Hi doctor I have been suffering from diabetes for the last six years but despite it I keep fast in the month of Ramadan (Ramzan. 1 week ago I felt low back pain that was unaffordable. My doctor suggested me to not to keep fast and said keeping stomach empty for diabetes patients may cause of increase of sugar level. I just want to know can I keep fast or not.

Dear lybrate-user, best wishes for the holy month of ramadan! you are very young and your diabetes should not be a problem for observing fast, in ramadan this year days are very long and hence dose of medicines need to be adjusted so well that you should not experience low sugar. You can avoid medication for some time by taking precautions. Avoid sugar, fried food, junk food and fruit juice. Exercise regularly and keep monitoring every six months.

Yes worm infestation is a common problem in children. I would suggest the following: 1. Repeat deworming dose every 2-3 weeks for 4-6 courses 2. Personal hygiene measures eg. Daily bathing, clean & soft towel, clean & loose clothes, clean socks (even if shoes are not worn), clean toys as the child has a tendency to put toys in the mouth, brushing of teeth, hand washing / sanitizer use & so on 3. Good nutritious food 4. In view of the itching, massage of the skin in general & area around the anus in particular, with oil / cream of your choice will be helpful 5. Periodic check by your paediatrician for this as well as immunizations and preventive & promotive health is a good idea.

1. Do regular exercise for 30 minutes daily like brisk walking 2. Reduce weight if overweight 3control diet a) avoid sweets, sugar/honey, milk, milk product excess calorie and carbohydrate rich diet like white bread, cereal, rice, pasta b) avoid all refined food like maida, pasta, starchy food like rice, potato, bakery item, processed food, c) include more fiber like pulse, green leafy vegetable, cucumber, carrot, tomato, j legume. D) avoid sweetened fruit like banana, mango, litchi, chickoo, sapota, seethaphal jack fruit, grape etc. Can have fruits like guava, pineapple, papaya, orange, mousambi, strawberry, watermelon, pomegranate, jack fruit, sweet potato, a handful of walnut daily (akhrot) e) avoid white/polished rice, take unpolished rice like brown rice/wheat/jowar/ragi, moong and chana dal. F) avoid ghee/ butter./coconut oil/excess salt intake like papad/pickle. G) avoid red meat. Take fish/chicken. H) avoid alcohol and carbonated drinks like soft drink 4. Adhere to a strict diabetic diet by consulting dietitian 5. Monitor your fasting, pp blood sugar 3 monthly and hba1c six monthly 6. Consult diabetologist for advice and treatment.

I will recommend you homoeopathic treatment along with your medicines and few changes in your daily routine with which your blood sugar, thyroid nd excess weight will come down. Diabonil (sbl) 20 drops in 20 mlfresh water 15 minutes before meal uranium nitricum 1x (wilmar schawe india) 2 tab twice daily direct in mouth nd chew eat little at a time every 2-3 hourly. Don? t eat too much at a time brisk morning walk for 30 minutes regularily dinner 2 hours before goin to sleep 10 minutes evening walk after dinner avoid sweets, mango, grapes, colddrinks in particular start doing kapalbhati pranayam daily for 15 minutes but start from 5 minutes daily if you r first timer. O inhale deeply trough your nose, this will expand you abdomen. O exhale using a great force so that your abdominal muscles contract to the maximum o these exhalations are similar to your breath outs, but they should be forceful o make sure while exhaling, the air should be pushed out of your lungs by contracting the diaphragm o after exhalation again do inhalation, but this time with out any effort. To do this just relax your lungs, they will automatically expand and fill with air o one cycle of kapalabhati pranayama include one deep inhalation, then forceful exhalation and an effortless inhalation. O a learner can practise up to three rounds, each with 15 exhalations. Small breaks between each round are allowed report after 3 months.

Mr. lybrate-user, If she has diabetes (Type 2), she has to be appropriately treated with specific medications. To advise medications I need to know fasting, PP glucose levels, HbA1c%, weight, height, extent of daily exercise being done and dietary pattern. Plus if she is on treatment, then drugs and dosage of the same. It will be better if you come in a private conversation with all these details, then I can give specific prescriptions and necessary advise. Mr. lybrate-user, You are right that there should be more research in the field of diabetes, and it is happening. The only difference between various diseases which you have mentioned and diabetes is that those are all infections some caused by bacteria and some by viruses. Infections can be cured by using drugs like antibiotics and anti -virals. Plus vaccination so that our body makes antibodies against the invading organism and fights them. But diabetes, hypertension, hypothyroidism, hyperthyroidism are part of non-communicable diseases. Some of them particularly diabetes and hypertension are directly linked with the lifestyle a person follows. T1 diabetes is is an autoimmune disease where our own immune cells produce antibodies against Beta cells which secrete Insulin, destroying them. This leads to diabetes which has to be treated only with insulin injection that too for the whole life. In 1921 Insulin was discovered, which is now helping us save life of scores of diabetic patients. T2 diabetes is increasing mainly because of increasing excess eating, taking in of lot of junk foods, inactivity, indulgence in alcohol and smoking. There is continuous research going on to find out how the patients can be helped. It wrong to say that doctors are not interested in curing the disease and want to make money. Enough efforts are being made. Infact increasing awareness about the disease and change in life style will help us in redcucing diabetes. Thanks.

