I have low hemoglobin and I have dark circles around eyes. What should I do?

Certified Diabetes Educator, Registered Dietitian (RD), PGDD, Bachelor of Unani Medicine and Surgery (B.U.M.S), General Physician
Dietitian/Nutritionist,
Do you also have puffy eyes or bags under your eyes? what are your sleeping patterns? there are many causes of dark circles. I need more information from you which will help you choose a better treatment plan. Too much caffeine/ tobacco and too little sleep, anemia, dehydration are common causes. If you dont have any of these causes then dark circle peels may be prescribed for dark circle which contain contains arginine, lactic acid, allantoin and aloe vera. Arginine is an amino acid derived from brown sugar which boosts collagen, increases elasticity and moisturises the skin. Lactic acid exfoliates and lightens the skin. Allantoin and aloe vera helps to soothe and regenerate the skin. This is used in conjunction with vitamin c and matrixyl that provides firmness and resistance to the skin around the eye. Do reply back for medication prescriptions. Being also a general physician and registered dietitian, I prescribe both evidence based herbal as well as allopathic medicines complementing with dietary guidelines and home remedies carefully personalized for each individual patient.

Application of Sunscreen

BHMS
Homeopath, Faridabad
Application of Sunscreen
Sunscreen protects us from harmful effects of sun rays and SPF tells about the duration of action of the particular sunscreen. The right way of application of sunscreen is to form a thick layer onto your skin, apply liberally, so that the skin doesn't get directly exposed (never rub the sunscreen rather spread it evenly) and sun rays will gets reflected back after falling on the skin, that too 20-30 minutes before stepping out (which gives enough of time for sunscreen to settle down on the skin). And in case, you are out in the sun for long hours, keep on re-applying the same after 3-4 hours.

During swimming, use water proof sunscreen. (chlorinated water makes us more prone to sun tan.

We indians need spf - 25 to 30 only. White people, albinos or those who lack melanin needs more spf as their skin is extremely sensitive in the sunlight (so, thanks to god for giving us melanin as a natural sun-protector!).

But as we all know synthesis of vitamin d takes place in the presence of sunlight, we should get ourselves exposed to sunlight without sunscreen, for at least 15-20 minutes 2-3times a week!

(choose right sunscreen as per your skin type).

Many face lighting creams are available in market with various combinations and concentrations but you should see your dermatologist prior of using any cream. He will see and suggest you the right combo required. Meanwhile you must always cover your face with sunscreen with spf 30 minimum. Coz sun exposure can worsen the dark spots.

Wanting to look young is not exactly a new idea

DDV, MBBS
Dermatologist, Mumbai
Wanting to look young is not exactly a new idea – the search for the fountain of youth has been going on for centuries.

There are many treatments available to combat the aging effects of the sun, from creams to peels to lasers. Plus, reducing the signs of sun damage can reduce your risk of developing skin cancer.

The sun and the years take their toll on your skin. Photo ageing is often the first sign that your skin has been damaged, but such repeated exposure can also lead to skin cancer. Luckily, many of the treatments for photo ageing not only restore skin youthful appearance, they also reduce your risk for developing skin cancers and pre cancers.

There are many non-surgical options offered to people who would like to look younger.
1) Creams – Sunscreens, Retinoids.
2) Injectables – Such as Dermal Fillers, Muscle Relaxers.
3) Skin Surface Treatments – Including Chemical Peels and Dermaroller / Micro Needling.
4) Laser, IPL and other heat treatments.
Sunscreens

If you don’t wear a sunscreen every day, today is the day to start. Daily application of sunscreen with an SPF of 15 or higher- whether on its own, or in a moisturizer – will prevent additional damage to your skin. Also, studies have shown that daily use of sunscreen can actually reduce the existing number of actinic keratoses – abnormal skin eruptions or pre cancers that can lead to skin cancer.

Retinoids
Retinoids have been a breakthrough discovery for the treatment of photo aged skin. Synthetic derivatives of Vitamin A, they can improve discoloration of the skin, degeneration of elastic tissue, and fine wrinkling by enhancing naturally occurring production of collagen and elastic fibers. In addition to giving skin back its youthful appearance, retinoids can inhibit tumor growth, decrease inflammation, and enhance the immune system. Continuing use of retinoids can reduce the number and size of actinic keratoses. Currently, two retinoids are available for treatment of photo ageing: (1) Tretinoin (2) Tazarotene. Note that retinoids can irritate the skin and cause dryness and photo sensitivity – an extreme sensitivity to the sun. They are available by prescription only.

Lunch Time Lifts
The “Lunch Time Face Lift” promises a younger, better-looking you with no surgery, no general anaesthetic and no down time. While “Lunch Time” and “Face Lift” may be a bit of a stretch, these days it’s increasingly possible to take a few years off your face in a day clinic without the expense, pain and temporary disappearance from public life associated with surgery.

