How Diabetes Affects the Nervous System

MBBS, Diploma in Diabetology, DDM, CCACCD
Diabetologist, Mumbai
How Diabetes Affects the Nervous System

Diabetic neuropathy is a nerve disorder caused by diabetes. The high blood sugar from diabetes affects the nerves and over time increases a person's risk for nerve damage. Keeping blood sugar levels within the target range recommended by your doctor helps prevent diabetic neuropathy.

Types of Diabetic Neuropathy:
Diabetic neuropathy can be classified as Peripheral, Autonomic, Proximal, or Focal. Each affects different parts of the body in various ways...

Peripheral neuropathy, the most common type of diabetic neuropathy, causes pain or loss of feeling in the toes, feet, legs, and hands.

Autonomic neuropathy affects the nerves that control involuntary body functions such as digestion, bowel and bladder function, sexual response, and perspiration. It can also affect the nerves that serve the heart and control blood pressure, as well as nerves in the lungs and eyes. Autonomic neuropathy can also cause hypoglycemia unawareness, a condition in which people no longer experience the warning symptoms of low blood glucose levels.

Proximal neuropathy causes pain in the thighs, hips, arms, or buttocks and leads to weakness in the legs and hands, resulting in difficulty in walking, standing, picking up objects, buttoning your clothes, etc.

Focal neuropathy results in the sudden weakness of one nerve or a group of nerves, causing muscle weakness or pain. Any nerve in the body can be affected.

How Diabetes Causes Damage to the Nervous System?
There are several factors that are likely to contribute to nerve damage through diabetes...

  • High blood glucose, a condition associated with diabetes, causes chemical changes in nerves. These changes impair the nerves' ability to transmit signals.
  • High glucose levels affect many metabolic pathways in the nerves, leading to an accumulation of a sugar called sorbitol and depletion of a substance called myoinositol. These changes are the mechanism that causes nerve damage. Nitric oxide dilates blood vessels. In a person with diabetes, low levels of nitric oxide may lead to constriction of blood vessels supplying the nerve, contributing to nerve damage.
  • Presence of mechanical injury like carpal tunnel syndrome in a diabetic patient worsens its symptoms and prognosis
  • inherited traits increase susceptibility to nerve disease
  • lifestyle factors, such as smoking or alcohol use

Symptoms:

  1. Numbness, burning sensations, tingling, or pain in the toes, feet, legs, hands, arms, and fingers
  2. Either hypersensitivity to touch or insensitivity, even to hot and cold temperatures
  3. Weakness in muscles and loss of reflexes
  4. indigestion, nausea, or vomiting
  5. diarrhea or constipation
  6. dizziness or faintness due to a drop in blood pressure after standing or sitting up
  7. problems with urination
  8. Changes in gait and balance
  9. Injuries that are taking longer to heal and are more prone to infections

Prevent Diabetic Nerve Damage:
Keeping your blood sugar levels in your target range, set with your doctor, may help prevent nerve damage from ever developing. The best way to do this is by checking your blood sugar and adjusting your treatment. It is also important to get to and stay at a healthy weight by exercising and eating healthy foods.

If you wish to discuss about any specific problem, you can consult the doctor and ask a free question.

I am having more weight even my daily diet is not good even many days I didn't took lunch or dinner.

PGDD, RD, Bachelor of Home Science

I am having more weight even my daily diet is not good even many days I didn't took lunch or dinner.
Hye. Thanks for the query. There’s no overnight fix when it comes to healthy weight loss, but you can adjust your lifestyle to promote quick, natural weight loss for the benefit of your health and your appearance. Successful, long-term weight management needs to focus on more than the food you eat and the pounds you lose. It needs to focus on your overall health. Reshape your body and your lifestyle by adopting healthy habits and breaking unhealthy ones. Replace all processed foods in your diet with vegetables, fruits, whole grains, low-fat dairy, lean meats, eggs, legumes, nuts, seeds and seafood. Focus on the variety of whole, unprocessed foods you can eat instead of dwelling on the junk food you eliminated from your diet. Eat smaller portions at every meal and avoid going back for seconds. Take a smaller scoop of rice,  half of a piece of chicken instead of a whole piece and one less egg in your morning omelet, for example. Make time for breakfast every morning. Eating breakfast helps prevent overeating and is a habit of people who successfully lose weight. Pack to-go meals instead of eating convenience foods when you’re not home. Store foods like chopped fruits and vegetables or cooked grains, legumes, eggs and meats in your refrigerator so you always have something to grab when you’re in a rush. Buy fresh vegetables and fruits from the grocery store if you forget to bring healthful food from home. When you can’t avoid dining at a restaurant, opt for a salad as a starter, and always ask for the dressing on the side. Clear soups, lean meats, seafood and grain-based dishes like roti, whole grain bread are other good choices. Exercise for 30 to 60 minutes at least three times per week to build muscle and burn more calories. Pilates, strength training, aerobics, swimming, dancing and yoga or a sport like football are ideal choices. If you’re new to exercising, start with brisk walking and jogging. Adopt physically demanding hobbies like bicycling, hiking, jogging, power walking to promote faster weight loss. If you don’t have time for new hobbies, increase your physical activity with things like walking the dog, playing with the kids or jogging instead of driving to a nearby store or taking the stairs instead of the elevator. Weigh yourself weekly and record your weight. Adjust your diet and physical activity according to the progress you’re making.

