Hi Thanks for your query and welcome to Lybrate. I am Dr. Akshay from Fortis hospital, New Delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - To maintain proper posture of your back while working and sleeping - If pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - Physical therapy initially under supervision of a trained physiotherapist and then to continue at home - Adequate calcium & vitamin d intake if levels are low in body - Ice packs can applied if your pain is acute, then hot fomentation can be done at home - Analgesic spray for local application can be used and is easily available. - Avoid lifting heavyweight/ acute forward bending in mornings etc We will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Its gout pain Your uric acid is high that leads to gout, gout is a form of arthritis characterised by severe pain, redness and tenderness in joints. If you have any symptoms of gout, consult physician for pain relieving medicines at the same time restrict protein intake.

Hi. Is there any history of injury or hurt to the area? any underlying medical condition? the pain in legs can be because of - muscle cramps (due to tiredness or dehydration), shin splints (doing activity over and over on hard surfaces can bring this on. You may also be more likely to get shin splints if you have flat feet or your feet turn outward. Rest your legs to feel better. Ice helps), tendinitis (you can get it from overworking the calf muscle or climbing the stairs, apply ice to get some relief), sprains (do rest, ice, compression & elevation). Other causes are varicose veins, arterial disease, sciatica, diabetic neuropathy. Medication: meantime, take homoeopathic medicines - arnica 200 and rhus tox. 200 - take both of them thrice daily for 1 week. Also, apply schwabe's topi-arnica and topi mp gel on the affected area in circular motions gently thrice a day. And wrap crepe bandage over it. Get back to me thereafter.

I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias ( electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. What medications you generally take and what has been your treatment history till now for existing problem. Please do not get too concerned with mri findings as a lot of people have mild disc bulges but need no intervention and improve with conservative management only. Actually pain at lower leg region just above ankle can be due to many reasons like involvement of tendons, nerves, vascular and some times stress fractures. If there is no associated swelling, redness, inflammation of skin, then my advise for you: - ice packs application intensively with elevation of your limbs at night - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning - slow resumption of physical activities with proper pre match warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - I suggest you to get some blood tests: - serum vitamin d and serum potassium levels, esr and crp once you have all these reports then you can contact me as I can advise you for proper supplementation do not hesitate to contact me if you need any further assistance.

Shoulder pain and injuries

The shoulder is a ball-and-socket joint that has three main bones: the humerus (long arm bone), the clavicle (collarbone), and the scapula (also known as the shoulder blade). These bones are cushioned by a layer of cartilage

The shoulder joint is a very mobile joint which makes it very susceptible to injury. It moves the shoulder forward and backward. It also allows the arm to move in a circular motion, and to move up and away from the body.

Shoulders get their range of motion from the rotator cuff. The rotator cuff is made up of four tendons. Tendons are the tissues that connect muscles to bone. It may be painful or difficult to lift your arm over your head if the tendons or bones around the rotator cuff are damaged or swollen.
You can injure your shoulder by performing manual labor, playing sports, or even by repetitive movement. Certain diseases can bring about pain that travels to the shoulder. These include diseases of the cervical spine of the neck, as well as liver, heart, or gallbladder disease.
You're more likely to have problems with your shoulder as you grow older. It is especially common after age 60. This is because the soft tissues surrounding the shoulder tend to degenerate with age.
Most shoulder pain problems are:

  •   tendonitis/bursitis, injury or instability of the joint, or arthritic disease.
  •   rotator cuff syndrome is a strain or sprain or tear of the tendons that make up the rotator cuff.
  •   rarely, tumors, infections, and nerve damage cause shoulder pain.
  •   much neck pain is caused by soft tissue damage due to injuries such as whiplash or wear and tear due to overuse.
  •   damage to spine joints and bones, such as cervical disk degeneration, arthritis, or traumatic injury can cause severe pain and disability.
  •   fibromyalgia (whole body pain syndrome) can contribute to neck and upper back pain. Poor posture while performing everyday activities can also cause significant neck pain. Rarely, infections or tumors will cause neck pain.
  •   pinching a nerve in the neck or shoulder, or breaking a shoulder or arm bone, are also causes of pain.
  •   a frozen shoulder is when tendons, ligaments, and muscles stiffen and become difficult or impossible to move.
  •   a dislocated shoulder is when the ball of the humerus pulls out of the shoulder socket. An injury due to overuse or repetitive use can cause injury

Hello Doctor, I have cervical spondylitis. From past 4 days I have pain in my back neck and front left side shoulder which is passing down to my chest. I feel it's some pinched nerve problem. The pain is coming to my throat also. I took medicine tolpidol. Brufen.flexura d and now neurobion forte but nothing is working on me. I feel better only when I take hot water shower. I also have fatty liver issue and Bp problem. should I ask Dr. to give injection as oral medicine are not helping me. I am really having discomfort while sleeping as I feel pain and spasm in chest area and sometimes burning sensation in back head. Please help what should I do Its been 5 days now pain is not going.

