Diagnosing Peripheral Vascular Diseases With Interventional Radiology

MD - Radio Diagnosis/Radiology, MBBS
Radiologist, Mumbai
Diagnosing Peripheral Vascular Diseases With Interventional Radiology

Peripheral vascular disease (PVD) is a circulation issue that affects the veins and blood vessels outside of the brain and heart. PVD commonly strikes the veins that supply blood to the arms, legs, and organs situated beneath the stomach. These are the veins that are located away from the heart. They are known as peripheral vessels.

In PVD, the width of the veins get limited. Narrowing is normally created by arteriosclerosis. Arteriosclerosis is a condition where plaque develops inside a vessel. It is additionally called 'solidifying of the arteries'. Plaque acts towards reducing the amount of blood and oxygen that is supplied to the arms and legs. As the plaque development advances, clumps may develop, which may further affect the vessel.

There are two main types of PVDs:

  1. Functional PVD: This doesn't include physical issues in the veins. It causes accidental side effects. Typically, these fits happen suddenly.
  2. Organic PVD: This includes changes in the vein structure. This sort of PVD causes irritation, tissue harm, and blockages.

The most well-known reasons for functional PVDs are as follows:

  • Emotional stress
  • Smoking
  • Cold temperatures
  • Operating Vibrating machinery

The common causes of such natural PVDs are given below:

  • Smoking
  • High circulatory strain
  • Diabetes
  • High cholesterol

The symptoms include the following:

  • Cramps
  • Achiness
  • Fatigue
  • Burning

Diagnosis:

PVD can be diagnosed using interventional radiology (IR).

IRis a sub-claim of radiology that gives an image-guided diagnosis, and if required, includes treatment of the organs as well.It has developed as a first-line treatment in the administration of PVD.

Advantages:

IR medications are for the most part less demanding for patients than surgery, since they include no surgical cut. They are less painful and have shorter stays at the hospital. By and large, the patients are discharged on the same day the procedure is done. This mainly includes angioplasty and stenting. The procedure is as follows:

  • Utilising imaging for direction, the interventional radiologist puts a catheter through the femoral artery in the crotch to the blocked vein in the legs.
  • At that point, the interventional radiologist expands a balloon to open the vein that is blocked.
  • Sometimes it is opened with a tiny metallic cylinder called astent.
  • This is a treatment that does not require surgery; only a scratch in the skin the extent of a pencil tip.

Alternative measure:

Angioplasty and stenting have totally replaced invasive surgical methods. Early trials have proven IR to be as successful as surgery for some blood vessel and artery impairments. Earlier, extensive clinical experience demonstrated that stenting and angioplasty are favoured as first-line treatments for more procedures all through the body .

Doctors as well as patients who have been through the same, believe that IR is much better for PVD than invasive surgery, since it reduces the risk of infection.

High triglycerides and cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Change life style .do meditation, yoga, Go walk daily, do any one physically activity daily i.e.play any game., Avoid fast foods, oily foods, junk foods, alcohol, smoking, fatty foods, non-veg.,avoid stress, take plenty of water in day time. And 1 glass of water before bed at night. use flax oil, omega-3 fatty meal or cap., take .green veg., whole grain. Garlic,. Cod liver oil. Take anti cholesterol medicines . Cap. Ecosprin gold-10 at night daily.

General Physician,
HIGH BLOOD PRESSURE-HYPERTENSION FACTS
==========================================
Some people with hypertension may react with alarm when they check their blood pressure at home or at the drugstore and the reading is elevated. It's not unusual for people with an isolated finding of elevated blood pressure to show up at the local Emergency Department with sudden concern, but in many cases, the real concern is not a single high reading, but the cumulative effect of persistent uncontrolled hypertension.
Hypertension is a risk factor for many negative health outcomes, including heart failure, coronary artery disease, stroke, kidney disease, peripheral artery disease and hypertensive retinopathy, a disease of the eye. Hypertension is also associated with type 2 diabetes mellitus, which is why the United States Preventive Services Task Force has issued a recommendation for diabetic screening in people with high blood pressure. Good control of blood pressure results in a rapid reduction of risk for cardiovascular disease.

