One of the dr suggested operation called" arthoscopic lavage and retinacular balancing' followed by +vissco supplement. Please advise whether this operation will relieve me of complete pain.

Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have symptoms of maltracking of patella due to a shallow groove, this can actually lead to anterior knee pain over a period of time due to softening of cartilage covering patella. Normally as an initial measure, we try rigorous physiotherapy with some supportive medications and see for the result, a lot of patients improve symptomatically with this but in some patients there may not be an improvement because of bad bio mechanics and for that we might have to think of a surgical procedure. Anyways if you send me all your investigations and doctor's prescription papers, then I can have a look and advise you accordingly. Thanks & regard.

Dear lybrate-user, chances of good results with knee replacement depend upon x ray picture, stage of disease, implant used and experience of surgeon in performing knee replacement. She should do fine with good quality implants though it will be costly affair. There are implants which can last for 25 years or more. You should go ahead with knee replacement. Hello do hot fomentation for 15-20 min thrice in a day. Avoid using computer, take rest for 5 min after working for 1 hour. You can use thin pillow while sleeping. In pain free range do neck exercises. Maintain proper body posture while watching tv/studying. This will help in reducing the pain. Analgesics, non-steroidal anti-inflammatory drugs (nsaids), weight loss and physiotherapy are used in the management of patients with spinal stenosis (disc space reduction) physiotherapy usually combines inactive therapy and therapeutic exercise. Inactive therapy includes heat or ice packs, ultrasound, electrical stimulation. These treatments help to ready the patient for active therapy by relaxing tight muscles and easing pain or discomfort. Therapeutic exercise includes stretching and prescribed exercises to help stabilize the spine, build strength and endurance, and increase flexibility. Please consult a physiotherapist for detail assessment and management of your condition. Best wishes.

My wife aged 37 years is suffering from ankle pain since last 2 months, the pain aggravates after she gets up after sitting for long, soaking in hot water gave her temporary relief. She has thyroid and taking thyronorm 75. Please advice.

BPTh/BPT
Physiotherapist, Hyderabad
Hello! Get a Xray done of the painful ankle first. There are many reasons for ankle pain. In general cases, it could be because of weight of the patient, prolonged standing ,weak joint or muscle weakness. Take the Xray report to a Physiotherapist and start with the relative treatment. Medications will give you just temporary relief and so, better to avoid frequent use of them. Choose a footwear which is soft and v comfortable in wearing. A heel sole will provide good relief. Avoid prolonged standing and prolonged walking.

Hi My wife is suffering from bronchitis. She stays in port blair. She have diagnosed in allopathy however that gives temp relief. She stopped allopathy then and tried homeopathy and ayurvedic. She came to back Bangalore. For 3 weeks and she was fine. However as soon as she went to Port Blair. She faces the issue again.

FRCSCTh, FRCS, MS - General Surgery, MBBS
Cardiologist, Bangalore
It is most likely bronchial asthma due to allergens in the air. She seems to be allergic to the pollens or dust in port blair. Either she has to move out from there to another city like bangalore or take inhalers regularly to control the symptoms. In any case consultation with a good pulmonologist should help.

You seem to be suffering from some form of autoimmune disorder or arthritis. Further details of your condition and test reports are needed to guide you. Deflazacort in such situations is just to control your disease process. For pain relief you need to take other medicines. Physical therapy in form of regular exercises like swimming, walking are crucial in systemic inflammatory conditions like yours. Such conditions can be associated with weight loss. Consult a pain specialist in your city regarding options available for pain relief in your situation. If you have queries consult again with details.

I am 26 years old, and have have backache for 45 days. What to do? Please help me.

COMT, MPTh/MPT
Physiotherapist, Surat
Pain in back is becoming very common days it is because of normal day to day activity and lifestyle. Especially found lot of females complaining of back pain because of there day to day working and pressure. There could be many reasons for back pain. Many of the problems occur for which we are not aware. I understand your pain. Let me recommend you the treatment as well as the precautions you can take at home to get away your backache. Precautions: 1. Avoid forward bending, if required then bend your knees first and then bend forward. 2. Avoid weight lifting. 3. Sit in chairs or car seats with proper back support. 4. Try to sleep in a position that helps you maintain the curve in your back like sidelying. 5. Get enough vit d from sun as vit d keep bones strong including spine, 6. Drink plenty of water to enhance the inter vertebral disks 7. Avoid standing in the one position for a long time. 8. Try to sleep in a position that helps you maintain the curve in your back. Treatment- 1. Apply ice or heat which ever suits you directly to the area of low back pain will decrease swelling and inflammation, and ease your discomfort. Apply it for 10 to 15 minutes only. 2. Relaxing for 5 minutes after ice or heat treatment. Massage - ask someone to massage your painful area using fingers or thumb. Massage using kneading technique. Slightly press the thumb and start rotating as it helps to induce relaxation, relieve pain, and reduce levels of stress chemicals, such as cortisol and noradrenaline. It will give you pain relief for time being. 3. After massage again relax for 5 min and apply any pain relieving ointment eg - volini or oraflame gel etc. Make sure not to do massage with the gel, it should not penetrate the body. Just a topical application of gel should be done. 4. Again relax your body for 5 minutes and perform some normal back extension and hamstring stretching exercises to get relief from your pain. While performing exercises if pain occurs then would recommend you to stop doing exercise. Massage or slightly rub the painful area and consult your near by physio to diagnose the actual cause of the pain as he/she can guide you proper exercise plan which will be beneficial for you after checking grade of your back pain. For any doubt please inform. Best regards.
