I am a 71 year old man, healthy, active and strong. When having sex with wife, I prolong to make her reach her climax as she has circumcised her clitoris and labia minor. It takes time. But in the bargain, my corona becomes numb PERHAPS and do not reach climax, in the end! Please tell me whether it could be from Prostate, or else?

Pigmentation - 10 Ways To Treat it

MBBS, MD - Dermatology , Venereology & Leprosy

Pigmentation - 10 Ways To Treat it

Pigmentation of the skin is a medical term, which refers to the phenomena of colouring. Our skin is made up of cells that produce a pigment named melanin. The colour of the skin largely depends on the production of melanin. If more melanin is produced, the skin looks darker and vice-versa. When the melanin producing cells are damaged due to long exposure to sunlight, pregnancy or any other disease, pigmentation of the skin takes place. The skin may get dark entirely or in patches due to pigmentation. In some cases, decolouration takes place or white patches are formed on the skin.

Causes and Types of Pigmentation

The type of skin pigmentation depends on the cause behind it. Here are some of the common causes and types:

  1. Melanin: The skin colour varies from one person to another due to the presence of a pigment called melanin. It is produced by melanocytes cells in our body. Melanin protects our skin from damage on exposure to sunlight. When we spend long hours under the sun, the melanin production is increased. This gives a tanned look.
  2. Freckles: These are caused by clusters of melanin. These gets accumulated or concentrated in clusters on being exposed to sunrays.
  3. Liver Spots/Age Spots: These are also known as solar lentigos. This can be due to ageing or excessive exposure to UV rays.
  4. Birthmarks: These are referred to the irregular blemish that appears on any part of the body soon after birth or at the time of birth. These are of two types: vascular and pigmented.
  5. Melasma/Chloasma Faciei: This is usually found in pregnant women. It occurs due to the over-activation of melanocytes as there is a hormonal imbalance.
  6. Post-inflammatory hyper pigmentation: This refers to inflammation as a result of inflammation caused by trauma.

Treatment: There are several ways to treat skin pigmentation. There are medicines and cosmetics available in the markets, which help in getting rid of the pigmentation. Skin pigmentation can also be tackled by certain home-based remedies. Here are some of these remedies:

  1. Milk and honey face pack: Milk is known to contain lactic acid. Apply a mixture of milk and honey on the affected part of the skin. This makes the hyper pigmented skin lighter in colour.
  2. Vegetable juices: Some vegetables like tomatoes, cucumber and potato are effective in getting rid of the skin pigments. Cut slices of these vegetables and rub them over the affected parts. The juices from these vegetables will lighten the pigmented part.
  3. Avocado magic: It is advisable to peel off an avocado and apply its juice with honey over the affected area.
  4. Add Vitamin E to your diet: Include foods rich in vitamin E in your meals. It helps in restoring the pigmented skin back to its normal form.
  5. Face mask: There are several face masks available that can work wonder in curing pigmentation of the skin. You can use face packs containing oatmeal, honey, sandalwood, turmeric powder, raw milk, and orange powder.
  6. Sun screen: It is important to guard your skin against the ultraviolet rays. UV rays lead to pigmentation of the skin. So, apply a sunscreen lotion or cream before stepping out of your home.
  7. Take up some exercise: A key to a healthy skin is a good lifestyle. One must exercise regularly to maintain proper blood circulation throughout the body. Proper flow of blood keeps the skin healthy and free from pigmentation.
  8. Avoid consumption of alcohol and cigarettes: The consumption of alcohol and cigarettes leads to the generation of toxins in our blood stream. This is harmful to our skin.
  9. Cocoa butter: It has nourishing and de-tanning effects on the skin. Use a cream that contains cocoa butter extracts on daily basis.
  10. Oil massage: Certain oils like olive and almond are good for treating damaged skin. If you wish to discuss about any specific problem, you can consult a dermatologist and ask a free question.

