Weight Gain Issue Among Exclusive Breast Feed Baby

Diploma in Child Health (DCH), MBBS
Pediatrician, Seoni

Doctor sahab my baby is not gaining weight properly or not eating anything is the one of the major complain in any parenting forum and in my clinic also. This blog is dedicated to those parents who always concerned about baby weight. I will try to cover all step by step. This post is only for exclusive breast feeding baby means below 6 months of age baby. At birth baby weight should be approximately 3kg or more than that. Less than 2.5 kg is low birth weight. Babies lost there weight in initial days and regain there birth weigh around 10th to 14 th day of life. In first 3 months growth rate is approximately 25 to 30 gm per day after that it will increase @ 400 gms per month till 1 year. Weight will double at 5 months and triple at 1 year. Usually we compare our baby weight with others and get frustrated our baby is not healthy. We expect chubby photogenic baby. So here I will try to explain the whole story.

Let's start from the very beginning the story start the day we decide to have a baby. Maternal nutritional status and medical fitness have a mojour role. Single infection can cause low birth weight baby or preterm. In us dental infection is one of the majour cause of preterm baby. We adviced to consult couples before and after pregnancy for any problem. You can have folic acid 3monthes before concepction. Mother, short interval of previous pregnancy, maternal hemoglobin level, nutritional status, high bp (pih, viral infection, bleeding per vaginal, chromosomal abnormalities can be cause of low birth wight, prematuered or iugr baby. Women should have healthy food, do some light exercise and try to live stress free life.

In second part of story baby born with certain weight and maturity. Weight less than 2.5 kg is low birth weight and birth before 37 weeks called premature baby. Baby weight gain is mostly affected by recurrent infection and feeding and temperature for initial days.

Temperature regulation:
Too hot or too cold weather can affect weight. Try to wear one extra clothes what we are wearing like if we are comfortable in half shirt we can wear him full shirt or if we are comfortable in full shirt we can wear him a half sweater more. In winter we have wear him cap and shocks but in summer only cotton jhabla is sufficient.

Infection control:
New born babies had low immunity so chances of infection is much more. To prevent infection you should remember these things

  • try to touch baby after hand wash and try to not touch the above the wrist joint. We touch by infected hand and baby keep the same hand in his mouth get stomach infection.
  • don't give any antibiotics without without consulting doctor and complete full course of antibiotics
  • immunize your child as per schedule.
  • avoid bottle feed if possible and if you are using it should be properly sterilized.

Feeding:
Exclusive breast feeding is adviced till 6 months. There is either problem in milk production and ejection or technique of feeding.

  • Milk production and ejection: Milk production is controlled by prolactin harmone. For most part of milk production is use it or loose it process. The more often baby nurse the more milk you make. Oxytocin harmone is responsible for milk ejection. Suckling, thought of the baby, sound and sight of
  • Baby stimulate milk ejection: Stress is the major suppress of milk ejection. Plenty of water, milk, apricot, poppy seed, papaya, oatmeal, carrot, bottle ground, funnel seed, basal leaf, can be used to augmentation of milk production.
  • Breast feeding: Proper positioning and attachment is necessary for successful breast feeding. Try to feed baby with one breast at a time so baby can get for milk and hind milk both. For flat or inverted nipple can be corrected by syringing method. Tounge tie or fungal infection can be cause of improper feeding. Feed your baby on demand or every 2hr. If baby satisfied he should sleep 2 to 3 hrs after feeding, passes 6 to 8 time urine. 5 to10 min sufficient for feeding if your baby takes half an hour means you had less milk production. Initial 3monthes feeding frequency8 to10 times day after 7 to 8 times a day may be sufficient.
  • Condition related to baby: For preterm and low birth weight kangaroo mother care is very useful. Where baby is kept in vertical position between the mother breast under the clothes so both skin can be in touch. You have to keep baby at least 1 to 2 hrs in a single sitting and as long as you can. It helping milk production and weight gain of baby. Repeated infection of respiratory tract, stomach infection, urinary tract infection, some congenital infection, metabolic disorder may cause slow weight gain. Neurological insult during delivery may cause slow weight gain with delayed mile stones.
  • Motherhood is a great honor and privilage: It is the most challenging and utmost satisfying job, and as in every job first year is most difficult one with less paying with no vacation. As a man we never understand these things. So big salaute to those moms who day and night care there baby and wish best of luck.

For constipation, give him fibrous diet and adequate water. Encourage him to go toilet daily or may be twice in a day for developing bowel habit. Isabgol husk may be tried with plenty of water at bed time. Pimples are common at this age. Don't scratch it, apply calamine lotion and consult skin specialist.

Vocational Guidance & Career Counseling Benefits For Students & Adults

Vocational Guidance & Career Counseling Benefits For Students & Adults

Choosing a career is a serious concern for students soon after completing 10th standard when one is required to make a choice of subjects for 11th grade. While still very young, many of us hardly know what is good for us and which field is best. For most of us, choosing a career is not a serious affair until 12th grade. However, once a student is done with 12th, he rethinks - “Did I make the right choice of subjects in class XIth?” I had always wanted to go in this field after school, but nobody guided me about subjects required for it in class XIth and XIIth. So, after completing school one feels very sad and frustrated that interest lay in A–field but wasted 2 school years doing B-Field, a totally different course of study.

