Hello, babies born before 37 weeks of gestational age are called pre mature babies. They are, of course, smaller than full term babies, but many are born without their organs having had enough time to fully develop. Some of these babies show obvious disabilities. They need a considerable medical care to survive. Mature babies are born after the completion of 37 weeks of pregnancy. Thanks.

Normally children will speak 2 to 3 word sentences at that age. Slight delay isn't a problem. If the height is around 87 cms, 2 cms more or less it's normal.

Diagnosis and Treatment of Valvar Aortic Stenosis in Children

FSCAI (Int Card), FACC (Card), FACP (Int Medicine), MBBS
Cardiologist, Delhi
Diagnosis and Treatment of Valvar Aortic Stenosis in Children

Valvar aortic stenosis, commonly called aortic stenosis, is a disorder which occurs when the aortic valve of the heart becomes narrow (know Main Indicators of a Heart Disease in a Child). The narrowing of the valve prevents its full opening, and hence, blood flow from the heart to the aorta is restricted. When the aortic valve gets obstructed, the heart has to exert much more effort to pump the blood. The heart muscles get weakened as a result. This condition is more common in children.

Diagnosis of Valvar Aortic Stenosis:
Before diagnosis, we should know about the symptoms of valvar aortic stenosis in children. They include:

  • Feeling breathless
  • Angina or chest pain with a feeling of pressure or tightness
  • Syncope of fainting
  • Palpitations and enhanced heartbeats
  • A steady decline in regular activities and energy levels
  • Fatigue due to little exertion
  • Not gaining weight
  • Poor eating patterns
  • Problems in breathing

The wall of the left ventricle also thickens muscularly, and the thick wall occupies more space in the lower chamber of the heart and hence, the room for adequate blood supply is reduced. This may lead to heart failure.

Echocardiogram: The initial test that is recommended for patients with symptoms of valvar aortic stenosis is Doppler Echocardiography. This test enables the doctor to estimate the aortic valve region, peak or mean transvalvular gradients and the maximum aortic velocity. These primary measures are required to assess the severity of the disease. Echocardiography provides important information on the valve function, left ventricular filling pressure and disruptions in other valves.

Other major tests which help in the diagnosis of valvar aortic stenosis include:

  • MRI or magnetic resonance imaging, which provides images of internal body structures with great detail.
  • CT scan or computed tomography where three-dimensional images are extracted.
  • Chest X ray
  • Cardiac catheterization where a dye is used to highlight blockages, if any, in the heart.

Treatment: Usually, there are no medicines for fixing valvar aortic stenosis as the disease is not reversible. Medicines can be used to treat the problems, which are caused by the condition.

Replacement of the damaged valve by surgery is the best treatment for valvar aortic stenosis. The surgery performed is called valvuloplasty. This is a cardioscopic surgery where a tube with a small balloon is inserted into a vein. The tube is guided into the heart, and the balloon is inflated. The balloon and tube are removed after the valve is opened. The damaged valve is replaced by mechanical valve or the valve of a cow or pig by an open-heart surgery.

Valvar aortic stenosis is a severe heart condition in children, and the only permanent remedy for this disease is a valve replacement surgery.

Hello, Baby cry when being put down on bed, being separated from her caregiver makes her feel that some thing is  different going on depriving her the affection of her mother. ▪how can she really ignore that she is being put down from the lap of her mother as she misses the softness of nature & body of her mother •smell of the body •breathing chest of her mother •the affectionate motherly touch as well. •here begins the  feeling of insecurity for the baby. ●Baby, misses  the temperature of the body of her mother waking her up while being put down. Henceforth, Baby should be wrapped up in a blanket while putting her down keeping her body warm. ●baby is not accustomed to pee or poop what she has to learn in due course of time so it's a natural phenomenon for an infants to cry while passing urine or stool. Some babies do have irregular motions. If the stools are very hard and if the baby struggles to pass stools then you must show to a pediatric surgeon. 7.75 kgs at 3 and half months is quiet high. Don't give any thing other than breast milk. Calcium is not a must but you can give vitamin d drops1 ml (400mg) daily as it's less in breast milk.

Learning Disabilities results from a difference in the way a person's brain is "wired" Children with learning disabilities are as smart or smarter than their peers. But they may have difficulty reading, writing, spelling, reasoning, recalling and/or organizing information if left to figure things out by themselves or if taught in conventional ways.
A learning disability can't be cured or fixed; it is a lifelong issue. With the right support and intervention, however, children with learning disabilities can succeed in school and lead successful lives, often distinguished careers later in life.
Parents can help children with learning disabilities achieve such success by encouraging their strengths, knowing their weaknesses, understanding the educational system, working with professionals and learning about strategies for dealing with specific difficulties. This condition affects the brain's ability to receive and process information. This disorder can make it problematic for a person to learn as quickly or in the same way as someone who is not affected by any such issues. First step in helping such brilliant minds is understanding without judging.

Your baby might be suffering from eczema which is a skin condition associated with extreme dryness. This condition slowly improves with time mostly by 2 years of age. Please use thick moisturizing creams like Oilatum for her skin and also you can apply coconut oil as home remedy. Although, see a Paediatrician once to confirm the condition.

I have two preterm daughters advice the age till which they will need exclusive breast feeding. Please tell.

Diploma in Child Health (DCH), MBBS
Pediatrician, Seoni
Hi if you ask exclusive breast milk then it is 6 months after you should start weaning. You have to gradually discourage breast milk because after 6 months water content is increase in breast milk nd baby did not get adequate calories. Up to 2 years we can cont. Breast feed but frequency should be less.

I need to know can I travel by air with my 7 month old baby. is it safe for the baby.?

MBBS, Diploma in Child Health (DCH), Pediatric Gastroenterology
Pediatrician, Delhi
Yes it is ok. If the baby has a cold and blocked nose, the ear pain may be severe. In that case, it is good to instill prophylactically one drop in each nostril of nasoclear saline nasal drops and continue feeding the baby during landing and takeoff.
