I am 36 yera old female and have a pain in my right shoulder for last 1 months. So what should I do?

Diploma in Acupuncture, Pranic Healing, MD - Acupuncture

Massage with your thumbs from the mid points of the shoulder blades on both sides towards the lower back of your head up to lower level of the ears. Use oil or water for massage. Rotate your head clockwise and anticlockwise, forward and down, left and right a few times. Do for 5 to 10 times morning and evening. Give pressure the dipping point on the portion between the thumb and the index finger on the back of the palm. Press and release for 15 times where the pain is more. This will solve the problem. Further do some wrist rotation exercise. Consult after 5 days.

You should start doing good warm up excercises, followed by stretches for shoulder before any strenuous activities. This will prevent the injury to shoulder. Also add on strengthening excercises on regular routine so that your muscles are actually strong enough. Don't forget to do back strengthening alongwith as that gives a strong base to shoulder movements.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Avascular necrosis is a condition in which the head of the femur due to lack of blood supply become non viable, collapses and may lead to arthritic changes in the joint. Also progressively as the disease progresses, it can lead to a limping way of walking and subsequently diminished movements and pain. Your earlier surgery must have been an attempt for re vascularization of the head but it seems that has not worked and that is common. I can advise you if I have the following information: - latest x ray pelvis with both hips-ap view - you examination of affected hip with note of movements present - your previous surgery details and if possible, follow up doctor's notes. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

Two month ago my left shoulder is dislocated I treat this but little pain in my shoulder what can I do.
You need to start strengthening exercises. Use elastic resistance band. Avoid rotating the arm outwards. Pain will ease of slowly. You are free to perform regular activities but avoid sudden movements. A few shoulders dislocate again. So complete your rehab and take care.

Fibromyalgia

CCPH, BHMS
Homeopath, Bhilai
Fibromyalgia

Fibromyalgia (FM) is a medical condition characterised by chronic widespread pain and a heightened pain response to pressure. Other symptoms include feeling tired to a degree that normal activities are affected, sleep problems, and troubles with memory. Some people also report restless legs syndrome, bowel or bladder problems, numbness and tingling, and sensitivity to noise, lights or temperature. Fibromyalgia is frequently associated with depression, anxiety, and posttraumatic stress disorder. Other types of chronic pain are also frequently present.

The cause of fibromyalgia is unknown; however, it is believed to involve a combination of genetic and environmental factors with half the risk attributed to each. The condition runs in families and many genes are believed to be involved. Environmental factors may include psychological stress, trauma, and certain infections. The pain appears to result from processes in the central nervous system and the condition is referred to as a "central sensitization syndrome". Fibromyalgia is recognized as a disorder by the US National Institutes of Health and the American College of Rheumatology. There is no specific diagnostic test. Diagnosis involves first ruling out other potential causes and verifying that a set number of symptoms are present.

The treatment of fibromyalgia can be difficult. Recommendations often include getting enough sleep, exercising regularly, and eating a healthy diet.

The defining symptoms of fibromyalgia are chronic widespread pain, fatigue, sleep disturbance, and heightened pain in response to tactile pressure (allodynia). Other symptoms may include tingling of the skin (paresthesias), prolonged muscle spasms, weakness in the limbs, nerve pain, muscle twitching, palpitations, and functional bowel disturbances.

Many people experience cognitive dysfunction (known as "fibrofog"), which may be characterized by impaired concentration, problems with short and long-term memory, short-term memory consolidation, impaired speed of performance, inability to multi-task, cognitive overload, and diminished attention span. Fibromyalgia is often associated with anxiety and depressive symptoms.

Other symptoms often attributed to fibromyalgia that may be due to a comorbid disorder include myofascial pain syndrome, also referred to as chronic myofascial pain, diffuse non-dermatomal paresthesias, functional bowel disturbances and irritable bowel syndrome, genitourinary symptoms and interstitial cystitis, dermatological disorders, headaches, myoclonic twitches, and symptomatic hypoglycemia. Although fibromyalgia is classified based on the presence of chronic widespread pain, pain may also be localized in areas such as the shoulders, neck, low back, hips, or other areas. Many sufferers also experience varying degrees of myofascial pain and have high rates of comorbid temporomandibular joint dysfunction. 20–30% of people with rheumatoid arthritis and systemic lupus erythematosus may also have fibromyalgia.

Try rest x 2 weeks and then physiotherapy x 4weeks. Grade 2 signal does not mean a complete tear. Partial/low grade injuries can heal.

Any of your daily activites are hampered due to this pain ? clinical evaluation needs to be done to see how much shoulder movements are restricted ,also is there any stiffness? Hi Thanks for your query and welcome to Lybrate. I am Dr. Akshay from Fortis hospital, New Delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - To maintain proper posture of your back while working and sleeping - If pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - Physical therapy initially under supervision of a trained physiotherapist and then to continue at home - Adequate calcium & vitamin d intake if levels are low in body - Ice packs can applied if your pain is acute, then hot fomentation can be done at home - Analgesic spray for local application can be used and is easily available. - Avoid lifting heavyweight/ acute forward bending in mornings etc We will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation. Thanks & Regards Dr Akshay Kumar Saxena Consultant Orthopaedics Fortis Hospital, New Delhi

Recurrent back pain does need an evaluation by physical examination by a spine orthopaedic surgeon. Since you don't report pain radiation to lower limbs, it may be localised lesion in disc or surrounding ligaments. An MRI may give clue As of now, your treatment may be conservative, with back strengthening exercises, a brace wearing in daytime, anti inflammatory, muscle relaxant gels or oral medications. Unless your routine work is badly restricted, then only surgery may be indicated. For partial prolapse of disc without radiation of pain to lower limbs, ozone injections are given by many spine specialists to shrink the disc and for ling term symptomatic relief. If severe prolapse, as shown by symptoms or MRI, percutaneous disectomy is advisable.
