also help you to get good sleep.

10. Avoid junk foods as they will provide you no nutrition.

11. Avoid stimulants like tea, coffee or colas as they may seem to perk you up but the energy does not sustain for long. Too much may even cause bloating or acidity.

12. Avoid aerated drinks as they are full of sugar and may cause a rapid rise in blood sugar levels causing a sudden rise and drop in sugar levels.

13. Chocolates may seem to may you feel good but the sudden rise and fall of sugar it causes in the blood can lead to trembling nervousness and panic.

14. Have your dinner if possible a little earlier than 9-10pm.

15. Keep about 2 hours between dinner and your bedtime.

16. Have a sound sleep for at least 7-8 hours daily.

Remember your body and mind need the fuel to help you perform the best. So, nourish it and take care of it and it will do the same for you.

Study Smarter and Not Harder!

There are few things I hear over and over again from students:

  1. How they are studying for so many in a day
  2. How they do not have even time for anything else in their life due to study pressure
  3. How they are sleep deprived
  4. How much they are stressed
  5. And finally, after all the sacrifices and hard work, their exam performance is not what they expect.

Here are few tips to study smart, improve your grades and yet have a life!
We all know that 2 hours of focused study will be much more productive than 4 hours of distracted study. And, the good news is that concentration is a mental skill that you can develop and improve. There are two ways to improve your concentration:

A. First, work on raising your brain's natural ability to concentrate or in other words, learn to increase your attention span.
B. Second, adjust the environment around you to make concentrating easier.

Improving your concentration takes a little time and effort, but it is worth it. In my personal experience with students, I have seen noticeable improvement in relatively short time.

A. Ideas for daily concentration boosting habits include

  1. Mindfulness meditation: Many studies confirm that 20 minutes of meditation or more per day improves concentration and attention span. Mindfulness meditation, where one focuses on one's breathing,  is one of the simplest way of meditation. Start with ten minutes in the morning and ten minutes before bed. To begin with you may try a guided meditation. One can easily find audios for guided meditation online.
  2. Proper sleep: I am sorry to say this. Pulling all nighters will not help with your grade. Unless you are getting sufficient restful sleep, you are not going to be as mentally focused as you could be to study as well as answer questions in exams.
  3. Food for your Brain: The more you use certain skills, the more they are reinforced in the brain. So it makes sense that playing concentration games and games that require you to focus will improve your concentration ability. You need to be consistent in this. Play for about 10 to 20 minutes each day. Have fun, and get the benefits but remember not to spend more than 20 minutes a day. Your goal is to improve your concentration and not just keep scoring higher in the game! Try Lumosity and use only free version this will help you restrict your daily play time to 20 minutes and have maximum benefit.
  4. Eat healthier: Your brain needs the proper nutrients to allow you to focus. Eat more vegetables and fruits and avoid consumption of junk food as much as possible. Instead you could keep some nuts such as walnuts, almond or peanuts as in between munches. Lot of research has now proven that sugar is very harmful for our health. Reduce sugar and refined white flour in your diet as much as you could.  
  5. Exercise: It is helpful in both reducing stress and increasing energy and focus. At least 20 minutes a day would go a long way. Going for a short jog of 20 minutes will make next 4 to 5 hours of study very efficient.

B. Optimize your mental state & environment

In addition to building the habits described above into your daily routine; also adjust your environment and your current mental state to improve your concentration when studying.

  1. Create a study space: Your brain loves routine. Create a place where all you do is study. An obvious choice is a secluded desk of some kind, but the trick is to make sure you only study in that spot. Studying in bed is a bad habit, for example, because your body is trained to want to sleep once you get into bed.
  2. Remove distractions: Turn off the ringer on your phone and take other steps to prevent interruptions. Don't study with the television on or with radio playing. Some studies have shown that soft instrumental music (no vocals) can help improve your concentration.
  3. Reduce background noises: If you have to study or work in an environment where it is too loud (classroom or open space) or too quiet (such as at home or at a library) you can get easily distracted by outside influences. With the right amount of background noise you can actually block out distractions and enhance your creative thinking.
  4. Use timer: Set a time limit when you need to study new material. For example, let's say you want to read one chapter in a book (and remember it). Decide in advance that you can have 45 minutes to read the chapter, and 15 minutes to review it. Set a timer to keep yourself honest, then pace yourself to keep within the allotted time.
  5. Stay motivated: If you see studying as burden, it's hard to concentrate. One way to stay motivated is to set up a reward system. Tell yourself you have to earn that hour of watching your favorite show later in the evening by first completing 2-3 hours of intense studying. That way, even if the material is dry, you have the reward to look forward to.
  6. Take breaks: Take a break every two hours. You mental energy will begin to decline after a long period of study. So every two hours or so, take a ten minute break. Walk around, eat a light snack, or just stare at the wall to relax your mind.

Concentrating is the first step to learning anything new. It only makes sense that if you improve your concentration, your memory will improve also.

He may still have cold though externally he looks fine and this irritation is the cause of crying . A 17 day old bay should be given only breast feed

If it is non-alcoholic, then there is nothing to worry about. It will not affect the baby much. But it will better if she avoids it until her breastfeeding time is over.