If you want to lighten your complexion, there are natural remedies that you can try to brighten your skin a few shades. Since many commercial creams are ineffective or unhealthy to use, turning to natural methods to get fairer skin is a smart choice. A common household ingredient — lemon juice — works especially well to lighten skin tone. Staying out of the sun and using natural facial masks will also keep your skin from getting darker. Protect yourself from the sun. Most of the normal darkening and discoloration of your skin is caused by exposure to the sun's ultraviolet (UV) rays. If you're extremely concerned about having fairer skin, staying indoors will keep your skin as light as possible. When that's not possible, protect your skin use SPF sunblock with a minimum SPF of 30. Most dermatologists agree an SPF 15 is not sufficient anymore. Dangerous UVA and UVB rays are still reaching your skin. You want 98% or more of the sun's rays to be blocked. Wear a hat to protect your face when you're outside. Especially in the summer. There are so many fashionable hats these days, you can also be a trend-setter while you are protecting your skin. Exfoliate your skin a few times a week. Doing manual exfoliation can really help to lighten the skin, since the process takes off dead skin cells. Keep in mind that exfoliation only helps if your current skin is darker than your normal skin tone, because it reveals fresh skin that is untouched by the sun. You can exfoliate your skin manually by using a body scrub or face wash with exfoliating particles. To make your own, just add a teaspoon of ground almonds or oatmeal to your regular cleanser. Using a scrub brush or a dry brush is a great way to exfoliate your entire body. For your face, use a special brush with softer bristles so you don't damage the fragile skin there. Keeping your skin healthy by protecting it from the sun, exfoliating and using natural lightening methods is your best bet for keeping it on the lighter side in tone. Remember that consistency is the key, and do your facial treatments several times a week. For medication consult online in private.

Can this pitted scars be healed by creams like maderma or do I need surgical procedure?

Can this pitted scars be healed by creams like maderma or do I need surgical procedure?
There are various treatments for acne scars which include creams, chemical peels, microdermaabrasion, microneedling, radiofrequency, surgery, fat fill laser etc. Creams work only for superficial pigmentation not for scars. You can start initially on milder depigmentation creams which contain kojic acid, arbutin, niacinamide like Melaglow at night and use sunscreen (SPF 35 and above) three times a day. There is options of glutathione injections to oral tablets with vitamin C, also laser toning also gives good results. Laser toning using Q switch Nd Yag laser is an option but results variable. You may need 6-8 sessions at 3 weeks gap. And depending on type of skin whether oily or dry you can choose a face wash. It need examination and personalized plan. You can consult in private with photographs.

Laparoscopic Surgeries - Advantages And Disadvantages You Must Know!

Laparoscopic Surgeries - Advantages And Disadvantages You Must Know!

Laparoscopic surgeries are minimally invasive surgeries where a very small incision is made on the patient’s belly button. After making this incision, a camera called the laparoscope is inserted from the hole and then the condition of the organ is evaluated upon a television screen in order to perform the operation. An air pocket via carbon dioxide is then made for the surgeon to carry out the operation. This procedure, although used widely has a couple of advantages and disadvantages which should be kept in mind before undergoing the surgery.

Advantages of Laparoscopic Surgeries
In comparison to a traditional, large surgery, laparoscopy is much more comfortable and is less painful as well. The small incision takes less amount of time to heal, therefore the patient is allowed to get back to his/her regular activities much sooner. Patients who go through laparoscopic surgeries spend less time in the hospital and get discharged on the same or the next day as well.

Laparoscopic surgeries help in reducing the risk of bleeding during the surgery as the incision is relatively very small than the ones made during a traditional surgery. The amount of blood loss is minimal in such a case and therefore the need of blood transfusion is also decreased, significantly.

Another important factor which can be counted in is that, with the help of laparoscopic surgeries the risk of exposing the internal organs to contamination is also reduced. This allows the chance of post-operative infection risks to diminish as well.

Disadvantages of Laparoscopic Surgeries
Just like a coin which has two sides, laparoscopic surgeries also have its advantages and disadvantages which you need to consider before going through it. One of the most important factors is that laparoscopic surgeries are extremely expensive as the instruments required to carry out the operation is very costly. A number of less affluent hospitals can therefore not afford to have the intrument; this makes laparoscopic surgeries less accessible to a vast number of people.

Laparoscopic surgeries may not be suitable for complicated surgeries. The tools required to carry out a laparoscopic surgery may not be sufficient for an operation which needs a 3-dimensional field of vision. For surgeries where large chunks of tissues are to be removed, these minimally invasive surgeries are not preferred.

Laparoscopic surgeries may also need a second open surgery in case of any complication during the procedure. Thus, the risk of having two surgeries increases for patients who opt for laparoscopic surgeries. These surgeries also need a special skill set and every hospital is not equipped specialized personnel who know how to operate the unit. Therefore, you should consider every aspect before going through the procedure.