Therefore, when selecting subjects in XIth, it is very necessary to have a career goal in place keeping the market in mind. In addition to this, selecting the right education institution in XIth is equally important. Eg. “I am interested in Fine Arts but my school offers only Science and Commerce.” So, the child takes one of these only and loses track of what he wants, sowing the seeds for future distress in the field thrust upon him. Unfortunately, not many students are able to discover their potentials and interests at this stage and end up making wrong choices.

In this scenario, seeking the help of an experienced and qualified career counselor becomes absolutely necessary who can assist in discovering your potential and aptitude and accordingly suggest the right course. Career counselor usually employs testing of three factors – aptitude, interest and personality. These aspects help the counselors in deciding the right career path for a student. With an increase in the number of opportunities in the market, the importance of career counseling has also increased manifold. People have begun to realize the fact that not all can become engineers or doctors and started exploring other fields depending on their talent and interest in alternate domains.

Sound guidance will depend on the qualifications, training and experience of the professional. It is always a good idea to first know about the professional expertise of the counsellor/psychologist on whom you depend for making a life-long career choice. Generally speaking, school counsellors are post-graduates with about 9 months diploma in counselling and guidance with a focus on school children. They may or may not have done M.Phil, or Ph.D to qualify as a doctor. Similarly, anybody with just a graduation degree in Psychology can self-proclaim as a Counsellor/Psychologist and offer guidance and counselling, charging much less a fees. For any professional you choose, the higher the educational qualifications, more the number of practical trainings from reputed institutes, and the greater the professional experience, more dependable will be the advice given by them.

Vocational guidance for students and adults is one of the best ways to choose the right career path. With a plethora of career choices out there, sound career counselling plays a pivotal role in ensuring that right career choices are made. This is rewarding, considering that an individual can choose the right career path as per the strengths and talents he possesses. Right counselling not only helps the person to grow in the job quickly but gives ample room to learn in his desired field.

Here is a list of some major benefits of vocational guidance and career counselling:

  1. Right assessment of weakness and strength: Career counselling is typically conducted by expert professionals with years of experience behind their back. They assess a candidate based on various aptitude tests. The test results rightly portray the strength and weaknesses of a candidate. This information plays a key role in determining the right career choice for a candidate. A person can ensure that he refrain from choosing a wrong career that leads to frustration and low work satisfaction.
  2. Right goal setting leads to better choice: A counsellor can help a candidate choose the right goal and help him to outline the process of achieving it. Furthermore, an experienced counsellor can help a candidate lay down a plan to reach the goal. A step by step blueprint of reaching the goal in a realistic manner helps a candidate to achieve his dream career faster. With right goal setting, a person can attend career success over a very short period of time.
  3. Explore various career choices: There are vast career choices that a person can explore that are available within the scope of a person’s expertise. A career counsellor can throw light on these choices and help a person focus on the right option. This ensures that a person saves a lot of time and energy while figuring out the best possible career choice that is out there. This is particularly true in cases where a person is trying to make a mid-career switch.
  4. The right guidance and support: An experienced career counsellor can guide a candidate to get hold of the right set of skills and expertise that may be required to achieve a particular career. This ensures that a candidate is adequately prepared before the big opportunity knocks on the door. Right guidance not only cut down the preparation time, it increases the chances of success as well.
  5. Search Support: While the job search is on, a good counsellor can help a candidate prepare in terms of resume preparation, cover letter, video introduction etc. They can also help a candidate with the right kind of networking to land that dream job.

To conclude, the crux of career counselling is to get the necessary guidance from a qualified professional that would help the student/adult in making the right choice regarding his career. If you wish to discuss about any specific problem, you can consult a Psychologist.

Need not worry this is a common problem in children of this age group. You can use dulcolax paediatric per rectal suppository, which you need to insert in the anus (rectal opening) better to get it done under supervision, take help of a nurse. You can use duphalac syrup also.

BABY TEETH CARE BASICS:---Few things are as adorable as a baby's totally toothless smile. But baby teeth development doesn't take long. Before you know it, you'll be navigating your way through the world of teething, brushing baby teeth, losing baby teeth and eventually welcoming in a mouth full of permanent pearly whites. Baby teeth development actually begins before baby is born. Primary teeth are almost completely formed at birth but remain buried under the gums. These mini chompers start showing up when a baby is about six months to a year old.Unfortunately, baby tooth development and eruption is a painful process. Teething can make an infant's gums sore, causing your little one to be uncomfortable and irritable. Experts suggest that during baby tooth development, you massage your baby's gums with a clean finger or give a chilled teething ring, a cool spoon or a cold, wet washcloth to help ease the discomfort.Parents should begin brushing baby teeth as soon as the first tooth arrives. Why bother? These little munchers help baby with proper chewing and speech and hold space in the gums for permanent teeth. It's never too early to start taking good care of your baby's teeth. Here are some baby teeth care basics:----- Prevent early childhood caries, also known as baby bottle tooth decay, by making sure baby doesn't sleep with a bottle containing any sugary liquids -- even breast milk. And never give your child a pacifier that's been dipped into anything sweet.- Start brushing baby teeth as soon as the first tooth appears. Routine baby dental care should also include massaging the gums with a clean gauze pad. When all teeth have erupted, floss at least once a day to help prevent the buildup of dental plaque.- Wean your baby off thumb-sucking if he or she is still doing so by age four. Otherwise, it can cause overcrowded or crooked teeth.- Consider a combination of fluoride treatment and dental sealants, thin plastic coatings applied on baby molars to keep dental plaque from accumulating. Talk to your dentist before giving your child any fluoride dental treatment and have your child use only un-fluoridated toothpaste until two years of age.- Take your child to the dentist after the first tooth arrives or by age one. Regular dental visits combined with daily baby teeth care can help give your baby a good head start on the road to dental health.A great dentist or pediatric dentist can answer any questions you might have about baby teeth development and help provide tips for proper baby teeth care.Call/Text/What's up us @ 8285666226 fir any help.

