Hi A female patient age 60 suffering from pain in many joints like knee, wrist, shoulder and back. Swelling in whole body specially face and lower limb. Hypertensive, blood sugar fasting 125 and pp 160. T3 and t4 level is normal but tsh is slightly elevated(4. 26) please tell diagnosis and treatment

I have pain in my back from last 20 days. I feel pain while I bend towards front down side. So I can't bend front down side. What should I do?

Hi, thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pain. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you, you can also add a muscle relaxant for 5-7 days - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Sir/madam I have preparing for physical test, but due to running I have pain in my right leg. please suggest me?

Hard running surfaces, doing too much too soon, inappropriate shoes and body shape can lead to over-use of muscles. Treatment at first includes regular ice, do it for 20-30 minutes every 3 to 4 hours for 2 to 3 days, or until the pain is gone, together with reduced mileage or rest. Addressing the causes is likely to include ensuring that shoes have adequate cushioning and stability, and building up training mileage slowly. Take anti-inflammatory painkillers. Do range-of-motion exercises. Go to physiotherapist to identify and treat issues in your legs or running mechanics that may be causing pain. A therapist can also help ease the pain and guide your return to sport. Best wishes.

I am a 35 years male have a problem swollen feet and knee pain and have regular pain below left chest sometime just below chest back side and sometime below right chest and now this are days my forehead use to be heaviness most of the time, before five year back I suffer from malaria twice in one year, within eight months gap, after that my stomach use get bloated several times I consult with with my doctor he ask me to take small meals in one hour gap not to eat to much at a time after few months every things was ok but in the year 2015 mid of june I felt sick, that was my first day I went to gym for physical fitness after doing a work out for half an hour or forty five minutes I was on my way back to my flat but suddenly my forehead was spinning I was getting unconscious my eyes was not visualized properly like heavy lights white and feeling vomote any how with the help of security I went to my flat and felt sleep then next day I went to a doctor and he advice for total blood test, the blood test reports were showing positive jaundice. Uric acid. Cholesterol. Alkaline phosphatase high. Uria creatinine. And hbv positive. Then doctor prescribed medicine for three months with a routine check-up after three months again doing all blood test report are showing everything was normal only usg report was showing liver is enlarge, doctor prescribed medicine for two more months with routine check-up everything was fine good, but again now this are days I am facing the problem like my feet are getting swollen stomach is getting bloated I am not feeling sleepy and not feeling hungry and getting pain again sometime below left chest and right chest below also and same table back sometime lower abdominal left and sometime getting senseless foots and hands forehead getting heavyness eyes are getting dizziness. Please doctor help me.

Quit habits strictly. Seems like you have oral submucous fibrosis (osmf). Kindly consult an oral & maxillo facial surgeon in person for further suggestion. We need more investigations with clinical examination to decide upon treatment. You may need conservative or surgical management for mouth opening depending upon the stage of osmf. Advise full mouth opening, ballooning exercise & side to side movement of the jaw. Wrap 25 to 30 ice-cream sticks, make a bundle, put it between upper & lower front teeth for 15 minutes thrice a day increase 1 sticks each day.

If you have lower back pain pain pls check your body weight and your haemoglobin levels, as Anaemia always leads to the symptoms of being tired and also having leg pain though there may not be any pathological reasons for back pain. . Ideally you should recover completely after 5 years of ligament injury. You should go for a physiotherapy rehabilitation program for knee for at least 3 months. Before planning the surgery. As physiotherapy will strengthen your joint and make it more efficient.

A high uric acid level can be caused when your body either produces too much uric acid or your kidneys don't eliminate uric acid rapidly enough. A high uric acid level may cause increasingly frequent attacks of gout, or it may never cause problems. A high uric acid level may also cause some people to develop kidney stones or kidney failure. And some people with a high uric acid level also develop high blood pressure, heart disease or chronic kidney disease, but it's often unclear whether this is a direct cause or merely an early warning sign of these conditions. Factors that may cause a high uric acid level in your blood include: Diuretic medications (water pills),Drinking too much alcohol,Genetics (inherited tendencies),hypothyroidism (underactive thyroid),Immune-suppressing drugs,Niacin, or vitamin B-3, obesity,psoriasis, Purine-rich diet — organ meat, game meat, anchovies, herring, gravy, dried beans, driedpeas, mushrooms and other foods Renal insufficiency — inability of the kidneys to filter waste Tumor lysis syndrome — a rapid release of cells into the blood caused by certain cancers or by chemotherapy for those cancers,Also, you may be monitored for high uric acid levels when undergoing chemotherapy or radiation treatment for cancer. Avoid protein diets, avoid more milk products. bad habits , fast food/fat/oily,salty foods. Coffee, smoking, alcohol intake. Non-veg., take whole grain,oats,green veg.,plenty of water in day time, 1 glass of water at night before bed.Take tab. Zyloric 100 mg once a day.Take tab. Bencid 500 mg once a day. Tab. Ulracet sos or tab. Naprosyn 500 mg once a day or sos for pain in joints.

She is suffering from back pain and neck pain from last 6-8 months and she is a student so she has to sit for long hours to study and she has to attend college where she has to sit so what's the problem with her. Can you suggest few measures to get rid of that? And she also suffers from insomnia means she is unable to sleep at night she just don't feel sleepy at all but during day time that affects her she feels drowsy then for all day but she doesn't feel like sleeping at night. Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

1.. Go for fasting once a week. You can go for absolute fasting or rely on a liquid diet to digest ama (toxin). Fasting should be followed by hot fomentation or by taking a bath with lukewarm water or steam bath. 2.. After fasting and fomentation, mix powdered long pepper, black pepper and dried ginger in equal quantities and take 1 teaspoon of this powdered mix with lukewarm water twice a day. This will help you restore your digestive fire. 3.. Take one tablespoon of castor oil before going to bed, once or twice a week. It helps in cleansing ama (toxin) build up and balancing vata. 4.. Mix equal quantities of powdered fenugreek seeds, turmeric and ginger powder. Take one teaspoon daily in the morning and evening. 5.. For dry fomentation of the affected joints, take a cloth and fold it into four, fill it with hot and sand make a potli (bag) out of it. Use this bundle for the fomentation of different joints. This is very helpful in reducing pain and swelling. 6. Do exercise and yoga daily...u have to decrease 4-5 kg weight.
