Orofacial Pain - Sources and Symptoms

An unpleasant experience which not only disturbs the usual routine of a person but also the psychological peace and causes extreme discomfort is called pain. When this pain involves the region above the neck, below the orbitomeatal line and in front of the ear is called as orofacial pain.

Multiple causes for orofacial pain may exist and the symptoms may include such diverse findings as headaches, neck pain, ear pain, dental pain, facial burning or stabbing sensations, and jaw joint pain.

The complaints may either develop gradually or have a rapid onset and can originate from musculoskeletal, neurovascular or neuropathic causes.

Sources of orofacial pain may include:

  1. Trigeminal Neuropathy – ant damage or degeneration to the trigeminal nerves that supplies mainly to the facial structures causes the orofacial pain. This is commonly referred as trigeminal neuralgia.

  2. Neurovascular Disorder – migraine is the most common neurovascular disorder that causes the radiating pain to the mouth eyes and other facial muscles.

  3. Temporomandibular Disorder – the major cause of orofacial pain is the temporo mandibular joint disorder which causes pain in the lower jaws. Orofacial pain may also be caused by any temporo mandibular joint surgeries and due to failure of joint replacement surgery of the jaw.

  4. Burning Mouth Syndrome – this condition is caused due to nerve damage, lack of adequate saliva production, fungal infection, certain medications and also noted for patients with diabetes. Orofacial pain is also noted in burning mouth syndrome.

  5. Cervicalgia – pain due to spinal cord injury, damage to spinal nerve, muscle and ligaments may radiate to face and cause orofacial pain.

  6. Trauma – accident is the most common cause of temporomandibular disorder and other spinal disorder that causes orofacial pain.

  7. Sleep disorders – sleep disorders include grinding or clenching of the teeth during sleep may also cause orofacial pain.

Symptoms

Though orofacial pain is caused by several factors, its symptoms includes dental pain, ear pain, neck pain, headache, pain in the jaw joint and stabbing sensation over the face. Some patients may also experience burning sensation over the face. Other symptoms include dizziness, insomnia, tinnitus (abnormal ringing or roaring sound in ears), poor muscle coordination, abnormal itching over the face and neck and tingling sensation. If you wish to discuss about any specific problem, you can consult a Dentist.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Actually your issues can be either due to diabetes or spine, it is difficult to actually say without examining you in detail. But if there is no back pain issues, then it may be unlikely due to a spinal pathology. Anyway a proper examination and investigations can actually tell us whether the issues are due to spine or due to diabetes. We can initiate with medications for neuropathic symptoms like pregabalin or gabapentin in small doses with or without vitamin b12 supplementation for 2-3 weeks and see from there how it goes. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

Having a neck pain since 4-5 year's. Plz suggest me what can I do to free from neck pain???

Having a neck pain since 4-5 year's. Plz suggest me what can I do to free from neck pain???
Hi lybrate-user, Neck pain is probably one of the most discomforting of all pains, that’s probably why the phrase ‘pain in the neck’ has become one of the most used ones in the English language. 1. Ice Pack - Ice cubes have a cooling effect that soothes the painful areas. The area that is cooled by an ice pack gets better blood circulation, especially the underlying tissue and muscle, which helps get relief from the neck pain. Applying a cold compress helps bring down the temperature surrounding the neck area and causes a temporary numbness that helps get immense relief from the pain almost instantly. It also prevents the blood from clotting in the region and helps gain immense relief. An ice pack is one of the best home remedies for neck pain. 2. Neck Exercises - Giving regular exercise to the neck muscles helps remove the stiffness in the neck region and helps get relief from neck pain. In addition to strengthening the muscles in the neck region, exercising helps enhance blood circulation as well. While strengthening the neck muscles, exercising simultaneously works on the back and shoulder muscles, and removes all stiffness and helps get relief from the pain. Neck exercises are one of the easiest home remedies for neck pain. 3. Cayenne Pepper - Applying Cayenne pepper powder on the affected areas inhibits the Substance P which transmits the pain signals to the brain. When these signals are stopped, the feeling of pain is subdued. Cayenne pepper powder is hot in nature and should be applied only on the areas where there is severe pain. If applied on non-painful areas it can cause irritation. Cayenne pepper powder is one of the best home remedies for neck pain. 4. Ginger - Ginger is a wonder herb root that has amazing medicinal properties. Apart from being a very effective antibiotic, it also has excellent anti-inflammatory properties. It has been used in folk medicine for ages for treating cough, colds, stomach disorders and aches and pains in the body. Moreover, ginger enhances the blood circulation and promotes blood flow in the affected areas to give instant relief from the neck pain. Ginger is the best natural neck pain treatment at home. 5. Turmeric - Turmeric is a wonder medicinal spice that is very potent and has excellent antioxidant and antibacterial properties. Ginger is also a great anti-inflammatory agent that helps reduce the swelling and also helps by easing the pain in the neck. The curcumin content in Turmeric gives it healing properties that help keep neck pain in control. Thus Turmeric makes an excellent home remedy for neck pain. Hope this helps. Good Luck.