Since the body area that needs utmost attention is the chest, one should try to keep it warm. Use fomentation bottle to warm up the chest, upper back, and back side of the neck. Keeping your feet in warm water is also helpful. It has been found by the researchers that drinking 3 glasses of orange juice everyday can bring down wheezing to 70%. Steam helps relieve the symptoms of all the respiratory tract ailments. Throw a towel on your head to inhale steam from a mini sauna, for 10 minutes. Steam will humidify your respiratory passages, easing your breathing, that will help in looking after the wheezing.

My father was suffering from fever so we had a common checkup but doctor said to have a test called hydronephrosis, after that he declared it and said to have a treatment for two days. So my intention it to know what treatment should have and will there any further problems in future. I wanted to know complete details about it. Please provide that information as soon as possible.

MBBS
General Physician, Faridabad
My father was suffering from fever so we had a common checkup but doctor said to have a test called hydronephrosis, a...
Treatment for hydronephrosis depends on what's causing the condition and how severe it is. In adults, the aims of treatment are to: remove the build-up of urine and relieve the pressure on your kidney(s) prevent permanent kidney damage treat the underlying cause Most people with hydronephrosis will have a procedure called catheterisation to drain the urine from their kidney(s). Depending on the underlying cause, medication or surgery may be needed afterwards to correct the problem. If the condition is severe or causing problems such as a urinary tract infection (UTI), you may to be treated soon after you're diagnosed. In less severe cases, it may be safe to delay treatment for a short period. Draining the urine The first stage in treating hydronephrosis is to drain the urine out of your kidneys. This will help ease your pain and prevent any further damage to your kidneys. A thin tube called a catheter may be inserted into your bladder through your urethra (the tube that carries urine out of the body) or directly into your kidney through a small cut in your skin. Read more about urinary catheterisation. In a few cases where one of the kidneys has already been severely damaged, it may be better to remove the affected kidney. Most people can function normally with just one working kidney, which won't usually have a significant effect on your health or lifestyle. Treating the underlying cause Once the pressure on your kidney(s) has been relieved, the cause of the build-up of urine may need to be treated. Some possible causes and their treatments are described below: kidney stones can be removed during an operation or broken up using sound waves – read more about treating kidney stones an enlarged prostate can be treated with medication or surgery to remove some of the prostate – read more about treating prostate enlargement narrowing of the ureter (the tube that runs from the kidney to the bladder) can be treated by inserting a hollow plastic tube called a stent, which allows urine to flow through the narrowed section – this can often be done without making cuts in your skin cancer that's causing hydronephrosis may be treated using chemotherapy, radiotherapy or surgery to remove the cancerous tissue If hydronephrosis occurs because you're pregnant, you won't usually need any treatment because the condition will pass within a few weeks of giving birth. In the meantime, catheters can be regularly used to drain urine from the kidney(s). Painkillers and antibiotics can also be given if you're in pain or develop a UTI.

Hello sir, I have a sinus problem due to which I am not able to breathe properly what can I do sir?

