It is not enough to quit smoking, which is in itself an excellent thing, but now you must do something to return your ravaged body, mind, and brain back to a healthy state. Depending on when you quit and how much you have smoked, it will take time for your health to revert to old times. To hasten it you must do some effective things immediately. The breathing problem probably indicates that you have already damaged your lungs from years of smoking. The tobacco tar in cigarette smoke lines your lung with huge patches that make that portion dysfunctional. Your oxygen carrying capacity is jeopardized, and breathing is impinged. But don’t be discouraged because it is not too late even now to reverse all the ill-effects of smoking. I don’t know what other organs could be damaged but do go for a thorough medical check up to assess the effects of your abuse. Regarding improving your lungs, do a lot of aerobic exercise breathing through your nostrils only. Then there are a lot of deep breathing exercises in Yoga which you can learn too. Deep breathing cleanses your system and lungs and restores it to good health. Deep breathing uses the 80% of the lungs which are not normally used in normal breathing and even when exerting yourself. The greatest effect is in the lungs because this substance is processed through the respiratory system directly through the lung surface. When quitting, you must take one day at a time; though you must give up all smoking at once, and promptly. If you look too far ahead you will get discouraged and will give up. If you sit and count the number of days you have given up, that will also focus too much on achievement. You must focus on the here and now, on that day to be precise. After all quitting is not that easy due to many factors. So even if you fail one day, you must try and try again. Very few people succeed in their first attempt. Your persistence will eventually pay off. The urge to discontinue is hampered by many factors but the worst of them is the conditioning. In fact these conditions can exist all through the day unlike other addictions. But your desire to want to stop is the most important factor. It is important to find out about the ill-effects of smoking to have a reason that alarms you about continuing in the habit. The information is really scary but true and you must remind yourself about this every day. The nicotine in tobacco smoke stimulates the heart beat to raise the blood pressure in no time at all. So you must avoid it like the plague: it is just very bad medicine for you. In fact it is highly toxic too. You will need to stop smoking or consuming any tobacco related items with immediate effect. You should start vigorous exercises, gradually increasing it over a period of time. Do deep breathing exercises every time you feel like smoking (try Yoga), and replace the oral urge with some healthy food substitute to satiate the need. You may also take hard candy or chewing gum. If the urge is too strong, then use nicotine gums and nicotine patches, for a little while until the smoking drive reduces, substantially. You may also join Smokers Anonymous in your city where the support is really extraordinary for like-minded people who are also in the same situation. Keep yourself occupied or engaged with interesting activities during the times when it is most tempting. Keep the company of non-smokers for some time i.e. at least for the first 21 days. Above all announce it to everyone that you have quit smoking. Here’s a piece of very good advice: even if you accidentally/willfully take a cigarette, it is not the end of the world – you can start the cessation all over again. You must persevere with the best support until you defeat this addiction. Counseling is very useful in the initial stages and when temptations are at their highest.

Multivessel Stenting Vs Bypass Surgery - Which is Better?

The results of a study conducted by the Department of Medicine, University of Ulsan, Korea, show that multivessel coronary stenting can be performed with a high success rate along with an acceptable clinical outcome. Coronary stenting has proven itself to be an accepted means for treating of obstructed coronary arteries. The need for multivessel coronary stenting has been inflated because of the larger number of patients with unfavourable cardiac profiles. Conventionally, bypass surgery is regarded as a standard means for relieving angina in cardiac patients with multivessel coronary disease as it could lead to a downright revascularization. Further, it also allows a prolongation of lifespan in a specified subgroup of patients.

How are they performed: Despite the coming of modern generation of stents, patients with multiple stringent arteries in the heart receiving coronary after bypass have recovered better than those whose arteries were grafted with balloon angioplasty or stenting. This report is presented in the 64th Annual Scientific Session in the American College of Cardiology. This report echoes past studies which shows that patients afflicted with multiple narrowed arteries receive better results with coronary artery bypass grafting, which is also known as CABG or bypass heart surgery. In case of multivessel stenting, which is known as angioplasty or percutaneous coronary intervention or PCI, a stent is put within the arteries to hold it wide open and facilitate the flow of blood.

Which one is better: In a new study, it is reported that patients with new stents are susceptible to 47% higher risk of any of the outcomes like death or cardiac arrest as compared to patients who undergo bypass surgery. In CABG, a vein or artery from other parts of the body is grafted on the constricted coronary artery for allowing easy blood flow to and from heart. This study reinforces present regulations that recommend CABG to treat patients with substantial constrictions in various arteries, a condition often termed as multivessel coronary artery ailment.