INJECTABLES Dermal / Skin Fillers
Filling substances are injected under facial skin to round out contours and correct wrinkles, furrows and hollows. A number of substances are currently used as fillers, including bovine collagen, hyaluronic acid, polymer implants, and the patient’s own (autologous) fat. To maintain the effects of bovine collagen and autologous fat, the procedure needs to be repeated periodically. Many of these treatments can be used in conjunction with one another to produce younger-looking. It is important to consult with a licensed dermatologist to discuss treatment options, as many of these treatments can cause damage in inexperienced hands

Rather than just smoothing or tightening skin, they can change the facial profile to a more youthful one.

Hyaluronic acid is now more popular. Hyaluronic acid lasts 6-12 months – although treating areas of the face that move less will last longer those that move more, such as the lips – and you tend to get better results with each treatment. There’s evidence that hyaluronic acid injections can also have a more permanent anti-ageing effect by stimulating collagen growth.

Muscle Relaxers
Botulinum toxin A temporarily paralyses muscles when injected. This prevents the skin above from creasing up and causing wrinkles. It only works on so-called expression lines, such as frown (vertical) lines and surprise (horizontal) lines on the forehead, smoothing them and preventing them from becoming more pronounced. The effects take a few days to kick in, and initially last a few months – longer after continued treatment.Read more…
Liquid Face Lift / Liquid Facial
Combined use of botulinum toxin A and dermal fillers is called a Liquid Face Lift / Liquid Facial.

SKIN TREATMENT

Chemical Peel
Acid, such as glycolic acid, lactic acid or trichloracetic acid, is applied to the skin and exfoliates the top layer of skin, causing dead skin cells to peel off. At stronger concentrations, new tighter skin forms as collagen production is stimulated in response to the wounding. It helps reducing fine lines, small scars, discoloration and sun damaged skin. The stronger the peel, the more dramatic and longer-lasting the results – stronger peels (concentrations of more than 30%) are better left to medical professionals rather than beauticians.
The benefits can last for two years for stronger peels, although it depends on the strength of acid used (which determines how deep the peel is). These days a course of several milder peels, rather than one strong peel, is common. Milder peels may require top-up peels every few months.Read more…

Dermaroller / Micro Needling
Dermarollers are used for a treatment process called micro-needling or skin needling. It is a form of collagen induction therapy. The actual dermaroller is made up of a small, solid wheel attached to a little stick that propels it backwards and forwards across your face in small sections. The roller is covered with hundreds of minuscule ‘micro-medical’ needles that penetrate the skin, creating lots of tiny micro injuries.Read more…

Intense Pulsed Light
Intense pulsed light, commonly abbreviated as IPL, is a technology used by medical practitioners to perform various skin treatments including hair removal and photo rejuvenation. The technology utilizes a broad spectrum light source, with a general spectral range of 515 to 1200 nm. Various cutoff filters are commonly used to filter out lower frequencies and narrow this spectrum to target specific structures and chromophores. Because of the broad spectrum of the IPL light source they are unique, as opposed to lasers, in the ability to filter the light spectrum to target various chromophores in the skin

Menorrhagia (Heavy Menstrual Periods) - A Common Indication For Hysterectomy

M.D. Gyn. & Obstetrics, MBBS, Diploma in Advanced Gynaec Laparoscopic Surgery, D.N.B., F.C.P.S.(Mid. & Gynae), D.G.O., D.F.P.
Gynaecologist, Mumbai
Menorrhagia (Heavy Menstrual Periods) - A Common Indication For Hysterectomy

Menorrhagia refers to very heavy and prolonged menstrual bleeding or periods in women. The bleeding is abnormal in nature, and if this continues for a long time, a hysterectomy surgery needs to be carried out to solve the problem.

Bleeding during the night time and passage of large blood clots during menstruation are other symptoms of Menorrhagia.

Causes:

The various possible causes of this kind of heavy menstrual bleeding are:

  1. Hormonal imbalance, specifically of estrogen and progesterone is a common cause, which is more likely in adolescents and in women nearing menopause. Dysfunction in the ovaries also causes hormonal imbalance.

  2. Non-cancerous tumors or fibroids may also be responsible for menorrhagia.

  3. Women suffering from a miscarriage or ectopic pregnancy are likely to experience heavy menstrual bleeding.

  4. Using blood thinners may lead to menorrhagia.

  5. An intrauterine device used for birth control measures can cause disturbance and may lead to heavy menstruation.

  6. A condition known as adenomyosis, where the glands in the uterus lining get embedded in the walls of the uterus, also causes menorrhagia.