Please repeat your thyroid test to know if your medicine is helping you. Any medicated sunscreen with spf higher than 30 will help you. A good multivitamin with biotin will help you.

U have to improve your food habits do 1. Take 2/ 3 glass of warm water in the morning before brush 2. Take more water in day 3. Take meals at fixed hrs, chew food properly/ completely, no eating quickly 4. Take small amount of food at a time, take more frequent meals - may be five times a day. 5. No spicy/ fried/ fast/ junk/processed food. Caffeine, mints or mint-flavoured foods, pungent, sour things, tomato-based foods. 6. No smoking, chewing gum, and carbonated beverages. 6. No milk for few days, can take curd, no uncooked salad 7. Avoid constipation 8. Use nibu pani (lemon water) 2/ 3 times a day 9. Do not drink/ store water in plastic bottles. 10.relax and walk for 30 mts daily 11. Take enough rest- do not lie down immediately after eating. Take dinner 3 hrs before sleeping. No late nights 11. Dear lybrate user, one of the major nutritional substance required for human body is carbohydrate. We take carbohydrate in the form of rice, wheat, sugar etc. The complex molecules of carbohydrate are broken down into simple glucose molecules during digestion & released in the blood stream to be absorbed by the muscle cells. Muscle cells utilise the glucose molecules to release energy by breaking down those glucose molecules. Insulin hormone plays a vital role in the absorption of the glucose molecules by the muscle cells. In abnormal conditions, either due to less production of insulin or due to resistance to insulin, the process of absorption of glucose by the muscles comes to a stand still. The glucose molecules remains in the bloodstream which results in the increase in the blood sugar level. The failure of utilisation of the glucose molecules by the muscle cells results in production of less energy than normal condition. This is externally manifested as the fatigue & tiredness of the patient. This excess amount of blood sugar compels the kidney to function more than normal level in order to excrete the blood sugar molecules through urine. As a result the patient suffers from frequent urination. This abnormal condition is clinically known as diabetes mellitus. Once a patient is diagnosed with diabetes he has to take regular medicine to keep his blood sugar level within normal limits. She should take homoeopathic mother tincture gymnema sylvestra q, 30 drops, thrice daily, after meals, in a cup of water. Also she need to walk at least 1 hour daily without any interval. Also she need to take a low carbohydrate rich diet. She should avoid rice, sweet dishes, fruits, cold drinks, fast foods, alcoholic beverages, potato etc.

Thyroid problems are not contraindications for feeding. You can continue feeding but some medicines are not advised in lactating mothers.

I have TSH levels of 22, it was >150 a year back which reduced to 22 after regular use of Thyrorich tablets. Currently I m using 175mg per day. My question is, will this thyroid disorder will affect my sex potency?

Mr Dinesh, Hypothyroidism can affect sexual desire as well can cause erectile dysfunction, particularly if not properly treated. A number of studies have shown that either hyperthyroidism or hypothyroidism does lead to ED, premature ejaculation. But once the condition is treated these defects disappear rapidly. Your TSH level is still very high, it should come down to around 4 milli units/ml as you are on treatment. Current dose of thyroxine needs to be increased a bit further to achieve that.

Hye, Thanks for the query.     For a hypothyroid condition there is no specific or special food. And it is best advised to have a diet planned as per the thyroid levels, lifestyle and history. Though you can follow some guidelines - Have small frequent meals. Do not fast for long hours. Avoid a very heavy meal at one time. Avoid fruit juices, aerated drinks or sugarcane juice, honey, jaggery or sugar if having insulin resistance. You just have to avoid raw, unprocessed soya, raw peanuts and raw cabbage. Avoid junk foods. Exercise and stay active. Do not attempt to try out too many remedies as you could go into lowered metabolism. Sleep well.            It's all a matter of discipline.