Treat Rheumatoid Arthritis and Osteoarthritis with the Help of Ayurveda

Treat Rheumatoid Arthritis and Osteoarthritis with the Help of Ayurveda

With age we often experience gradual wear and tear of our body parts, which leads to slow down of body functions. Wear and tear of bones and joints often make movement difficult in various parts, such as hands, hips, legs, knees, ankles, etc. Rheumatoid and osteoarthritis are a two serious and common conditions where wear and tear of the bone is significant. The pain and stiffness in the affected joints vary, but for most people, restricted movement is the main issue. The relieving news is that there are effective, natural methods in Ayurveda which are tested over centuries and used even to date.

These methods aim at improving stiffness, pain control, controlling progression of inflammation, reversal of damage (if not severe), improving range of motion, improving strength and improving overall energy levels.
Ayurveda believes that body’s health is managed with the balance between three vital elements that is vata, pitta, and kapha. Arthritis (both osteo and rheumatoid variety) is believed to be related to vata and arthritis is also known as sandhivata (sandhi meaning joint).

Treatment includes overall body treatment and focused treatment of the affected joint. Oral medicines, oils and external applicants, detoxification and rejuvenation through panchakarma, diet changes, healthy lifestyle are all part of the treatment regimen. Oil and external application on the overall body and affected joints will help lubricate the joints and will allow easy movement. This will help reduce stiffness and improve range of motion.

Dietary changes: Reduce fats, sugar, salt and alcohol. Include a lot more of fish, soy, olive oil, low-fat dairy products, broccoli, green tea, citrus fruits, whole grains, beans, garlic and nuts and seeds. In addition to protein, these have various ingredients, which are useful for bone health and act as anti-inflammatory agents.

Some common panchakarma treatments include:

  1. Choorna Kizhi, where the person is made to assume different positions and mixtures of various herbs are applied to make the person sweat profusely.
  2. Abhyangam is a special oil massage lasting 45 minutes that the patient undergoes for 14 days. In addition to arthritis, it also helps control body ache, fight obesity, revitalises the whole system, induces sleep, and helps fight stress.
  3. Njavarakizhi is similar to the earlier Choorna Kizhi, various oils are used to massage the whole body while the patient is made to sit in different positions. There is also a Njavara rice that is cooked in milk to make a porridge and fed to the patient. This again is continued for 14 days.

The key to managing this condition is to seek help in as early stages of the disease as possible. When there are risk factors that put you at risk, reach out to seek medical help as soon as you realise onset of symptoms.

Back pain inside the back bone, right hand side. 24 hour continuously. please suggest.?
Hi Thanks for your query and welcome to Lybrate. I am Dr. Akshay from Fortis hospital, New Delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - To maintain proper posture of your back while working and sleeping - If pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - Physical therapy initially under supervision of a trained physiotherapist and then to continue at home - Adequate calcium & vitamin d intake if levels are low in body - Ice packs can applied if your pain is acute, then hot fomentation can be done at home - Analgesic spray for local application can be used and is easily available. - Avoid lifting heavyweight/ acute forward bending in mornings etc We will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation. Thanks & Regards Dr Akshay Kumar Saxena Consultant Orthopaedics Fortis Hospital, New Delhi

Spinal Cord Injury: Know Its Causes & Symptoms!

FRCS - Neurosurgery(UK), M.Ch - Neuro Surgery, MS - General Surgery, MBBS
Neurosurgeon, Ahmedabad
Spinal Cord Injury: Know Its Causes & Symptoms!

A spinal cord injury is a damage to the spinal cord. It’s an extremely serious type of physical trauma that’s likely to have a lasting and significant impact on most aspects of daily life.

The spinal cord is responsible for sending messages from the brain to all parts of the body. It also sends messages from the body to the brain. We are able to perceive pain and move our limbs because of messages sent through the spinal cord.

If the spinal cord sustains an injury, some or all of these impulses may not be able to ‘get through’. The result is a complete or total loss of sensation and mobility below the injury. A spinal cord injury closer to the neck will typically cause paralysis throughout a larger part of the body than one in the lower back area.

Causes-

A spinal cord injury is often the result of an unpredictable accident or violent event. The following can all result in damage to the spinal cord:

  • a violent attack such as a stabbing or a gunshot
  • diving into water that’s too shallow and hitting the bottom
  • trauma during a car accident (specifically, trauma to the face, head and neck region, back, or chest area)
  • falling from a significant height
  • electrical accidents
  • severe twisting of the middle portion of the torso

Symptoms-

Some symptoms of a spinal cord injury include:

  1. Problem in walking
  2. Loss of control of the bladder or bowels
  3. Inability to move the arms or legs
  4. Feelings of spreading numbness or tingling in the extremities
  5. Unconsciousness
  6. Pain, pressure, stiffness in the back or neck area
  7. Signs of shock
  8. Unnatural positioning of the head

If you suspect that someone has a back or neck injury:

  • Don’t move the injured person – permanent paralysis and other serious complications may result
  • Keep the person still
  • Place heavy towels on both sides of the neck or hold the head and neck to prevent them from moving until emergency care arrives
  • Provide basic first aid, such as stopping any bleeding and making the person comfortable, without moving the head or neck

Prevention-

Because spinal cord injuries are often due to unpredictable events, the best you can do is reduce your risk.

 Some risk-reducing measures include:

  1. always wearing a seatbelt while in a car
  2. wearing proper protective gear while playing sports
  3. never diving into water unless you’ve examined it first to make sure it’s deep enough and free of rocks. Consult an Expert & get answers to your questions!