  7. Pelvic inflammatory diseases, infection in the uterus or fallopian tubes are other common causes.

  8. Heavy bleeding may occur in patients with ovarian or cervical cancer.

  9. Diseases of the kidney, liver or thyroid diseases may also cause menorrhagia.

Diagnostic Tests:

Several tests and methods are carried out for the diagnosis of menorrhagia. They are:

  1. A general physical examination.

  2. Bleeding diary of the woman.

  3. Blood tests to detect anemia or thyroid.

  4. Pap Smear for the evaluation of cervical inflammation, infections or cancer

  5. Ultrasound for evaluation of pelvic organs like uterus, ovaries and pelvis.

  6. Hysteroscopy, where a camera is inserted into the uterus to observe the linings.

  7. Sonohysterography, where a fluid gets instilled into the uterus along with an ultrasound test.

Treatment:

Certain drugs are used to treat menorrhagia. They include:

  1. Iron supplements for treating anemia.

  2. NSAIDS or non-steroidal anti-inflammatories for treating and reducing blood loss.

  3. Tranexamic acid is used for reducing menstrual bleeding.

  4. Several oral contraceptives are used for regulation of the menstrual cycle. Oral progesterone corrects hormonal imbalance and reduces bleeding.

Surgical procedures for treatment of menorrhagia include:

  1. Hysteroscopy, dilation and curettage, where uterus lining is scraped and evaluated.

  2. Uterine artery embolization treats menorrhagia with fibroids.

  3. Focused ultrasound ablation enables the killing of the fibroid tissue.

  4. Myomectomy, endometrial ablation and endometrial resection are other surgical techniques.

  5. Hysterectomy surgery is the penultimate solution, where the uterus, cervix or ovaries are completely removed.

Menorrhagia is characterized by abnormally heavy menstrual bleeding in women. This is quite unhealthy and proper measures must be taken for curing the heavy bleeding.

I have a slight white patch on my face. After three months of medication the patch seems too fade a bit but is still very visible. What treatment shall I take for this? Is chemical peeling an option to get rid of these scars? Or shall I try homeopathy once. Please help.

White spots on can occur due to a number of reasons, of which conditions can be mild or severe. The condition can be mild or severe, and generally covers the majority of the skin on a particular area of the body. They may also cause irritation or itching. The occurrence of whites on the face can be extremely distressing. People affected with this problem should obtain medical advice to diagnose what is causing the condition and the suitable treatment. Causes of White Spots White spots on the face occur for various reasons. Understanding some of these reasons may be beneficial in treating the condition. Furthermore, understanding the cause can also help people avoid certain circumstances that increase the risk. 1. Tinea Versicolor This is a chronic fungal infection of the skin. Also known as pityriasisversicolor, this fungus causes discolored patches on the skin as it interferes with the normal pigmentation. The color of the patches may be darker or lighter than the surrounding skin and most often, affects the back and trunk of adolescent males. Exposure to sunlight, hot, humid weather, oily skin and excessive sweating may aggravate the condition. 2. Vitiligo This is a skin condition that results in the loss of melanocyte pigmentation of the skin. This results in white patches that feel as if it is part of the skin. The cause of vitiligo is unclear although it is believed to be an autoimmune disease. However, mild trauma to any area of the skin including the face can also result in the development of vitiligo. Vitiligo can affect the face, hands and feet, elbows and genital area. 3. Idiopathic Guttate Hypomelanosis White spots on the face may also be due to Idiopathic Guttate Hypomelanosis, a skin condition resulting in 2 to 5 mm flat, white spots commonly found on the shins and forearms. This skin condition mostly affects fair skinned people, although it may occur in dark skinned individuals too. The white spots may occur more commonly in people over the age of 40. Treatments for White Spots on Face The medical treatment for white spots depends entirely on the root cause that underlies the condition. There are certain things that you can try to accelerate the process of treatment. Some of these mentioned treatments are professional while others are home remedies. Home Remedies.  For treating vitiligo, consumption of ginger juice is recommended as it promotes blood flow. Neem seed oil applied to the affected area will also bring beneficial results.  Idiopathic Guttate Hypomelanosis may not require treatment as it is harmless. However, topical steroids and retinoids may help treat the condition.  Washing the face with mild soaps and exfoliating the skin at least twice a week with a honey, rice, turmeric and sandalwood powder paste can treat Tinea Versicolor. So identify your problem of white spots with the help of your local doctor so that appropriate treatment is given or send me your picture depicting those white spot along with the complete history of your spots, so that I am clear in making a diagnosis.

Hello, Take healthy balanced diet including whole grains, green leafy vegetables, fruits and nuts. Drink 10 to 12 glasses of water daily. Take a sound sleep of 8 hours minimum. Apply bakson's under eye cream on the dark circles. Apply Ramdev's aloe vera gel on the whole face at night. Home remedy:-1.Cucumbers have skin-lightening and mild astringent properties that help fix those raccoon eyes naturally. Plus, they have a soothing and refreshing effect. Cut a fresh cucumber into thick slices and chill them in the refrigerator for 30 minutes. Put the slices onto the affected skin area for about 10 minutes. Wash the area with water. Repeat twice daily for about a week or more. Another option is to mix cucumber juice with lemon juice in equal amounts. Use a cotton ball to apply the mixture on the affected skin. Leave it on for 15 minutes and then wash it off with water. Follow this remedy daily for at least one week. Apply topi Berberis on pimples.