Hypertensive heart disease results from changes in both the structure of the heart and blood vessels and their function as the body adapts to abnormally high blood pressure. If you can imagine the heart as a pump operating against a fluid circuit, picture the increased work of that pump if pressure within the circuit is increased. Some of the changes caused by persistent high blood pressure can be partially reversed with treatment using ACE inhibitor medications.

Hypertension results in wear and tear of vital organs and vessels as a result of mechanical stress.

Blood vessels have mechanisms to regulate blood flow throughout the body, and when high blood pressure occurs, the blood vessels constrict in initial response. Eventually, the walls of the blood vessels thicken and tissue damage occurs, resulting in hardening of the vessel walls.

Hypertensive disease of the eye is known as hypertensive retinopathy. In the retina of the eye, changes in the blood vessels eventually result in disruption of the integrity of the vessel, and blood or fatty acids may leak onto the delicate retinal tissue. This results in permanent vision change, and it's very preventable.

Complications of stroke can be due to hemorrhage of the blood vessels that supply the brain, or, more frequently, can result from poor blood flow through arteries damaged by persistent high blood pressure. Approximately 85% of strokes are ischemic, or due to poor blood flow. Another manifestation of hypertensive disease that affects the brain is hypertensive encephalopathy. This condition refers to a change in mental status, frequently accompanied by headache and nausea, that results from swelling in the brain caused by alterations in blood flow at very high blood pressure. This condition is associated with malignant hypertension, or hypertensive emergency.

Persistent high blood pressure is one of the leading causes of chronic kidney disease, frequently requiring dialysis. Blood is filtered through the kidneys, and when the small blood vessels of the kidney are damaged by chronic uncontrolled hypertension, the body becomes unable to filter waste. People with kidney damage eventually spend hours hooked to dialysis machines that can remove waste products from their bloodstream, or they may require a kidney transplant. In either case, prevention of the complication is preferable and less complex.

Unfortunately, it is easy to underestimate the cumulative effects of high blood pressure, since the damage is asymptomatic initially. It's important to recognize the inevitable consequences of ignoring persistent high blood pressure. If you have high blood pressure, it's important to realize the silent damage occurring over time if your blood pressure is not adequately treated. If you have a high blood pressure reading, you should repeat measurement on two more occasions. If your blood pressure remains above the level your doctor has suggested, it is essential to follow up. There are many excellent and affordable medications available for treatment of hypertension. Some people require a combination of drugs to maintain control. Whatever the case, the inconvenience of treatment is far exceeded by the benefits to your health over a lifetime.

If you are used to having milk & egg every day without having any digestive problems, then go ahead and have it. They are good for your health. A bp of 104/80 (occasionally) is not alarming for a 51 years old like you. Try meditation, 30 mins daily & get your bp checked in the evenings, once in a month. If it is bothering you. Or else, you are fine & be relaxed!

If you are not having symptoms then do not bother about having a bp of 100/80. If there are symptoms, you can increase your bp by increasing the salt and fluid intake.

If you want to get rid off this problem naturally, I suggest him to do regular exercises, yoga, pranayamas, regularly.& tell him to quit those smoking alcohol consumption, though less frequently it is. Taking more water, daily minimum 3-4 litres irrespective of his thirst, consuming more raw vegetables,[whatever he can eat. Like carrots, beet roots, lady's fingers, keera etc], fresh all seasonal fruits, sprouts will help keeping general health, take daily 1 glass of lemon juice mixed with honey. 1-2 times/ day. Don't take pickles, spicy, junk, oily, fast food, which will take up to risk of increasing the bad cholesterol levels. Taking 1 litre of water in morning with empty stomach will help maximum to keep all vital organs function well.

Only 18 but suffering from gastric problems, before some tests are suggested, do take care about your diet. Eat moderately, regularly, nouritious food which has simple proteins like milk & curds and less oils, besides raw vegetables that can cause distention. Do write back after a week, if problem is not resolved, some tests will be suggested.

Many people have no symptoms in the early stages of avascular necrosis. As the condition worsens, your affected joint may hurt only when you put weight on it. Eventually, the joint may hurt even when you're lying down. Pain can be mild or severe and usually develops gradually. Pain associated with avascular necrosis of the hip may be focused in the groin, thigh or buttock. In addition to the hip, the areas likely to be affected are the shoulder, knee, hand and foot. You should consult a neurologist for for the proper treatment of clot in the brain.
