Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. : keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Please do morning walk daily. Take a glass of milk daily. Check your blood pressure once a week. Stay in sunlight at least 30 minutes daily. Take mag phos 6 x - twice daily for next 2 weeks and get back to me for further treatment.

Hello. Maintain a right standing and sitting posture, keep your back always straight, avoid carrying weight on back. Hold or carry weight divided equally in both the hands or shoulders (to avoid over straining on one particular hand or shoulder). Lie down on bed on your back without pillow. Exercise: Practice Shavasana or Corpse Pose as it looks like a dead body. If practiced correctly and with full efforts, can relax every body part, relaxing not only the body but also the mind: Spread the legs one to two feet apart, the toes are turned outwards, the heels facing each other, a comfortable distance apart. Bring the arms a little away from the body, palms turned upward. Relax the neck and allow it to turn to the side if it is more comfortable. Close the eyes and focus the attention on the body, breathing normally.

Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

Treatment for lower back pain depends upon the patient's history and the type and severity of pain. The vast majority of lower back pain cases get better within six weeks without surgery, and lower back pain exercises are almost always part of a treatment plan. Here are some tips for how to handle your back pain: -stop normal physical activity for the first few days. This will help relieve your symptoms and reduce any swelling in the area of the pain. -Apply heat or ice to the painful area. One good method is to use ice for the first 48 to 72 hours, and then use heat. Take pain relievers medicine. --While sleeping, try lying in a curled-up, fetal position with a pillow between your legs. If you usually sleep on your back, place a pillow or rolled towel under your knees to relieve pressure. -Find a mattress with back support. A good mattress should provide support for the natural curves and alignment of the spine. Medium-firm mattresses usually provide more back pain relief than firm mattresses. -Do not perform activities that involve heavy lifting or twisting of your back for the first 6 weeks after the pain begins. -Avoid bending forward and sitting in a place for long time -Good posture plays a key role in your spine health, helping to prevent additional back and neck pain and even injuries to spinal structures. Try to be aware of your posture and make good posture part of your normal routine. -head up, shoulders back, rear tucked under -Stand and walk with correct posture -Sit with support -Lift and carry carefully -And finally, keep moving:-Maintaining a static position for too long—more than twenty or thirty minutes at a time—is hard on the back. Prolonged static posture can slowly deplete elasticity from the tissues. By simply moving—be it a change of position, a stretch, or a short walk—the tissue elasticity needed to protect your joints can be restored and the associated discomfort reduced. -Stretching and strengthening exercises are important. But, starting these exercises too soon after an injury can make your pain worse. -Some Physiotherapy treatment like IFT current can give you relief. -Consult a Physiotherapist for detail assessment and management of your back pain. Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. An anti-tetanus serum is not required in routine injuries. It is only recommended in an intensively crushed and dirty wound, in which case it is given within the first six hours after the injury. A routine tetanus toxoid is also not necessary if one is previously fully immunised. In case one is not, then the toxoid is administered within the first 24 hours after the injury.

Identify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. I do not think that the pain is due to sitting on floor and or peddling the sewing machine. It could be due to any vaginal infection/injury/infection. Please take a note if there is any eruption on the area or not. Take Belladonna 30 - twice daily for pain when required.
