My sister has a pain around the areas of anus so give me a reason why it happening plz.

PGDMLS, PGDHHM, LLB, PhD Surgery, MS - General Surgery
General Surgeon, Gurgaon
The commonest cause for such pain is fissure. Anal fissure improves with general measures but may require surgery. Piles of an early grade also improves with general measures. The general measures I would suggest are 1. Take lot of vegetables & fruits in your diet 2. Avoid chillies & spices 3. Take lot of fluids 4. Avoid constipation 5 periodically do anal exercises (alternately contracting & relaxing the buttocks, anal area & upper thighs) 6. Take sitz bath 3-4 times a day. This means sitting in a tub of warm water & doing anal exercises 7. Atleast once get it examined by a general surgeon.

My friend's husband both kidneys have failed and 38 years old. Other than dialysis, transplant is there any natural method to retrieve kidney function by consumption of leave etc.

MBBS, MD (Med), DNB (Nephrology)
Nephrologist, Mumbai
Dear lybrate-user, Allopathy has dialysis and transplant as only alternative. Dialysis should be started if kidney power calculated GFR is less than 10. Transplant is better than dialysis if age less than 60. For further Detailed consultation, you can consult with me privately.

Modern treatment for Kidney Cancer

Felllow in Endourology, MCh Urology, MRCS, MS, MBBS
Urologist,
What is kidney cancer?
Kidney cancer (renal cancer or renal cell carcinoma) is one of the ten most common cancers in both men and women.
It occurs when the cells in kidney grow uncontrollably and start invading surrounding tissues (i. E, undergoes malignant change). The risk factors are age > 40 years, smoking, obesity, long term dialysis, certain genetic conditions, family history, high blood pressure and male gender. However, it can occur to anybody.

What are the symptoms of kidney cancer?
1. Blood in urine
2. Pain or lump in side of upper abdomen
3. Weight loss or loss of appetite
4. Long term fever
However, many of kidney cancers are detected on routine ultrasonograms done for other purposes. Thus, regular ultrasonogram testing can pick up these tumors very early.

What are the diagnostic tests?
Contrast enhanced computed tomography (ct scan) of abdomen is the current gold-standard test to diagnose kidney tumors and evaluate for local spread. Chest x-ray or ct chest helps in diagnosing spread to lungs. Some blood tests will help to evaluate function of kidney and general health condition.

What are the best treatment modalities?
Surgical removal is the best treatment for kidney cancer. Chemotherapy or radiation therapy does not act on kidney cancer and hence are not used.
If the size is small, only the tumor is removed and rest of kidney can be preserved (nephron sparing surgery or partial nephrectomy).
For large tumors, the whole kidney, surrounding tissues and lymph nodes are removed totally (radical nephrectomy).
Both these surgeries can be done through keyholes (laparoscopy), thus reducing the pain and leading to faster recovery.
If the tumor has spread all over the body, then targeted therapy is given using specific drugs (usually in the form of tablets) like pazopanib, sunitinib, sorafenib etc.

How to follow-up after treatment?
Regular check up using ultrasonogram, ct scan, chest x-ray, renal function tests, liver function tests are needed to maintain good health.

Take home message:
Regular ultrasonograms after the age of 40 years (at least annually) can help in diagnosing these tumors and curing them 100%.

I am 30 years old male my problem 10 mm stone in right kidney. What I do
Please solve my problem.
Kidney stones can be easily removed with the help of ayurvedic diet & lifestyle measures. You can do the following home remedies: 1) take a equal mixture of lemon juice & olive oil 4-4 spoons 2) have a mixture of apple vinegar & honey. 3) boil two figs in a cup of water and drink it on an empty stomach early in the morning 4) drink a glass of tomato juice with a pinch of salt and pepper every morning.

I am 42 years male and my creatine level is about 1.8, how do I keep it under control.

PDDM, MHA, MBBS
General Physician, Nashik
I am 42 years male and my creatine level is about 1.8, how do I keep it under control.
I recommend the following 1. Find out how much of protein you are losing in the urine. Protein in urine rises the creatinine. Tablets like ace/arb can reduce protein in urine 2. Avoid red meat 3. Avoid weight lifting exercises 4. Tab nephrosave twice daily has given good results in my patients 5. Most importantly make sure bp is within 130/70 at all times.
