Fluoride and Dental Health

Fluoride is a mineral that occurs naturally in many foods and water. Every day, minerals are added to and lost from a tooth's enamel layer through two processes, demineralization and remineralization. Minerals are lost (demineralization) from a tooth's enamel layer when acids -- formed from plaque bacteria and sugars in the mouth -- attack the enamel. Minerals such as fluoride, calcium, and phosphate are redeposited (remineralization) to the enamel layer from the foods and waters consumed. Too much demineralization without enough remineralization to repair the enamel layer leads to tooth decay.

a)Fluoride helps prevent tooth decay by making the tooth more resistant to acid attacks from plaque bacteria and sugars in the mouth.

b)It also reverses early decay.

c)In children under 6 years of age, fluoride becomes incorporated into the development of permanent teeth, making it difficult for acids to demineralize the teeth.

d) Fluoride also helps speed remineralization as well as disrupts acid production in already erupted teeth of both children and adults.

In What Forms Is Fluoride Available?

As mentioned, fluoride is found in foods and in water. It can also be directly applied to the teeth through fluoridated toothpastes and mouth rinses. Mouth rinses containing fluoride in lower strengths are available over-the-counter; stronger concentrations require a doctor's prescription.

We dentists, apply fluoride to the teeth as a gel, foam, or varnish. These treatments contain a much higher level of fluoride than the amount found in toothpastes and mouth rinses. Varnishes are painted on the teeth; foams are put into a mouth guard, which is applied to the teeth for one to four minutes; gels can be painted on or applied via a mouth guard.(as shown in figure)

When Is Fluoride Intake Most Critical?

It is certainly important for infants and children between the ages of 6 months and 16 years to be exposed to fluoride. This is the timeframe during which the primary and permanent teeth come in. However, adults benefit from fluoride, too. New research indicates that topical fluoride -- from toothpastes, mouth rinses, and fluoride treatments -- are as important in fighting tooth decay as in strengthening developing teeth.

In addition, people with certain conditions may be at increased risk of tooth decay and would therefore benefit from additional fluoride treatment. They include people with:

  • Dry mouth conditions : Also called xerostomia, dry mouth caused by diseases such as Sjögren's syndrome, certain medications (such as allergy medications, antihistamines, anti-anxiety drugs, and high blood pressure drugs), and head and neck radiation treatment makes someone more prone to tooth decay. The lack of saliva makes it harder for food particles to be washed away and acids to be neutralized.
  • Gum disease : Gum disease, also called periodontitis, can expose more of your tooth and tooth roots to bacteria increasing the chance of tooth decay. Gingivitis is an early stage of periodontitis.
  • History of frequent cavities: If you have one cavity every year or every other year, you might benefit from additional fluoride.
  • Presence of crowns and/or bridges or braces: These treatments can put teeth at risk for decay at the point where the crown meets the underlying tooth structure or around the brackets of orthodontic appliances.

Are There Risks Associated With Fluoride Use?

Fluoride is safe and effective when used as directed but can be hazardous at high doses (the "toxic" dosage level varies based on an individual's weight). For this reason, it's important for parents to carefully supervise their children's use of fluoride-containing products and to keep fluoride products out of reach of children, especially children under the age of 6.

In addition, excess fluoride can cause defects in the tooth's enamel that range from barely noticeable white specks or streaks to cosmetically objectionable brown discoloration. These defects are known as fluorosis and occur when the teeth are forming -- usually in children younger than 6 years. Fluorosis, when it occurs, is usually associated with naturally occurring fluoride, such as that found in well water. If you use well water and are uncertain about the mineral (especially fluoride) content, a water sample should be tested. Although tooth staining from fluorosis cannot be removed with normal hygiene, your dentist may be able to lighten or remove these stains with professional-strength abrasives or bleaches.

Keep in mind, however, that it's very difficult to reach hazardous levels given the low levels of fluoride in home-based fluoride-containing products. Nonetheless, if you do have concerns or questions about the amount of fluoride you or your child may be receiving, talk to your child's dentist, pediatrician, or family doctor.

A few useful reminders about fluoride include:

  • Store fluoride supplements away from young children(these are not available in India).
  • Avoid flavored toothpastes because these tend to encourage toothpaste to be swallowed.
  • Use only a pea-sized amount of fluoridated toothpaste on a child's toothbrush.
  • Be cautious about using fluoridated toothpaste in children younger than age 6. Children younger than 6 years of age are more likely to swallow toothpaste instead of spitting it out.

Hello. I am vinay goel and 22 yrs old. My teeth is little outside and I want braces on it. I try 2 dentist 1. Tell that it is a 1 hr procedure and take 8 or 9 months for this and in rs 18000. But 2. Tell that it is 10 mint procedure and take 2yrs for this. Then which dentist is right? And there is a pain in this procedure?

