WHEN SHOULD THE CATARACT BE OPERTATED?

Total or partial opacification of transparent lens of the eye is termed as cataract. Such opacification essentially occurs in old age but it can occur in children or young age due to diabetes, trauma, congenital causes, use of steroid drops in the eyes or secondary to glaucoma.

Cataract affects vision depending upon its degree and location. Very slight cataract at the posterior pole can obstruct central vision and cause marked glare blindness or inability to read small letters whereas, cataract in the cortical area may take long time to affect vision.

Treatment: no medicines can cure cataract. Once it occurs, it has to be operated.

When to get operated?

When you are professionally blind or unable to carry out your routine duties/work. A driver or a watch repairer has to operate very early due to glare blindness whereas an old lady can wait for some time. It is ideal to get the surgery done with immature or early cataract than to wait for the cataract to get mature. A mature or hypermature cataract is difficult and risky to operate.

What surgery?

Phacoemulsification with ultrasonic rays is the modern method os surgery. You get only 2-3 mm of an incision, no sutures. The lens is emulsified and aspirated and a foldable IOL (lens) is injected by a syringe which unfolds and occupy the same space as your original lens. 10 minutes surgery with practically no pain. This lens lasts for a life time and gives you youthful vision.

The surgery can be performed even in patients with diabetes or hypertension and at any age. Maybe 90-100. Practically no post-op restrictions.

मोतियाबिंद आंख की एक स्थिति है जिसमें आंखों के अंदर लेंस अपारदर्शी हो जाते है, जिसकी वजह से दृष्टि अस्पष्ट हो जाती है। आयुर्वेद में, मोतियाबिंद को टाइमर या लिंगानाशा के रूप में जाना जाता है, जो उत्तेजित वात दोष के कारण है।
हमारी आंखें प्राकृतिक कैमरे के रूप में कार्य करती हैं। बाह्य स्रोत से प्रकाश हमारी आंख में प्रवेश करता है, और कॉर्निया और आंख की पुतली को पार करने के बाद लेंस पर पहुंचता है। लेंस फिर रेटिना पर प्रकाश को केंद्रित करता है, जहां छवि बन जाती है। मोतियाबिंद होने पर, आंख का लेंस प्रोटीन भंडार से धुंधला हो जाता है, जो लेंस से प्रकाश को गुजरने से रोकता है। जिससे दृष्टि धुंधली हो जाती है। 
यह एक सामान्य बीमारी है। ज्यादातर मोतियाबिंद उम्र बढ़ने की प्रक्रिया का परिणाम है। उम्र से संबंधित मोतियाबिंद के मामले में, लेंस की शक्ति में वृद्धि हो सकती है, जो निकट दृष्टि दोष (मिओपिया) का कारण बनता है। मोतियाबिंद, आमतौर पर दृष्टि हानि पैदा करने के लिए धीरे-धीरे प्रगति करता है। हालांकि, यदि अनुपचारित छोड़ दिया जाए, तो यह पूर्ण दृष्टि हानि पैदा कर सकता है। 
 

मोतियाबिंद के विभिन्न प्रकार हैं:
- आयु से संबंधित
- जन्मजात
- माध्यमिक: ये अन्य चिकित्सा स्थितियों, विषाक्त पदार्थों, पराबैंगनी प्रकाश, विकिरण या कोर्टिकोस्टेरोइड या मूत्रल जैसी दवाइयां लेने के परिणामस्वरूप होती हैं।
- ट्रॉमेटिक: यह आंख को चोट के परिणामस्वरूप होती है
 

मोतियाबिंद का आयुर्वेदिक इलाज
यदि प्रारंभिक अवस्था में इलाज किया जाता है, तो मोतियाबिंद में आयुर्वेदिक उपचार बहुत प्रभावी होता है। आयुर्वेदिक उपचार में उद्देश्य, शरीर की बढ़ती ऊर्जा को कम करना होता है। आमतौर पर मोतियाबिंद के उपचार के लिए निर्धारित आयुर्वेदिक दवाएं हैं: 
1. महा त्रिफला घृता:
महा त्रिफला घृणा की मुख्य सामग्री त्रिपला और गाय का घी है। यह मोतियाबिंद  के लिए सबसे अच्छा उपाय है, लेकिन यह केवल बीमारी के प्रारंभिक चरणों में उपयोगी है। यह आमतौर पर दूध के साथ खाया जाता है। इस जड़ी बूटी के 2 चम्मच की दो बार दैनिक खपत मोतियाबिंद का इलाज करने में मदद कर सकती है।
2. त्रिफला:
यह आमला, बिभीतकी और हरिताकी का एक संयोजन होता है। रात भर के लिये पानी में कुछ त्रिफला पाउडर भिगोएँ। सुबह अपनी आंखों को धोने के लिए इस पानी का उपयोग करें।
3. चंद्रोदय वाटी:
यह रक्त परिसंचरण में मदद करता है और आंसू को बढ़ावा देता है। यह आंख के ऊतकों को पोषण भी प्रदान करता है। इससे पेस्ट बनाने के लिए शहद के साथ मिलाएं और इसे अपनी आंखों पर लागू करें।
 

