Piles - Know Its Causes And Surgical Treatment!!

Piles are inflamed masses, clumps, or cushions of tissue in the anal canal. Also called haemorrhoids, piles can be a very unpleasant condition to live with.

However, the fact is that we all have some amount of inflammation in our anal canal tissues. But it’s only when the haemorrhoids become too inflamed that symptoms occur and this is when you are diagnosed as having piles.

The size of piles is variable and they can be found inside or outside the anus.

Internal haemorrhoids typically occur from 2 to 4cm above the opening of the anus. They are the more common ones and are divided into four grades.

  1. Grade 1 – Less inflammation occurs and it’s usually inside the lining of the anus. These piles are invisible.
  2. Grade 2 – These are larger haemorrhoids but are inside the anus. When passing stools they can get pushed out, but return back into the anal canal unaided.
  3. Grade 3 – These are called 'prolapsed haemorrhoids' and appear outside the anus. They can be pushed back into the anal canal with a finger.
  4. Grade 4 - These haemorrhoids cannot be pushed back and need treatment in the form of surgery as they are large and hang outside the anus all the time.

External piles occur on the outside edge of the anus and are called perianal hematoma.

Symptoms and Diagnosis

  1. You may feel a hard lump around the anus. And this consists of coagulated blood which is painful
  2. A feeling of fullness even after going to the toilet
  3. Bright red blood discharged after a bowel movement
  4. Itchiness around the anus along with redness and soreness
  5. Mucus discharge while defecating
  6. Pain while defecating

Causes

Piles occur due to chronic constipation and diarrhoea, lifting heavy weights, pregnancy or straining during defecation.

Treatment
Piles are often not serious and disappear without any treatment and usually with the help of lifestyle changes like drinking more water, cutting down on caffeine, eating more fibre etc. Just 10% of patients consulting a doctor for piles require surgical treatment. And surgery may be necessary for grades 3 or 4 haemorrhoids only.

Surgical Procedures-

  1. Hemorrhoidectomy – This involves removing the excess tissue that is causing the bleeding. This is an effective procedure for treating grades 3 and 4 haemorrhoids but it can cause complications like difficulty passing stools, as well as urinary tract infections.
  2. Haemorrhoid stapling – The blood flow is stopped from reaching haemorrhoid. It is less painful than a hemorrhoidectomy, but it comes with risks like haemorrhoid recurrence and something called a rectal prolapse in which a part of the rectum pushes out of the anus.

Ejaculation of semen during sexual intercourse before or immediately after penetration. Premature ejaculation occurs when a man ejaculates sooner during sexual intercourse than he or his partner would like. Premature ejaculation is a common sexual complaint. Estimates vary, but as many as 1 out of 3 men say they experience this problem at some time. As long as it happens infrequently, it's not cause for concern. The primary symptom of premature ejaculation is the inability to delay ejaculation for more than one minute after penetration. However, the problem may occur in all sexual situations, even during masturbation. Premature ejaculation can be classified as lifelong (primary) or acquired (secondary). Lifelong premature ejaculation occurs all or nearly all of the time beginning with your first sexual encounters. Acquired premature ejaculation has the same symptoms but develops after you've had previous sexual experiences without ejaculatory problems. Many men feel that they have symptoms of premature ejaculation, but the symptoms do not meet the diagnostic criteria for premature ejaculation. Instead these may have natural variable premature ejaculation, which is characterized by periods of rapid ejaculation as well as periods of normal ejaculation. Psychological causes Some doctors believe that early sexual experiences may establish a pattern that can be difficult to change later in life, such as: •Situations in which you may have hurried to reach climax in order to avoid being discovered •Guilty feelings that increase your tendency to rush through sexual encounters Other factors that can play a role in causing premature ejaculation include: •Erectile dysfunction. Men who are anxious about obtaining or maintaining an erection during sexual intercourse may form a pattern of rushing to ejaculate, which can be difficult to change. •Anxiety. Many men with premature ejaculation also have problems with anxiety — either specifically about sexual performance or related to other issues. •Relationship problems. If you have had satisfying sexual relationships with other partners in which premature ejaculation happened infrequently or not at all, it's possible that interpersonal issues between you and your current partner are contributing to the problem. Biological causes A number of biological factors may contribute to premature ejaculation, including: •Abnormal hormone levels •Abnormal levels of brain chemicals called neurotransmitters •Abnormal reflex activity of the ejaculatory system •Certain thyroid problems •Inflammation and infection of the prostate or urethra •Inherited traits •Nerve damage from surgery or trauma (rare) Common treatment options for premature ejaculation include behavioural techniques, topical anaesthetics, oral medications and counselling. Keep in mind that it may take a little time to find the treatment or combination of treatments that will work for you. Behavioural techniques In some cases, therapy for premature ejaculation may involve taking simple steps, such as masturbating an hour or two before intercourse so that you're able to delay ejaculation during sex. You may try avoiding intercourse for a period of time and focusing on other types of sexual play so that pressure is removed from your sexual encounters. You can also try diverting your mind before ejaculation is ready to come and proceeding after a brief break, thereby prolonging duration of intercourse / action The pause-squeeze technique You and your partner can use of a method called the pause-squeeze technique. This method works as follows: 1.Begin sexual activity as usual, including stimulation of the penis, until you feel almost ready to ejaculate. 2.Have your partner squeeze the end of your penis, at the point where the head (glans) joins the shaft, and maintain the squeeze for several seconds, until the urge to ejaculate passes. 3.After the squeeze is released, wait for about 30 seconds, then go back to foreplay. You may notice that squeezing the penis causes it to become less erect, but when sexual stimulation is resumed, it soon regains full erection. 4.If you again feel you're about to ejaculate, have your partner repeat the squeeze process. By repeating this as many times as necessary, you can reach the point of entering your partner without ejaculating. After a few practice sessions, the feeling of knowing how to delay ejaculation may become a habit that no longer requires the pause-squeeze technique. Topical anaesthetics Anaesthetic creams and sprays that contain a numbing agent, such as lidocaine or procaine, are sometimes used to treat premature ejaculation. These products are applied to the penis a short time before sex to reduce sensation and thus help delay ejaculation. A lidocaine spray for premature ejaculation is available Although topical anaesthetic agents are effective and well-tolerated, they have potential side effects. For example, some men report temporary loss of sensitivity and decreased sexual pleasure. In some cases, female partners also have reported these effects. In rare cases, lidocaine or procaine can cause an allergic reaction. Oral medications Many medications may delay orgasm. Although none of these drugs is specifically worthwhile as people believe, approved medications may be prescribed for either on-demand or daily use, and may be prescribed alone or in combination with other treatments to treat premature ejaculation, some are used for this purpose, including antidepressants, analgesics and phosphodiesterase-5 inhibitors You can ask me personally if you want these tablets with your full health d.