Dear lybrate-user. Tooth has natural tendancy to go back to their place. So treatment will be completed in one and half year. For next 6 months you have to wear retainer plates to maintain the treatment out come. So total duration 2 years. You can also ask your dentist for esthetic braces if you are concerned. Those are slightly costlier than regular one. Regards.

The type of treatment you have for toothache /sensitivity/swelling will depend on the cause of the pain, so your dentist will examine your mouth and may carry out an X-ray to try to identify the problem. •If your toothache / sensitivity/swelling is caused by tooth decay, your dentist will remove the decayed area and replace it with a filling. •If your toothache / sensitivity/swelling is caused by a loose or broken filling, the filling will be taken out, any decay will be removed and a new filling put in place. •If the pulp inside your tooth is infected, you may need root canal treatment. This procedure involves removing the infected pulp and then inserting a special type of filling to seal the tooth and prevent re-infection. •If your toothache / sensitivity/swelling can't be treated using these methods, or if your tooth is impacted (wedged between another tooth and your jaw), it may need to be removed.

Teeth pain may be due to various reasons like infection, sensitivity or trauma. If your teeth is decayed it is better to get it filled or treated by a dentist. If you are opting for root canal treatment do not forget to get a crown on the effected tooth. If you want to go for only medicinal treatment so let me tell you only medicine are not going to be very effective untill you get this treated by a dentist.

Hello doctors , I have bad mouth ulcers, severe pain, its keeps coming to me , I as using b complex tablet, having milk , applying dologel ,but no much improvement, some doctors Suggested me the following tablets and Gell Need to know which is more effective and more good 1) Folvite MB 2) HEXIGEL TUBE 3) ZINCOVIT TABLETS (VIT B12) 4) MUCOPAIN GEL 5) kenacort gel Of this which is more effective , I want to get rid of it permanently, Also if any tablet the let me know when should I have before food or after food and also let me know what all time I should have , (morning , noon and night ) please confirm me all this urgent. Please help.

Hi lybrate-user' hexigel and multivit tabs that you are taking shd help in controlling ulcers. However, if the ulcers persist beyond a month like this then you should get it checked to rule out any other possibility. Using mucopain gel wl only numb the surface. Severe pain and recurring ulcers require thorough check up n followup. More stronger medicines can be prescribed under supervision only. Pls avoid tobacco in all forms as this can lead to cancerous changes.

3 days back I got removed my wisdom tooth and took antibiotic as per dentist suggestion. Within a day I got mouth ulcer (roof of the mouth) I stopped using antibiotic. Using Denogel very often. It does not help. I am not able to even drink plain water so much pain is there. Please suggest.

Hi, ulcers could be because of prolonged use of antibiotics, vitamins deficiency due to improper diet intake and also could be due to stress. Advised you to take proper balanced diet, lots of liquid, water and use ulcer gel before meal. They will take 5-7 days to get healed properly. Start taking multivitamins also.

U r suffering from a disease called oral sub mucous fibrosis. Its a pre cancerous condition. If u don' t stop the habit completely then there are very high chances that it might convert into oral cancer.

Kindly consult a dentist in person for further suggestion. We need more investigations with clinical examination to decide upon treatment. You may need root canal treatment. Until then apply clove oil on the decayed tooth. Rinse frequently with mouth wash. Advise c. Amox 500mg 1-1-1, thrice, t. Metrogyl 400mg 1-1-1, thrice, t. Imol 1-1-1, thrice (contraindicated in conditions with gastric ulcer, asthma, wheezing, and allergy), and c. Becosules 1-0-0 once, all for 3 days only. Advance procedures can be done with laser. You may consult me in person. Dental tips: - visit a dentist every six months for cleaning and a thorough dental check-up. Limit sugary food to avoid tooth decay. Gargle your mouth thoroughly after every meal. Scrub gently to clean your tongue with a tongue cleaner. Floss all your teeth inter dentally & brush twice daily, morning & night, up & down short vertical strokes, with ultra-soft bristles, indicator brush. Tooth brush to be changed every 2 months.

There are several causes of tooth discoloration, including: Foods/drinks. Coffee, tea, colas, wines, and certain fruits and vegetables (for example, apples and potatoes) can stain your teeth. Poor dental hygiene. Inadequate brushing and flossing.... Here is home remedy for brighter teeth--- Mix several teaspoons of baking soda with enough fresh lemon juice (or water) to make a paste. Wipe your teeth and any extra saliva off of them with a paper towel. Put a good amount of paste onto your toothbrush and apply. Leave the paste on for 1 minute, then rinse, so as to avoid the acid effecting enamel. Sir in d night wen you r going to bed, please emove your dentures nd keep dem in normal tap water, in d morning before wearing dentures you may use a b normal tooth brush and clean dem, no need to apply any paste to dem. Nowadays denture cleanising tablets are also vailabli in market, regrds.
