It is as important for the man as it is for the woman to undergo certain tests before the IVF cycle is started. Fertility is not only an issue for a woman, but it also might be for the man in question. There are certain tests that need to be conducted on men before they can gear up for IVF:

1. Semen Analysis: One of the most important tests for male infertility is semen analysis. Sometimes, a man may be completely healthy, but his sperm production could be of poor quality. Semen analysis is done to provide an accurate evaluation of sperm number, the ability to move (motility), size and shape (morphology), as well as the consistency and volume of the sample collected. Semen analysis (SA) test includes only the sperm count. Sperm Funtion test (SFT) will include in detail on morphology, motility and other parameters of the sperm.

2. Trial Wash: This is also known as sperm wash. It is mostly done to remove chemicals from the sperm. Not doing a sperm wash might end up affecting the later stages of the IVF treatment. In this process, the misshapen and immotile sperm is separated so a clean sample of motile and healthy sperm can be collected.

3. TUNEL Assay: Since sperms build up about half of a child's genetic makeup, a TUNEL Assay or 'sperm DNA test' is required. This test is done to measure the sperm population that have fragmented DNA. Negative impacts on genetics can hamper the success of the fertility treatment.

DNA fragmentation index (DFI) test is usually recommended to men with:

  • Leukocytes detected by the sperm analysis
  • Age approaching 50
  • A history of diabetes II type or prostatitis
  • Exposure to chemicals or prolonged heat at workplaces

Pregnancy is that wonderful and beautiful phase that most women rejoice about. However, many may find it hard to believe that pregnancy can bring with it a condition known as ‘antenatal depression’. This is a common condition in expecting mothers and should not be ignored.

Depression during pregnancy like regular depression is a clinical condition and requires further investigation and management. Pregnancy is related to the female hormones and therefore, mood swings are common (as during PMS and menopause). However, depressed pregnant women would typically have the following associated symptoms too.

  1. Altered eating habits

  2. Altered sleeping patterns (too much or too less)

  3. Loss of attention/ability to focus

  4. Losing interest in activities which usually interest the person

  5. Feeling very anxious

  6. Constant feeling of sadness

  7. Uncertainty about the future

  8. Feeling of worthlessness

  9. Suicidal tendencies

It has been noted about 25% of women can get depressed during pregnancy for varying periods of time. Some potential reasons for this are as below.

  1. Strained relationships: One of the most important factors for a healthy pregnancy is a happy relationship, not just with the partner, but with immediate and close family members with whom there is a high level of regular interaction.

  2. Work stress: For working women, a stressful office environment could take a toll on their moods.

  3. Previous miscarriages: This can cause anxiety and depression about possible repeat incident.

  4. Potential complications: If the periodic examinations showed up possibility for complications in pregnancy, the chance of depression in the mother increase.

How it affects pregnancy?

A depressed mother would not be able to care for herself and  therefore, the baby may not get adequate nutrition for its development. Additionally, the potential for nicotine and alcohol abuse increases, which also negatively impacts the baby’s growth. There could be low birth weight, premature birth and developmental delays after birth.

How it can be managed?

While the hormonal changes during pregnancy causes mood swings, diagnosing if it is depression is important. A consultation with a psychiatrist may be required for some women.

Some of the options available for treatment include support groups, individual counseling, and medications.

  1. It is important to understand that this is a common condition and you are not the only one going through this. Talking to other women who feel depressed can help in mood uplifting of all involved.

  2. If you are too shy about it, individual counseling could be the next best thing.

  3. Stress management including light exercise, music, meditation, rest, diet, and support from close friends and family are highly recommended.

  4. Since most antidepressants would reach the baby, it is advisable to avoid these less you need them as a last resort.

Laparoscopy, also known as keyhole surgery, is a type of surgery, which involves the usage of small tubes, surgical instruments and video cameras for operations through small incisions or cuts in your body. Though different people experience the surgery differently due to difference in health conditions, there are some points everyone should know about a laparoscopic surgery.

1. The problems that laparoscopy addresses
Conditions like ectopic pregnancy, endometriosis and pelvic inflammatory disorders are generally treated using laparoscopic surgery. Moreover, laparoscopy is also used to remove the gallbladder, appendix, patches of endometriosis or detect adhesions, fibroids and cysts. Also a biopsy of the organs inside the abdomen can be done through laparoscopy.

2. The duration of your stay in hospital
Usually performed on an outpatient basis (release on the same day as the surgery), a laparoscopic surgery may require you to stay overnight at the hospital if your condition requires a complex or lengthy surgery. Moreover, if the doctor feels that a bowel resection or partial bowel resection needs to be performed, you may have to stay at the hospital for a few days.

3. The preparation required pre-surgery
If you are going to have a laparoscopic surgery soon, ensure that you inform your doctor if you are taking any blood-thinners like aspirin or if you are allergic to any medications like anaesthesia. Moreover, let the doctor know if you are pregnant or planning to conceive. After giving all this information to the doctor, strictly follow all the instructions he/she gives you. Also, make sure that someone is available to drop you home after the surgery as you might be too weak or in too much pain to go back home by yourself.

4. Pain management post surgery
Laparoscopy is generally followed by a sore feeling around the cuts as well as shoulder pain. If the pain is unbearable you can ask for medication from your doctor or consider common analgesics. Recovery time for a laparoscopic surgery is only a few days, and to get through this period easily seek the help of a friend or family member to manage your medications and lift your spirits.

5. Restrictions you need to follow during the first couple of weeks
For the first couple of weeks after the surgery, your doctor might ask you to abstain from driving, tub bathing, swimming and having sexual intercourse. Make sure that you follow these rules and get adequate amount of sleep to ensure speedy recovery.

You can go for medical abortion, it has to be done under a gynaecologist supervision. Prior to abortion ultrasound has to be done to confirm the duration and location of pregnancy.

Even if one drop has gone inside vagina, and there is not safe period, there are chances, so to be 100 percent sure, go for pregnancy test.