Renowned cardiologist Seung Jung Park from Asan Medical Centre in Seoul, South Korea opines that CABG is still a much preferred option on the basis of their medical data. Another study known as Bypass Surgery Versus Everolimus - Eluting Stent Implantation for Multivessel Coronary Artery Disease or BEST trial deserves mention. It is one of the two randomly controlled trials for comparing bypass to angioplasty since the introduction of modernised stents that emits medication, which would prevent blood clot.

This study was implemented on 880 patients at 27 healthcare centres in four countries. Each patient had multivessel coronary artery disease and were determined to be equally eligible to go through either of the methods. Half of these patients were randomly chosen to be given angioplasty with everolimus-eluting stents, and the other half received bypass surgery.

All the patients were tracked for about five years and during this prolonged follow up, angioplasty was related to a considerable upsurge in the incidence of myocardial infarction, target vessel revascularization and often death. Because, it is a more invasive process, bypass surgery is normally recommended only for patients afflicted with higher-risk constrictions in more than one artery.

I am unable to communicate with others freely. I hesitate to talk with any one also I doubt to speak. Chikungunya Fever - Causes and Treatment

The tiny mosquito can pose a lot of health issues. While malaria was the only dreaded disease until some time ago, the incidence of chikungunya is constantly on the rise. In fact, the nonspecific symptoms and absence of vaccination make it extremely difficult to prevent and diagnose this in the early stages.

Causes: First described in the African continent (Tanzania), the word chikungunya means “to walk bent over” in the Makonde or Kimakonde language. This appearance is because of the effect on bones, leading to severe bone and joint pains. More than 90% of the people who are bitten by the infected mosquito develop symptoms.

Chikungunya is a member of the Togaviridae family and is transmitted by bites of the infected female mosquito. In rare cases, the infection can spread through blood transfusions of the infected person.

After entering the bloodstream, it divides and multiplies rapidly in different types of cells including epithelial cells, endothelial cells, and connective tissue cells. These produce the symptoms including the rashes and the joint pains.

Symptoms: The challenge with chikungunya is that there is no specific, telltale symptom indicative of the condition. The patient develops nonspecific symptoms as below, somewhere between 4 to 7 days after the infected mosquito bite.

  1. High fever (as high as 40 degrees Celsius)

  2. Pain involving multiple joints which can be debilitating

  3. Headache

  4. Nausea and vomiting

  5. Muscle pain

  6. Skin rashes

  7. Swelling around the joints

  8. Conjunctivitis

Though the disease is not fatal, it can be quite limiting. The fever and other symptoms may disappear within a week, but the joint pains can last up to 6 months to a year.

Diagnosis: A blood test is the most diagnostic test for confirming the disease. Dengue fever and Zika virus need to be ruled out, as their presentation is very similar.

Treatment: There is no definitive treatment for this condition and it is mostly symptomatic.

  1. The patient is advised rest to recover from the fever and fatigue

  2. Pushing fluids, including juices and electrolytes is recommended to prevent dehydration

  3. Fever needs to be managed with antipyretic medications

  4. Joint pains are managed with nonsteroidal painkillers

  5. Avoid aspirin as it increases the tendency to bleed

  6. Physiotherapy is recommended to improve severe bone/joint pains to improve mobility and reduce swelling

  7. Check with your doctor to see if your regular medications need to be revisited

There is no vaccine to prevent chikungunya, though virus like particles that can be a potential vaccine are in the final stages of trials. Prevention of mosquito bites is also highly recommended as an important step in controlling the disease. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Hello, Its always better to avoid, but if at all it should not go beyond 2 drinks per week...thats max.. Diabetes is all about metabolic problems and alcohol further slows down metabolism. Which is surely for good for the glucose uptake.

Aftr one hour of tooth extraction you can have something cold like ice cream & aftr the effect of numbness goes you can eat anything which is cold & soft like curd rice, daliya, khichdi, just avoid taking hot & hard food for one day & aftr one day you can start with your regular diet.

This may be due to viral infection due to season change do warm saline gargle regularly have homemade freshly prepared lemon juice which is rich in vitamin c thereby increasing your body autoimmune system which prevents infection. In the mean time you can have a tab paracetamol or calpol you may also have a tab cetrezine as and when required. It will be allergic.

Mr. Lybrate-user, diabetes mellitus occurs (t2) either due to lack of insulin or resistance to its action or both. In t1 there is total absence of insulin, beta cells of islets of langerhans in pancreas do not secrete any insulin. Since main reason is insulin deficiency, best drug for treatment is insulin. However, in t2 initially metformin is used, which helps in reducing resistance to insulin action and also helps in weight reduction. Then other drugs known as secretogogues (insulin secretion stimulators) are used. If diabetes is very severe with very high blood sugar levels and hba1c% >9, then insulin is preferred. So it depends on individual case and needs of each patient. There are multiple choices now, recent addition to treatment has been sglt-2 inhibitors, which act independent of insulin.
