I am from mumbai and require to ask one query about my dad. My dad have got a paralytic attack on his back head and even he is suffering from sponderlysis. He is not able to walk precisely and not able to live a life like common man as he is completely loosed his confidence level. Can you help me

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. By paralytic attack, do you mean cva or some clot in brain types? I will advise seeing his condition to consult a neurophysician, some medications for strengthening like vitamin b12 supplementation and one for brain can be given but mainstay will be intensive physical therapy under supervision of a trained physiotherapist. Adequate calcium and vitamin d supplementation to maintain bone strength should also be given because he is not mobilizing much which may make his bones osteoporotic. Do not hesitate to contact me if you need any further assistance.

I Had acute embolic infarct left side stroke. Narrow escape because of immediate hospitalisation. Please advise for future life.

MBBS, MD - Medicine, DM - Neurology
Neurologist, Ghaziabad
To prevent stroke in future, management of risk factors of stoke should be done. The reversible risk factors of stroke are ? Being overweight or obese ? Physical inactivity ? Heavy or binge drinking ? Use of illicit drugs such as cocaine and methamphetamines ? High blood pressure ? Cigarette smoking or exposure to secondhand smoke. ? High cholesterol. ? Diabetes. ? Obstructive sleep apnea ? a sleep disorder in which the oxygen level intermittently drops during the night. ? Cardiovascular disease, including heart failure, heart defects, heart infection or abnormal heart rhythm. Management of above conditions and lifestyle modification can reduce the risk of stoke significantly.

I am a 21 years old girl. I am suffering from migraine. When I take some stress a strong pain starts. It makes me irritating. From a month I am not getting proper sleep also. Is there is any connection between this?

Bachelor of Ayurveda, Medicine and Surgery (BAMS)
Ayurveda, Udupi
Migrane is one of the common cause for head ache. The exact cause of migraine is not known. Meaning that, it is not known why some people are prone to migraine-related headaches and some others are not. In people with migraine complaints, it is observed that there are usually some triggering factors that bring on the migraine headache. It is advisable to avoid known trigger factors, irregularity in meals, and physical and emotional stress. Diet management is one of the key factor in migraine treatment. Regulate the diet in such a way that food is easily digestible, does not cause any burning sensation. Medication: regulate the digestive level by syp himcocid 2tsf-0-2tsf before food once digestive level is fine treatment can be planned accordingly.

From what you have described, it is likely your father is suffering from a condition called REM behaviour disorder, a sleep disorder in which a person enacts out their dreams. Since it may be associated with underlying neurological conditions, he needs to be examined in detail by a neurologist and evaluated. He may require a sleep study also.

Dear, many people become forgetful as they become older. This is common and is often not due to dementia. There are also other disorders such as depression and an underactive thyroid that can cause memory problems. Dementia is the most serious form of memory problem. It causes a loss of mental ability, and other symptoms. Dementia can be caused by various disorders which affect parts of the brain involved with thought processes. Most cases are caused by alzheimer's disease, vascular dementia, or dementia with lewy bodies. Symptoms of dementia develop gradually and typically become worse over a number of years. The most important part of treatment for dementia is good-quality support and care for the person with dementia and for their carers. In some cases, treatment with medicines may be helpful. Please consult a physician first.

I recently read that researchers believe certain foods might cure Alzheimer’s disease. Is this true? If so, which foods do this and how much would you have to consume to get the benefits?

PDDM, MHA, MBBS

Several epidemiological studies show that eating a Mediterranean diet dramatically reduces the risk of cognitive impairment and Alzheimer’s disease. That means plenty of vegetables, beans, whole grains, fish, and olive oil—and limited dairy and meat.

When to Seek Medical Help for Epilepsy

MBBS, MD - Psychiatry
Psychiatrist, Mumbai
When to Seek Medical Help for Epilepsy

Epilepsy is a form of chronic disorder and it is characterized by recurrent seizures. The episodes of epileptic seizures may differ from person to person. These seizures could be a result of genetic disorder or a result of trauma or stroke. During a seizure, a patient may also experience symptoms of neurological disorders and sometimes lose consciousness.

Medical help for epilepsy
Epilepsy itself cannot be cured using medication, but proper medicines help in eliminating recurrent seizures. These medicines stabilize the electrical activity within the brain preventing seizures.

How effective is the medication for epilepsy?
The success of controlling seizures using medicines depends on the type and severity of the epilepsy. Medicines for epilepsy are usually very effective and may fully keep seizures under control. However, controlling seizures caused due to brain problems may be more difficult. Usually, epilepsy medicines can control seizures for a long period of time when they are taken regularly.

When is medical help needed?
The decision about when to start medicines for epilepsy is a tricky one. This is because a first seizure cannot confirm whether a person has an on-going epilepsy problem. A second seizure may occur after many years or may not happen at all. Prediction of seizures is also quite difficult.

The severity of seizures also indicates when to start medicines for treatment. In case a first seizure is quite severe, medication should be started at once. Some people have very mild seizures even though they may be recurring in nature, and medication can be avoided in this situation.

All the pros and cons must be thought over before starting medications for the treatment of epilepsy. It is advisable to wait for a second seizure and then start medications for treatment. In most cases, medication is started after a second seizure occurs, twelve months within the first seizure. You should always consult a doctor to know when you need to start taking medicines to treat the condition.

For making the most out of the medicines to control seizures, you should follow certain steps:

  1. You must take medications exactly as your doctor has prescribed.
  2. Before switching to generic versions of your medicines or before taking other prescribed medicines, you must consult your doctor.
  3. You should never stop taking the medicines.
  4. In case you experience enhanced depression, mood swings and suicidal thoughts, you should talk to your doctor immediately.
  5. In case you have migraine, you should let your doctor know so that he can prescribe you anti-epileptic medicines, which also prevent migraines.

Medicines cannot treat the underlying cause of epilepsy, but these help in controlling seizures and this is the most common symptom of epilepsy. Medication should be started at a proper time and must be continued without stopping.
