My mother has been diagnosed with spine tuberculosis. She is having persistent pain in back. Is this normal? How long will this pain stay? What is course of treatment and precautions to be taken?

Erasmus Mundus Master in Adapted Physical Activity, MPT, BPTh/BPT
Physiotherapist, Chennai
Spinal tuberculosis is a destructive form of tuberculosis. It accounts for approximately half of all cases of musculoskeletal tuberculosis. Spinal tuberculosis is more common in children and young adults. Characteristically, there is destruction of the intervertebral disk space and the adjacent vertebral bodies, collapse of the spinal elements, and anterior wedging leading to kyphosis and gibbus formation. The thoracic region of vertebral column is most frequently affected. Formation of a ‘cold’ abscess around the lesion is another characteristic feature. The incidence of multi-level noncontiguous vertebral tuberculosis occurs more frequently than previously recognized. Common clinical manifestations include constitutional symptoms, back pain, spinal tenderness, paraplegia, and spinal deformities. The most important factor in SCI rehabilitation programme is early rehabilitation. The positioning in the acute phase, early starting of passive, active-assisted and active exercises will greatly contribute standing of the patient earlier and to mobilize. Standing and mobilization are not recommended in the acute period for these patients. Generally, standing and ambulation are recommended during the subacute period. Patients with spinal tuberculous, bracing with a conforming orthosis (plaster or molded thermoplastic) has been used in combination with antituberculous drugs as initial treatment. Bracing is continued 3 mo after the first radiologic sign of bony fusion[37]. The onset of pain or increase in pain during exercise programs in the early period should be evaluated carefully. Pain aggravating exercises should be avoided and the exercise program should be discontinued if there is a significant increase in pain intensity disturbing the patient following the rehabilitation program. The patient should not be exhausted during exercise and mobilization and should have adequate resting after exercise. High calorie diet regimens should be provided since metabolic requirements are increased during both disease and the rehabilitation period.

Hi I am fedup of my cold and cough. Can you please tell me some remedy so that I can get rid of my cold and cough permanently.

PGD Maternal Child Health, MBBS
General Physician, Akola
You seem to be suffering from perennial rhinitis leading to secondary respiratory infection off and on causing cough. Save yourself from common allergens like dust, smoke, pollens by covering your nose. Keep away from places where cleaning is going on. Use freshly laundered bedsheets and pillow covers as these are the commonest sources of allergens. In case a bout of running nose and sneezing starts, get some antihistamine preparation from your family physician immediately without waiting for day 2 or day 3 as the flowing secretions tend to descend the respiratory tract and then later on antihistamines make them more viscid and difficult to cough out. Don't use cough suppressants like corex or phensidyl. Instead get some good expectorant for initial 2 or 3 days only to bring out any tough secretion out. Following all this must help you a lot in future.

Sinus problem occurs bcz of inadequate drainage of sinus tha may be due to some mass in nose, deviated septum, allergic polypoidal mucosa. You need to take antihistaminics and steroid nasal spray. Avoid exposure to dust. Get a ct scan done and review with report.

I felt I have some breathing problems, sometimes I force fully take breath and leave it. How can I recover this problem?

FCCP (USA), MD - Pulmonary Medicine
Pulmonologist, Indore
Get a simple breathing test called spirometry done with reversibility testing. This will evaluate the degree of bronchial obstruction if present and will also be an assessment for asthma. Then you will be relieved by inhaled bronchodilators.

Hello there, for cough, take 2-2 teaspoonful of pranacharya kasantak by mixing it in hot water twice daily. Sitopaladi, praval and tankan bhasm churna taken with honey can also bring relief. Consume this 3 to 4 times a day for relief from dry cough; you can also drink tea with basil leaves or add ginger and black pepper in it. Salted hot water gargling also helps. And stay away from cold drinks and don't take milk when having cough.

My 15 month old has been diagnosed with bronchiolitis twice in a month. She was wheezing. What age can you say she is asthmatic. How to know she is distress if asthma.

My 15 month old has been diagnosed with bronchiolitis twice in a month. She was wheezing. What age can you say she is...
A wait and watch policy is best. Wheeze is a symptom and a sign! Asthma is a diagnosis made with clinical review of the condition. The main character of Asthma is the reversible, response to bronchodilators, recurrent nature of the cough, wheeze or breathing difficulties!