You can try Kegel’s exercises ( stop and flow method during urination). No medicine can increase penile size, you can try vacuum erection device exercise (contact sexologist for details).

Hi lybrate-user. A cough is a sudden and often repetitively occurring reflex which helps to clear the large breathing passages from secretions, irritants, foreign particles and microbes. Ayurveda addresses cough as the kasa roga. Ayurvedic sciences primarily name the vata dosha as the prime cause of cough although kapha dosha and pitta dosha are also considered responsible. Ayurvedic treatment for cough can be bifurcated into two parts, the usage of herbs and abiding by dietary restrictions. Medication:- medication:- 1. Cof x-5 ml with warm water twice a day. 2. Sitopaladi churna-1/4tsp with honey daily 3-4 times a day. For 5 days. Ayurvedic treatment for cough and cold is also about dietary regulations. Some of the norms to be abided by would include: 1. Warm up the water before drinking as this would soothe the throat. 2. Avoid consuming anything cold directly from the fridge. 3. Do not consume veggies and fruits which have a cooling impact on the body. These would include papayas, oranges, watermelons, green bananas and cucumbers. 4. Preferably you should opt for consuming rice from an older crop if possible.

If you have tested for HIV and it is negative after one year. Then no chance of its infection. Always use protection while intercourse.

Take pranacharya kasantak syrup 2-2 tsf in hot water twice a day. Take sitopaladi. Praval and tankan bhasm churna with honey 3-4 times a day. Take ginger black pepper and basil leaves tea. Gargle with hot water (salted) avoid milk. Cold drinks. Take hot water steam.

Rest. Take a break from your normal activities. Ice. Place an ice pack or bag of frozen peas on the sore area for 15 to 20 minutes three times a day. Compression. Use a compression bandage to reduce swelling. Elevation. Elevate your foot to help reduce swelling.

Stop watching pornograpuy. Form habit of regular masturbation based on natural physical need and psychological need. Keep the timing at night with taking bath or any suitable time. Avoid stimulating your self artificially like by pornograpuy. Respect nature. Release of psychic tension that occurs time to time is not harmfull. No need to totally discontinue it but the idea is to master it. So keep it regular and avoid unnatural sexual stimulation. You are right age think about getting married also.

Post diarrhoea it is common to have constipation for few days. Take high fibre diet and drink plenty of water. Eat curd. It should settle.

Hi, please go for a detailed blood investigation like cbc, widal test, mp, montaux and routine urine examination and then revert back with the reports. Till then keep recording your temparature. If it is too high, do tepid sponging. Wear loose cotton garments. Drink plenty of fluids. Consume light and easily digestable food. Continue to take tab. Paracetamol 1-1-1 till the fever subsides. Drink only boiled and cooled water.
