5 Things to Know about Chronic Pain

Fellow of Endoscopic spine surgery (FENDS) , Fellow of Interventional spine & pain (FISP) , MD-Anaesthesiology & Pain Medicine, Fellow of Pain Management (FPM) , DA( Gold medal-Anaesthesiology & Pain Medicine) , MBBS

5 Things to Know about Chronic Pain

Aside from the obvious problem with chronic pain - there are many other downsides to chronic pain that are important to know about. For those living with and enduring chronic pain, pass this along to your loved ones to help them understand and be supportive.

1. Pain is rarely 'all in your head'
People in pain are often treated as if their pain is actually made up or greatly exaggerated. While it is true that pain is subjective (people simply perceive pain differently) and some people may report pain because they have other agendas - for the vast majority, the pain is real and present. It is not made up. The problem is that chronic pain is often caused by types of anatomical problems that are difficult or impossible to diagnose using standard medical tests, and pain cannot be diagnosed like other medical problems (such as a broken bone that can be seen on an X-ray).

Fortunately, most in the medical community are now trying to understand and appreciate that chronic pain is real and needs to be treated and managed differently.

2. Pain is not the only problem - it breeds other health problems
Thoughts and emotions related to the pain also can come into play and aggravate or alleviate the pain. For example, depression, which is a serious disease, can worsen the pain. Sleep problems again caused by the pain, can also make the pain worse. And increased pain usually leads to increased sleep problems.

Often all conditions related to the pain need to be treated concurrently in order for the patient to get any relief.

3. Pain is deeply personal
Everyone experiences and expresses pain differently. Any two people with the exact same health condition are likely to feel and express their pain in unique ways depending on a number of factors. Newer chronic pain theories now have physiological explanations for how and why people experience pain differently.

When it comes to back pain, this is especially true. Two people can have the same type of herniated disc, but one feels only slight discomfort and the other feels intense, burning pain that is unresponsive to conventional treatment. It is also not uncommon that no anatomical cause of the pain can be detected.

Why is this point important? It means that chronic pain often needs to be treated as the primary problem, which is different than the conventional medical approach of identifying and treating the underlying problem causing the pain.

4. Chronic pain is its own beast
Unlike acute pain, which functions as a warning signal (e.g. I just stepped on a nail - better move my foot!), chronic pain does not have any useful function. It just is.

Often, chronic pain is caused by nerves that continue to send pain signals to the brain. When dealing with chronic pain, often one of the most frustrating things is that there is nothing to 'fix'. It just exists in your body.

5. Chronic pain is LONELY
After awhile, many people with chronic pain - especially pain that is caused by a condition that cannot be seen - begin to feel isolated. Here the Internet has done a world of good helping people in pain connect with others in similar situations and find a supportive peer group through online communities of people in similar situations.

Having a clearer understanding of how chronic pain works, as well as the central role that the mind plays in the experience of chronic pain, is becoming more mainstream in the medical community. Patients who start to gain more understanding of their own chronic pain may also benefit in terms of gaining increased emotional support, more effective and sustainable pain management, and even possibly harnessing the power of their minds to assist in coping with the pain.

hello sir I want to know how much milk is enough for a man (age 20,2000 calories) and I recently came to know that we shouldn't eat pure ghee with other oils ,is it true?

One cup of milk everyday is enough for normal adults. You should eat pure ghee because it is healthy, but only in limited amounts. And other oils/ ghee such as dalda, cotton seed oils, etc should be avoided as much as possible. Sunflower oil, safflower oil, olive oil, etc are healthy oils and a better choice.

I am suffering from migraine, wat are the diagnostics and medicines I have to use.

migrane can be managed with lifestyle changes and medications, try muscle relaxation exercises. Relaxation may help ease the pain of a migraine headache. Get enough sleep, but don't oversleep. Get an adequate amount of sleep each night. It's best to go to bed and wake up at regular times, as well. Rest and relax. If possible, rest in a dark, quiet room when you feel a headache coming on. Place an ice pack wrapped in a cloth on the back of your neck and apply gentle pressure to painful areas on your scalp. It is very important keep a headache diary. Continue keeping your headache diary even after you see doctor. It will help you learn more about what triggers your migraines and what treatment is most effective. A dose of riboflavin in appropriate dose as prescribed by me, also may prevent migraines or reduce the frequency of headaches. Coenzyme q10 supplements may decrease the frequency of migraines, magnesium supplements may also be used to treat migraines do reply back for medication prescriptions. Being also a general physician and registered dietitian, I prescribe both evidence based herbal as well as allopathic medicines complementing with dietary guidelines and home remedies carefully personalized for each individual patient.

I have deviated nasal septum (Dns) So doctor has suggested me 3 surgeries. What should be the combined cost of these 3 surgeries? 1.Septplasty 2.Radiofrequency nasal Turbinate reduction 3.Balloon dilatation of Sinuses And is it necessary to do all the 3 surgeries? And do I need to stay for 3 days in the hospital for these surgeries? Is it possible that in a day I can go home?

MBBS, Diploma in Otorhinolaryngology (DLO)
ENT Specialist, Bangalore
The problem is that Indian Insurance Companies have not woken up to the fact that medical technologies have come a long way in the last 60 years! Thus they insist on 24 hour hospitalisation for any surgery! If I were to do the above surgeries, and you were a cash paying patient, I would discharge you within 3 hours after surgery! If it was balloon sinuplasty along with RF assistedTurbinate Reduction, I can do it as an Office case! Under local anaesthesia in my clinic chambers, so no OT required! and you can go back to work immediately after the surgical procedure! However Septoplasty does require hospitalisation but can be discharged within 3 hours of surgery, however it also required daily follow-ups at my clinic to clean the nose so as to keep you comfortable for about 1 week.

Every night I have erection while m sleeping. After when I walk up. I check my urethra. Some fluid discharge from it which is sticky and colour less . Is this normal?

BHMS
Homeopath,
Every night I have erection while m sleeping. After when I walk up. I check my urethra. Some fluid discharge from it ...
Every person experience an erection after a sleep of 6-8 hours. Such an erection during waking up from sleep is a typical characteristic of normal sexual system. An uninterrupted sleep of 6-8 hours pacifies the nerves which get hyper-activated after a long day's work & results in a sexual arousal by channelising the blood flow towards the male sexual system. However, the sticky fluid which you are experiencing is the secretion of prostate gland. This phenomenon is clinically called retrograde ejaculation. It is quite normal.

My mom is 58 years old. Past few days she's been complaining of a continuous drum like sound from her right ear. Also she's been feeling dizzy every morning as she wakes up from bed. Our physician prescribed her some medications (vertin 16 mg twice a day) and exercises for vertigo. But it is not helping much. Is it an ent problem (inflammation in inner ear?)?

PG Diploma in Emergency Medicine Services (PGDEMS), Bachelor of Ayurveda, Medicine and Surgery (BAMS), MD - Alternate Medicine

This is because of hypertension. So her blood pressure should be controlled. And start 1-1 drop of cow ghee in both nostrils at night time before sleep. And take ayurvedic medicines like brahmi. Smritisagar ras. And sarpagandha vati morning and evening.

Now a days due to change in lifestyle of people heart problems are increasing, like sedentary life, excess of junk food, intake of oily fatty food, lack of exercise, drinking alcohol, smoking, stress. Previously the blood pressure should be in control, reduction of weight if overweight, eating nutritious diet including fruits, salad.