Bronchial Asthma

MBBS, MD - Pulmonary Medicine, DNB - Pulmonary Medicine, MD - Internal Medicine, Diploma in Tuberculosis and Chest Diseases (DTCD)
Pulmonologist, Delhi
Play video

Good mroning, I am Dr Ashok Rajput, a Pulmonologist, practising in Delhi. Today I will talk to you about very common disease, that is, Bronchial Asthma.

Asthma starts in early childhood and may persist in adult life. It is an episodic disease which is intermitted initially. Later, if left untreated, it become continuous and perennial. Whne one is exposed to dust, fumes, pollen and smoke then the inner lining of the airways swells and becomes inflammed and the lumen narrows. So it becomes difficult to breathe. So these patients get cough, wheezing, snoring and running nose.

The diagnosis of this condition is very easy. What you have to do?

You do Spirometry or a lung function study in which you blow into a computer and you get a graph. You repeat the test after giving bronchodilators, say 15-20 minutes. If you find an improvement in FEV1, one of the parameters, more than 12% you clinch a diagnosis of Asthma.

The treatment of Asthma is very simple these days. The main stay of treatment is inhaled corticosteroids which is given through the inhalers. There are many types of inhalers available with good drugs which act for 12 hours which you need to take only in morning or evening and your Asthma gets controlled. When I say controlled, that means you will no longer have cough, wheeze or sneeze. You can sleep undisturbed in the night and can perform all the activities. You can lead a normal life. What you require is small dose of this inhaler.

Thank you for listening to me.

If you want to contact us, you can come to our clinic or contact through Lybrate.

6 Ways To Help Your Baby Learn Speaking

FELLOWSHIP IN PCCM, FELLOW-PEDIATRIC FLEXIBLE BRONCHOSCOPY, FELLOWSHIP IN PEDIATRIC CARDIAC CRITICAL CARE, D.C.H., M.B.B.S
Pediatrician, Ahmedabad
6 Ways To Help Your Baby Learn Speaking

The role of parents in language development of children is a primary one. Basically, it will be a one way communication as babies will not respond, but that should not deter you from speaking to them. It has been shown that talking to children early on helps them to talk faster and learn more words. The number of words a child hears is directly proportional to the amount of vocabulary he/she will be able to master.

6 ways to help your baby learn speaking:

  1. Let them listen a lot: Children learn speaking by listening first. So more and more words they will listen, so do they will speak. Make sure your child comes in contact with children in the peer group and plays with them as with the help of their friends they can learn many words.
  2. Read a book: You should start reading to your child as early as possible. The type of book is not as important as you may use, various touch and feel books such as graphic novels. Initially, you may start with board books and then move on to picture books and finally to story books. It helps in increasing the child's vocabulary.
  3. Talk as much as you can: Part of a child's ears and brain that respond to sound are developed since birth, so even though talking to your baby may not make much sense, talking to them enhances their development of speech. The infant absorbs the words which facilitates speaking.
  4. Look for cues: If the child is interested in something such as a book or a toy, then engage with the child on that subject. Encourage him/her to ask questions and interact as much as possible. These interactions help in enhancing the child's language skills.
  5. Limit television: Using television to teach vocabulary to your child is not as effective as talking directly. The primary reason is that characters in television do not react to your child's cues. This does not allow the child interact, which results in reduced language learning.
  6. Treat ear infections early: It is important to treat ear infections early as this causes hearing problems which in turn delays language learning. Consulting a pediatrician to get treatment for ear infections of your child is advisable. You should see to it that the child is being administered medications at regular intervals.

We always advise against bottle feeding as it is inherent with many problems. Only breast feed & homemade semi solids are the ideal food for a 4 month old baby. If you stop bottle the stool should become normal. I can give you constant support and advice if you give all details like birth weight and immunisation details, better to ask privately.

As you have already started with bottle milk and the child is used to bottle feed, its difficult to feed the child normally with spoon. Try to avoid bottle feeding as much as possible. Even if you continue bottle feeding just take care that you maintain proper hygiene by boiling both bottle and nipples atleast once daily. Do not leave the child when child is feeding through bottle.
