Diagnosis and Management of Asymptomatic Neoplastic Pancreatic Cysts

MBBS, MD - General Medicine, DM - Gastroenterology
Gastroenterologist, Delhi
Diagnosis and Management of Asymptomatic Neoplastic Pancreatic Cysts

The diagnosis as well as management of pancreatic cystic lesions is a general problem. Nearly 1% of the patients in the chief medical centers have been observed to have pancreatic cystic lesions on cross sectional imaging. It has also been observed that a quarter of all pancreas scanned in an autopsy series contain pancreatic cysts. Earlier, these cystic lesions were regarded benign but with increasing evidence made available from the cystic lesions, they are regarded as origin of pancreatic malignancies.

Information on Asymptomatic Neoplastic Pancreatic Cysts: The most vital medical tools that are used in the diagnosis and management of pancreatic cystic lesions include the endoscopic ultrasound and cross sectional imaging. These are used to distinguish non-mucinous cysts from mucinous cysts. The identification of pancreatic cysts creates a lot of anxiety for the clinicians as well as the patients related to the probable presence of a fatal tumor. The findings of a macro cystic lesion that enclose viscous fluid loaded in CEA are helpful in the analysis of a mucinous lesion.

The most common pancreatic cysts are the non-neoplastic inflammatory pseudo cysts, and they can be detected easily by imaging. The identification of pancreatic irregularity with probable association with malignant cells is a vital source of referral for the specialist. The set of guidelines that have been proposed for the management and diagnosis of patients with asymptomatic neoplastic pancreatic cysts are based specifically on the analysis of the quality of the data. It is also designed to address the most important and frequent clinical scenarios. The diagnostic suggestions are provided based on the clinical problem as well as the risk of malignancy.

Imperative Guidelines to Follow: To achieve accurate diagnosis of asymptomatic neoplastic pancreatic cysts is indeed a great challenge. It is all the more important to find the reproducible methods that can be used to stratify threat of cancer for the patients. The main guidelines include a two year screening interval of cysts that can be of any size as well as stopping observation after 5 years, in case there is no change.  The new guidelines, for the most part, recommend surgery if more than one concerning feature is confirmed on the MRI by use of endoscopic ultrasound. The new guidelines even suggest discontinuation of inspection after the surgery if no dysplasia or invasive cancer is identified. The guidelines have mainly been developed by use of Grading of Recomendations Assessment, Development and Evaluation.

My wife is suffering from tubacular lymphednitis (confirmed by fnac last year) on the left side of the lymph node of neck. It is in cervical lymph node and is in swollen condition. She took t. B. Medicine and during t. B. Treatment swollen lymph node started coming to the normal shape. But after the 5th month of medicine, lymph node suddenly started increasing its size. In the seventh month we done the ultra sound of that lymph node, which showed there is a pus inside it. I have also gone for the fnac again and it again showed that t. B. Infection is still there. For further clarification and the drug sensitivity, I go for the culture test of myco bacterium of t. B. Of the pus aspirated from swollen lymph node, which further show negative results as no afb came in that. This situation has become very complicated as fnac show t. B. Infection but culture shows no signs of t. B. Infection. I again go for the fnac which again came positive with t. B. Infection and after that I have also gone for the histopathology for the specimen of the lymph node and again the culture of the specimen of the lymph node, its again came negative. Histopathology only showed inflammation and necrotising nothing else. So please suggest something helpful as the situation is very complicated as fnac test is showing t. B. Infection and other test are coming negative.

Hello sir, Please tell me is there any treatment for reduced erection due to diabetes .

PDDM, MHA, MBBS
General Physician, Nashik
Hello sir, Please tell me is there any treatment for reduced erection due to diabetes .
Try kegel exercises this is a great way of strengthening your pelvic muscles and gain strength for sex. It is like going to the gym, but for sex. Let me tell you how to do kegel exercises. You have to practice it first. When you urinate, try to stop the flow of urine without your hands, just by contracting your muscles. The muscles you contract then are the pelvic muscles that are used during sex. Do not do this while urinating again, you can do this while you are sitting or standing, while working or doing other things. Once you know how to contract them try doing this a few times every day and this will strengthen them. Aim for at least three sets of 10 repetitions a day. You should see results in a few week.

Intestinal gas is typically caused by the fermentation of undigested food, such as plant fiber, in the colon. Gas can also form when your digestive system doesn't completely break down certain components in foods, such as gluten or the sugar in dairy products and fruit. Other sources of intestinal gas may include: Food residue in your colon Changes in intestinal bacteria due to antibiotics or other medications Poor absorption of carbohydrates, which can upset the balance of helpful bacteria in your digestive system Swallowed air that moves to your colon Constipation, since the longer food waste remains in your colon, the more time it has to ferment Sometimes, gas indicates a digestive disorder, such as irritable bowel syndrome or lactose intolerance. Tips for controlling excessive gas: Following simple diet and lifestyle changes can help to reduce gastrointestinal gas and relieve symptoms Be careful while eating Eat more slowly, Chew your food thoroughly and Relax while you eat. Make sure that you don? T swallow air while you eat. Do not deliberately swallow air to force a belch Eat what suits you Foods that may cause gas include beans; vegetables such as broccoli, cabbage; fruits, such as apples, and peaches; whole grains; soft drinks and fruit drinks: milk and milk products, Exercise regularly Exercise regularly and Walk for 10 to 15 minutes after eating. Homeopathy can be a great help for those who suffer from this problem. Homeopathic medicines are natural and completely safe without any side-effects. Homeopathic medicines are prescribed after in-depth analysis of symptoms. You can consult me online via Lybrate for medicines. Your feedback and recommendation matters. Thank-you.

How To Recognise a STROKE, Before it's Too Late!

MBBS, DNB (General Medicine)
Endocrinologist, Delhi
How To Recognise a STROKE, Before it's Too Late!

Strokes occur when blood to the brain gets interrupted or reduced. When this occurs, brain cells begin to die as they don't get enough nutrients and oxygen. Such life-threatening episodes can strike you anytime.

There are certain signs that can help you to clearly recognise an occurrence of stroke:

  • Face drooping on one side
  • Weakness or numbness on one side of the body, usually a leg or an arm
  • You're unable to think or say anything clearly
  • You experience slurred speech
  • Difficulty in moving your tongue
  • You get a sudden and severe headache

What Tests can be Done to Identify the Stroke?

There are certain tests that you can do to identify these warning signs.

  • To identify facial weakness, the smile test is used wherein you're simply asked to show your teeth to check for any changes in this facial expression - a lopsided smile.
  • To check for weakness in the body, the arm test is performed in which you raise both your arms to the same height. In case you're unable to do so, you may be experiencing arm weakness, a sign of stroke.
  • For slurring of speech, you may be asked to repeat sentences such as 'Don't cry over spilt milk' or 'The pen is mightier than the sword'.
  • Stroke can also be identified through the walk test wherein with the help of another person, you walk across the room to check for changes in your step.

If not treated in time, strokes can cause permanent brain damage. More importantly, unlike heart attacks, anxiety may not always be the cause of your stroke symptoms. That's why, being aware of these symptoms and recognising them on time can help you to quickly seek medical assistance.