Why does my son suffers from diarrhea and nausea after taking augmentin. He was recently given it for 7 days and on 9th day he started vomit and then complained of stomach pain and is still suffer from it. Last time when he took the same medicine he did suffer from loose motions and doctor advised enterogermina. So this time also I am giving enterogermina. Is it right.

You should tell your doctor about this medicine to your doctor so that he does not prescribe it again, because he will have loose motions with this medicine every time throughout his life

Ideal wt for this age is 14 kg. U may increase number of feeds including ghee cooked nutritious diet rich in protein of his choice n at his liberty. No force feeding.

Sad to hear tht dear parent. But equally strange is tht with such a diagnosis, i am sure your child is under care of sone neurologist or specialist, so rasimg this question in an open forum like this and that too with a general pediatrician, simply reflects the lack of ur faith in the treating team. Then how can you rely on an answer by sm1whim u dont kno and who has nt even seen ur child. My suggestion would be tostic to the treatimg team and do not change teams also too fequently as such medical issues require a lot of patience from both doctors and parents side. Wishing you a speedy recovery. Dr Vipul.

My 3 an Half Year old daughter won't talk to class teacher even she didn't write what ever teacher write on blackboard. Which is making problem to the teacher? Outside of school she is write, talk to others. At home she do her homework but most times we have to say many times for it to write but its ok as she is just 3. Presently many times she became angry and started shouting or speaking loudly and not listening to us. What should we be doing now so that when she go to school she can be more open to speak up? Like to present some speech in front of many students of her same age. As I saw other child doing best as they give speech 2-3 lines on mic in front of class but mine just standing silently. What to do? How we come to make her talk in class and all I hope you would understand what I mean to saw? Please help.

Dear Lybrate user. I can understand. Your daughter is just 3 and half years. Please consider her age. Children do act according to what they are trained for. You must understand you can easily train your child to do whatever you want. For training you have to first understand the child and her needs. You can use trial and error method or reward punishment method. If you don't have time, a psychologist should help you. I suggest online behaviour therapy for your child. Take care.

My son is 3 years & 11 months old he always repeat what ever we say same thing repeat he always like to watch rhymes he always sings rhymes in home we generally speak Telugu and English but if we say something will not focus at all and is there any problem and required speech therapy and we feel that he is not able to understand how to answer. At this age child is quote competent to express himself. It appears he lacks in this field. Alone speech therapy is not helpful. His wt 13 lbs is very-very low even if I take it as 13 kg, he is under wt. He requires thorough examination.

Cystic Fibrosis - Can it be Life Threatening?

Does your baby’s skin taste too salty when you kiss him or her? Did he or she not poop after being born? These symptoms indicate cystic fibrosis in your baby, which is a condition that changes the way your body makes sweat and mucus. It also affects the functioning of the lungs and digestive system. The condition occurs from a flawed gene. If you have cystic fibroids, your mucus may be too thick or your sweat can be too salty. Heavy mucus clogging in your lungs may affect your ease of breathing. This is a serious and life-threatening disease, but it affects different people in different ways.

Causes
The gene known as CFTR is present in everybody. It makes a protein which controls the amount of salt and water that are moved in and out of the cells of the body. In case of cystic fibroids, the CTFR gene is broken, which occurs if you inherit two bad copies of the gene. A bad CFTR gene may lead your cells to make the wrong amount of water or salt. This makes the body incapable of getting sufficient oxygen and nutrients, as the body loses them via sweating.

Symptoms
Apart from the primary symptoms, several other symptoms are also likely due to cystic fibroids. The common symptoms which your child may experience are as follows:

  1. Coughing up thick mucus, wheezing and shortness of breath
  2. Sinus infections, pneumonia, or very frequent bronchitis
  3. Development of polyps in the nose
  4. Bulky, oily and foul-smelling stool
  5. Low bone density, widening and rounding of the fingertips and weight loss
  6. In later life, cystic fibroids may cause liver disease, gallstones or pancreatitis.

Treatment
There is no specific treatment for cystic fibroids. Several treatments are followed for relieving the symptoms and help patients feel better, breathe easier and have lesser infections or stomach problems.

  1. Airway clearance technique can help you with breathing as the mucus from the lungs is cleared in the process. There are several ways to undertake airway clearance and special devices can be used for assistance.
  2. Several inhaled drugs such as salt solutions, steroids and antibiotics are used in thinning the mucus, clearing the lungs and fighting infections.
  3. Several medicines are used for fighting the inflammation and reducing swelling in the lungs.
  4. In case your pancreas are not working properly, you can take enzyme pills for digesting proteins and fats for absorbing more nutrients from the food you take.
  5. Vitamin A, E and D supplements provide the nutrients, which your intestine cannot absorb.

In case you experience the symptoms of cystic fibroids, you should consult a doctor immediately. This will allow you to go for an early diagnosis and effective treatment.

Start giving her foods in semisolid or solid form as all children do at 3-4 hours intrerval. Discourage for breast or bottle feed. Toilet training is not related to dietary modifications. Now is the time nervous system is developed to eastablish toilet control