अन्य प्राकृतिक उपाय और सुझाव जिनका पालन घर पर किया जा सकता है:
1. गरम दूध मे इलायची के दो टुकड़े मिला कर रात मे सोने से पहले पीएँ।
2. लहसुन की 2-3 लौंग खाएं या उन्हें अपने दैनिक आहार में शामिल करें। लहसुन एक अच्छा शुद्धीकरण एजेंट है और आंखों में क्रिस्टलीय लेंस को साफ करता है।
3. गाजर का रस मोतियाबिंद के लिए एक बहुत अच्छा और सरल घर उपाय है। इसे दो बार दैनिक रूप से लिया जाना चाहिए। 
4. अंगूर और संतरे भी मोतियाबिंद को ठीक करने में मदद करते हैं।
5. मोतियाबिंद के लिए एक बहुत ही सरल और लाभकारी घर उपाय किशमिश और खजूर लेना है। बस उन्हें रात भर पानी में भिगोएँ और फिर सुबह में खाएं।
6. बादाम, काली मिर्च, पानी और चीनी का पेस्ट बनाएँ। मोतियाबिंद का इलाज करने के लिए इसकी खपत भी फायदेमंद है।

Symptoms and treatment for Retinal Detachment

My name is Dr. Pandurang Kulkarni. I'm a Vitreoretinal Surgeon and Uveitis Specialist. I'll be talking about Retinal Detachment. A retina is a light sensitive tissue lying at the back of our eye. Light rays are focused on the retina where the impulses are produced and it is carried by an optic nerve to the brain where they are interpreted as images. Healthy and attached retina are necessary for clear vision. The middle of the eye is filled with a liquid called vitreous. It is in contact with the retina. As the age advances, the vitreous may shed and may separate from the retinal surface. Usually, the vitreous separation doesn't cause any problems but sometimes the separation can lead to the formation of breaks in the retina at one or multiple places.

Who is at a risk of developing retinal detachment? People with near sightedness, previous eye surgeries, severe eye trauma, family history of retinal detachment, retinal detachment in the fellow eye and presence of weak areas in the retina. Now, what are the symptoms of retinal detachment? A sudden increase in size or number of floatus, increase in flashes of light, drop in vision and appearance of shadow in the periphery of fluid transmission. The appearance of gray curtails like objects in the field of vision. Retinal detachment is a surgical condition. Retinal detachment is treated by pneumatic retinopathy, Sceral, Vitreo surgery or a combination of sceral and vitreous surgery. People at risk of retinal detachment should see an eye specialist regularly or at the onset of new symptoms which cause a problem in vision.

For prk you will need 4-5 days rest. You can begin your day to day activities after that. Just take care you touch /rub your eyes for 5-6 days. Put medicine as per prescription. You can work on pc 4-5 days after surgery in prk. Sunglasses for atleast one week when you go out. At home usually not required.

For pain take tablet paracetamol 650 mg and I will suggest you to buy a steam inhaler machine of crescent company and take a flow of steam (of plain water only) over the forehead through the inhaler and can repeat the process after eight hours as and when required.

Hello lybrate-user. Redness of the eye is one of the most prevalent problems observed. Redness of the eyes has multiple causes associated with it. To enumerate a few common causes: (1) allergy of the eye. It is usually caused by improper eye hygiene. It occurs with symptoms of pricking, itching, irritation, watering, foreign body sensation and discomfort. There is a tendency to rub the eyes with fingers, handkerchief, cloth, etc. This is not a good habit because it tends to transmit lots of virus, bacteria and dust into the eye. Allergy usually presents with symptoms of watering (clear water), redness and itching of the eye. Thus, eye hygiene remains the principal modality of treatment along with eye drops as a supplement. (2) infectious conjunctivitis: this is usually associated with white-yellowish discharge. There may occur associated pain and sensation of itching, pricking, watering and discomfort. If so, it would be a good idea to go and show to an eye specialist in person. (3) foreign body lodged in the eye. This is more commonly associated with people who drive 2 wheeler vehicles without protection. It occurs more commonly in one eye. Foreign bodies that commonly get lodged are eyelashes, insect wings, small insects, dust particles, iron particles (more common with people working in the iron industry) the solution for a foreign body lodged in the eye would be better provided by an eye specialist at the clinic because it would require immediate removal from the eye and appropriate therapy. (4) raised eye pressure: this is a condition known as glaucoma. It is not so common as the previous mentioned conditions. There is usually associated pain in the eyes. It is commonly bilateral (occurs in both eyes). There is usually a positive family history of glaucoma associated in a majority of the cases. (5) inflammation of the covering layers of the eyeball. This is known as" uveitis" it is also a relatively less common condition. There is associated pain. It would require a thorough eye checkup including the retina-thus a good idea would be to visit the eye specialist in person. (6) increased blood pressure. This has known to cause redness which is also known as sub-conjunctival hemorrhage. The redness in this case tends to be deep dark and usually occurs only in one portion of the eye. In this regard, a complete ophthalmic evaluation and a systemic evaluation is warranted. (7) injury/ trauma to the eye in any form may cause redness of the eyes. This should be taken seriously and assessed in detail by an eye specialist in person. From the symptoms described by you, it seems more likely to be a case of allergy. Maintaining the precautions and hygiene that I mentioned should probably work well for you. You could also use a lubricant eye drop along with an anti-histamine eye drop. (3-4 times daily) also, you might require a complete eye evaluation in order to rule out other causes. This would be possible only by examining your eyes in person. Hope my suggestions helped you. 4 Tips For Better Vision!

