I am a 42-year old male with height 5'5" and suffering from Ankylosing Spondylitis (HLA-B27 positive) for the last 19 years. I'm soon going to have a total/partial hip replacement but before that I'm buying a new car for myself. Please recommend the type of car (hatchback/sedan/SUV) which will be suited for someone with replaced hips. Is the height of the seat from the ground level important for a replaced hipped person and will it be safe to drive after the replacement? Thank you.

Hello lybrate-user, Whatever car you buy, it is more important to know the trunk, hip knee and ankle angulation to sit after hip replacement. Avoid driving a car immediately after surgery. Your trunk should be straight with lumbar lordosis maintained properly with a back support. Hip should be in 90 degree, knee in 110 degrees and foot resting well over the break and accelerator. Do not hang the ankle joint if your height is small.

According to ayurveda, for treatmnet of obesity, you firstly stop causative factors from your life style. Like drinking too much cold water, eat too much sweets, day sleep, water after meal, lack of exercise, curd, menda etc. I will need your detail history. Till you can follow this * start drinking hot water * stop day sleep * start any exercise in morning, like suryanamaskar, walking, joging, sports * drink juice of turmeric n ginger with honey * take jau barley, instead of ghenhu wheat. * use cow ghee n milk only * boild rise in open pot, don't use cocker. Withdraw water if rice. Don't sit in one position for more than 1 hour. Take rest for 5 min. Walk little & start work. Take hot steam twice a day.

Core strengthening exercises- straight leg raised with toes turned outward, repeat 10 times, twice a day. Back stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day.

Quadriceps strengthening exercises- Quad Clenches: Lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, Hold for 3 secs. Repeat 10 times twice a day. Short Arcs: Lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight Leg Raise: Lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise. Hamstring stretching Towel Hamstring Stretch Lie on the back, supporting the thigh with the hand or with a towel wrapped around it.. Slowly straighten the knee until a stretch is felt in the back of the thigh, trying to get the bottom of the foot to face the ceiling, one leg at a time. Hold the position initially for 10 seconds, and gradually work up to 20 to 30 seconds. Seated Hamstring Stretch While sitting at the edge of a chair, straighten one leg in front of the body with the heel on the floor. Then, sit up straight and try pushing the navel towards the thigh without leaning the trunk of the body forwards. Hold this stretch for 30 seconds Repeat 3 times for each leg.

Hi I can understand your problem. # relax & stay positive. # take hot and cold foementation. # learn posture care exercise. # use lumber support. # take break every 20 minutes. # do strengthening exercise of back and core muscles not all back pains are same, you need specific treatment plan according to your symptoms. Hello, I met with an accident on 25 august 2016 my forefoot bone broke down right foot. Now after getting plaster for 6 weeks bone is still not fully joined. I am worried for it, my plaster was removed on 7 October. I feel pain while walking I am unable to put body weight on my foot. Can anyone help me or guide me in this? Can my bone break down again if I walk or there is no such type of risk? Can I resume my activities or driving?

Hi, first of all do you went for X-ray recently if in cream it shows union of joints properly than you need to do wax bath, strengthning exercises and if you feel restriction of movement than some streatching exercises also. Do let me know if you have any doubt.

It needs to be investigated. Investigations advised - hb, tlc, dlc, esr, sugar f & pp, uric acid, vit d3 urine r/e x ray of the affected part. Anyway you may try- sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, inform.

Most probably you are suffering from oa of knee. Knee surgery-total knee replacement (tkr) is quite safe & gives uniformly good results, in our hands. We have well equipped center in delhi. Do ask for detailed treatment plan. Surgery should be done when patient is still active & will be able to take benefit of surgery. Do not ignore. It could be beginning of a serious problem. Kindly show me x- rays before a final decision can be taken. You will be thoroughly investigated before surgery is under taken. Risk in surgery is less than the risk in crossing road.

It is a misconception that diabetes medicines cause kidney damage. On the contrary not taking medicines regularly leading to high blood sugar, or a constantly fluctuating blood sugar level causes urine and kidney infections. Other cause of kidney damage could be stones or high blood pressure.

What are the reasons for low bone density? and also what we should do to get rid of this problem?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Low bone density has many causes: it is common in females after menopause because of hormonal changes, in both sexes bone density decreases with increasing age because bone degradation is more as compared to formation. It is also secondary to many diseases like diabetes, thyroid disorders and many more smokers have low density, so there are various reasons for it. Medically it is advisable to keep yourself physically fit, so regular exercises and walks is important as it leads to good bone turnover and keeps bones healthy, adequate calcium and vitamin d in body is important for bone metabolism. Also diets rich in calcium like milk and milk products, fresh fruits and vegetables etc should be taken. Hi I'm 36 years male suffering from ankylosing spondylitis since year 2007. Now I'm taking biological drugs i.e. *(Adalimumab) Exemptia* Injection for last 6 months (Took Exemptia 40 mg 6 injections and now taking Exemptia 20 mg (already completed 4 doses of 20 mg). I got married in May 2017 and now we want are planning for a baby. So is it right that we can conceive now? As I have a doubt that as I'm taking tnf blocker i.e Adalimumab so may affect the unborn fetus? So should we conceive now or wait?& also does Adalimumab damages the sperm?

No significant difference of sperm quality between healthy controls and anti-TNF treated patients with SpA, sperm abnormalities were also found in 102 healthy controls Sperm abnormalities are a common finding in healthy men, they are more pronounced in patients with active SpA. The sperm quality of patients with SpA with inactive disease receiving long-term TNF would like to advice you to under went IuI procedure to conceive.