Certified Diabetes Educator, Registered Dietitian (RD), PGDD, Bachelor of Unani Medicine and Surgery (B.U.M.S), General Physician
Dietitian/Nutritionist, Mumbai
Do you have pain on cheek bones on pressing it? hot fomentation is really helpful in sinus besides drink fluids, such as water or juice. This will help dilute mucous secretions and promote drainage. Avoid beverages that contain caffeine or alcohol, as they can be dehydrating. Drinking alcohol can also worsen the swelling of the lining of the sinuses and nose. Moisturize your sinus cavities. Drape a towel over your head as you breathe in the vapor from a bowl of medium-hot water. Keep the vapor directed toward your face. Or take a hot shower, breathing in the warm, moist air. This will help ease pain and help mucus drain. Apply warm compresses to your face. Place warm, damp towels around your nose, cheeks and eyes to ease facial pain. Rinse out your nasal passages. Use a specially designed squeeze bottle (sinus rinse, others), saline canister or neti pot to rinse your nasal passages. This home remedy, called nasal lavage, can help clear your sinuses. If you make your own rinse, use water that's contaminant-free distilled, sterile, previously boiled and cooled, or filtered using a filter with an absolute pore size of 1 micron or smaller to make up the irrigation solution. Also be sure to rinse the irrigation device after each use with contaminant-free water, and leave open to air-dry. Sleep with your head elevated. This will help your sinuses drain, reducing congestion. Do reply back for medication prescriptions. Being also a general physician and registered dietitian, I prescribe both evidence based herbal as well as allopathic medicines complementing with dietary guidelines and home remedies carefully personalized for each individual patient.

Can Homeopathy Help Treat Diabetes?

B.H.M.S
Sexologist, Hyderabad
Can Homeopathy Help Treat Diabetes?

Diabetes mellitus is a chronic condition wherein the body doesn’t metabolise sugar properly. Insulin, a hormone produced by the pancreas, is responsible for carrying the sugar or glucose, from the bloodstream to the individual cells. When the pancreas fails to produce insulin, glucose builds up in your bloodstream and enters your urine. Homeopathy focuses on the overall health of the individual. Homeopathic medicines for diabetes include:
1. Abroma Augusta: This homeopathic medicine is best for diabetics who have weak muscles, increased appetites and frequent urination.
2. Phosphorus: If symptoms include weak vision, Phosphorus is the best remedy for it.
3. Syzygium Jambolanum: This is one of the best homeopathic remedies for diabetes mellitus. It acts efficiently and promptly in lowering sugar levels.
4. Phosphoric acid: If you feel exhausted or weak all the time, either physically or mentally, then Phosphoric acid is beneficial. Weak memory, forgetfulness and numb feet are also treated with Phosphoric acid.
5. Gymnema Sylvestre: Sometimes, diabetics drastically lose weight and have low energy levels. Gymnema Sylvestre is an excellent remedy for such symptoms.

Sometimes one or more homeopathic medicines may be combined to treat symptoms. Some common combinations include:
1. Lachesis, Arnica, Belladonna and Phosphorus are combined in a treatment to treat retinopathy, which is damage to the eyes caused by diabetes.
2. Serum Anguillae, Arsenic Album and Lycopodium are used together to treat kidney damage (nephropathy) in diabetics.
3. Helonias, Sulphur and Phosphoric acid are taken to deal with neuropathy or nerve problems such as numbness in feet and hands.
4. Syzygium Jambolanum is combined with Secale Cornutum to treat skin ulcers, which is a common problem among diabetics.
5. For people suffering from constipation due to diabetes, Natrum Sulph, Defloratum and Carlsbad are the best medicines.
6. To improve weak memory, Phosphoric acid, Nux Vom and Kali Phos are the best remedies for diabetics.
7. Sometimes diabetics complain of extreme, incapacitating weakness. To improve energy and boost overall health, Carbo Veg, Phosphoric acid, Phosphorus and Arsenic Album are recommended.

I got 3 children. The eldest daughter is suffered with fits during her childhood and become 50% mentally retarded. Other two are above normal in intelligence. Doctors says that she is behind 10 years of actual age. My question is how her condition will be cured?

MBBS, MD - Psychiatry
Psychiatrist, Chennai
Well sir usually mental retardation (intellectual disability) is an arrested development of human brain which is more of a permanent condition. Any child with early neonatal seizures with early childhood problem has more risk. But on the positive side usually children in the borderline or mild category can have good improvement with occupational therapy and domain specific training. My advice to you is kindly try to train her as any other kid daily and she should do well. Kindly consult an occupational therapist with expertise in this field for good results. Also be in touch with your psychiatrist for follow up. All the best.

Hi , there are many reasons for burning sensation in penis. Your history is inadequate. Do you have it during passing urine or all the time. Better you visit the same urologist once again because your problem takes time to settle and it is better if you stick on to one doctor.