An important organ of the body, eyes must never be taken for granted and need to be protected at all times. In addition to protecting them, you also need to improve as well as maintain their overall health.

Here are some of the ways of keeping your vision in top form:

1. Eat for good eye vision

To have healthy eyes and consequently good vision, you need to have foods that keep them in good health. Nutrients like Zinc, Vitamins C and E and Omega-3 fatty acids can help to fend off age-related sight problems such as cataracts or macular degeneration (a condition characterized by the deterioration of the middle portion of the retina). Foods like eggs, nuts, green leafy vegetables, oranges and citrus fruits are what you need to have to get good eye health.  

2. Kick the smoke habit

Smoking can adversely affect your eye health and can make you prone to eye conditions like cataracts, macular degeneration and damage of optic nerve (a nerve that transmits impulses from the eyes to the brain). By kicking this habit, you end up saving your eyes from irreversible damage.

3. Use sunglasses

Exposure to the sun’s ultraviolet rays for a long period of time can make you prone to common eye problems including cataracts. To protect them from the sun’s harmful rays, you need to use sunglasses that block 99-100% of ultraviolet A (UVA) and ultraviolet B (UVB) rays.

4. Go for a regular eye checkup

Going for regular eye examinations can help in protecting your vision and ensure you see clearly. Eye examinations are important to help you find eye problems like glaucoma (a condition wherein the optic nerve is damaged, resulting in vision loss). A comprehensive eye examination can help in identifying problems like nearsightedness, farsightedness or astigmatism (an eye defect that occurs to a deviated spherical curvature).

My eyes are always heavy. Eyes fatigue ness, heaviness is always there. I only want to close my eyes all the time. I can sleep for so many hours because of this tiredness. I never feel fresh after waking up. What is d reason for this and what to do for this.

This type of complaints happen with continuous use of smartphone & continuously watching TV without break, this may lead to dryness causing this type of complaints, you can use lubricating eye drops 4-5 times a day (Refresh liquigel) ,if don't get relief within a week ,consult some local experienced ophthmologist.

This in most case is a normal occurence, direct staring at sunlight should not be done any which ways. Though this maybe abnormal if associated with flashes/floaters or a red eye. If thats the case you need an eye examination at the earliest.

Hello, You Can take 1.Tablet Crocin (Paracetamol 650 mg) one tablet after food when required for headache after food for 2-3 days ( maximum 2 tablets with gap of 12 hour can be taken in a day ) 2. Adequate sleep 6-8 hr in a day 3. Get check eyes for refractive error 4. We need to go for ENT evaluation to rule out sinusitis 5. get your BP checked 6. regular physical exercise and meditation and yoga and kindly get your CBC checked and review with reports and kindly give me complete history of your headache for further Management like like duration, severity, aggravating factors/relieving/ triggering/ associated factor to reach diagnosis

Hello, Include the following for getting better vision: Vitamins A, C, E, and minerals like copper and zinc are essential to eyesight. Get the antioxidants from dark leafy greens, egg yolks, yellow peppers, pumpkin, sweet potatoes, and carrots. Include garlic and onions. Blueberries, grapes as they have antioxidant and anti-inflammatory properties that can help improve your vision. DHA is a fatty acid found in coldwater fish like wild salmon, sardines and mackerel. Exercises for eyes: Warm your eyes. Rub your palms together to create heat, and then place them against your eyes for five seconds. Repeat this three times. Roll your eyes. Start by looking up and then slowly circle 10 times clockwise and 10 times counterclockwise. Focus. Hold a pen at arm's length, focus your eyes on it, and slowly bring the pen closer until it's about 6 inches away from your nose. Then slowly move it back, keeping your eyes focused on the pen, 10 times in all. Massage your temples. Using your thumb knuckles, massage your temples in small circles, 20 times in one direction and 20 in the other. Repeat the same actions above the mid-point of the eyebrows at the forehead, then below the eyes on both sides of the bridge of the nose. Take a mini-nap. Put your head back, close your eyes, and relax for 3 minutes. Insufficient sleep may weaken your vision, so shoot for 8 hours of sound sleep a night.