Spondylitis is one of the most common causes of back and neck pain, and results from inflammation of the vertebral joints. It is curable this is cervical pain (neck pain) and that's the reason the pain radiates until the shoulder blades. If you keep ice that would help to reduce the pain and feel relaxed and comfortable. You shall also do hot water fermentation that would improve the blood circulation to the painful area and thereby the pain would be reduced. You shall use cervical collar which would help you to reduce the radiating pain and also that would improve the cervical angle and improve the anatomical posture. The pain actually triggers due to nerve compression and that would definitely improve by wearing a cervical collar. You have to go for intermittent cervical traction and also you can do interferential therapy. Please consult the near by therapist for quick relief. Since you have pain for more than 10 years it is suggestible to go for cervical muscle strengthening exercises and also to use cervical collar to improve the contour of your muscle texture and to improve the cervical angle so that the posture is improved and it also gives you comfort and relieves the pain. It looks like you are anaemic. That's the reason your body becomes tired though you had good sleep during the night. The pain in the shoulder and back shows that your muscles become too tired especially in the area of the body where you get maximum body weight (ie. Back) and shoulder which moves excessively during the day for any activity which we do with our hand. If the pain subsides after the day starts ie. Once your metabolic activity starts in the body then you become alright, so you can take bath in warm water which would help you to break the tiredness. And also you can check with your calcium & vitamin levels in your body by consulting a general physician. You can also improve the protein diet in your body. Whenever you do breathing in and out you will feel pain in the upper back, middle back and also the pain will radiate upwards until the shoulder and also up to the chest. You might feel chest congestion and tightness and that be due to gastric trouble. Kindly do breathing exercises which might help you to get reduced with pain in the back and shoulder. Drink hot water to reduce the gastric trouble.

I am 23, having a pain just above left lumbar side from last 2 year, especially when I lay on bed night. Now a days it began to feel in day time also. I did sonogram test but found nothing. Hard exercise sometimes helps to recovered from the pain for a few hours. What can I do.

Firstly avoid toward bending and lifting heavyweight then start heat fermentation twice a day and apply pain relief ointment. Sit straight and start spine extension exercise.

Apply hot fomentation twice daily. Avoid bending in front. Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang asana -- lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core strengthening exercises- straight leg raised with toes turned outward, repeat 10 times, twice a day.

Follow the foods regularly. Black grapes 100gm/day. Musturd oil 2 teaspoons/day. Whey protein food supplements regularly. Aswagandha, spirulina capsules twice a day. Soya beans and sprouts nuts & cereals regularly. If you are not recovered within 3 weeks, take the acupuncture treatment or privately.

Anti-inflammatory drugs may help a little, here's what you can do instead
Back pain can really get disabling. The nagging pain can not only put you on stress but also meddles with your daily routine. According to a recent study published in'annals of the rheumatic diseases, non-steroidal anti-inflammatory drugs (nsaids) may help a little in getting rid of back pain. On the other hand, excessive usage of such drugs may cause bleeding and ulcers in stomach among other gastro-intestinal problems.

Experts from the george institute for global health, australia conducted experiments to test the effectiveness of existing medicines that used commonly used for treating back pain. The team examined more than 6000 people from 35 trials.
 It was found that only one in every 6 patients received any considerable benefit from consuming such medications.
The team also brought to light the fact that, in most cases, the effects of nsaids are short-lived and often harm people's health. Taking these facts into account, the experts urged to focus more of preventive measures over treatment.

Back pain can be caused by a host of different reasons. From an old injury resurfacing, to excessive sitting, sedentary lifestyle and incorrect posture - the culprit can be any or a cocktail of all. Prevention would certainly involve taking short breaks at work, investing in physical exercise, practicing flexibility enhancing techniques like yoga, assuming a correct sitting posture and eating a balanced diet. A series of backward bending yoga postures can help relieve back pain. Some of these would include
