Trigger Thumb

(syn. Stenozing tenosynovitis of flelexor pollicis longus tendon)
Trigger thumb is a painful clinical condition in which the terminal phalanx if the thumb gets locked in a state of flexion. Sometimes it is bilateral i. E. Involving both hands. Big toes of the feet are usually not involved. The condition at some stage may become painless. It is more common in diabetics.
Trigger thumb is caused as a result of inflammation of the tunnel of the thumb in which the (flelexor pollicis longus) tendon moves. As a result the tunnel gets narrowed, usually at the metacarpo phalangeal joint and arrests the gliding movement of the tendon. Sometimes there is a nodule formation in the tendon at the same place producing obstruction to the tendon movement.
Treatment
Non steroid anti inflammatory drugs help in reducing pain and swelling. Physiotherapy helps in regaining movements of the joint. Sometimes in local infiltration with steroids gives relief. However long term effects of steroids are not desirable.
Surgical treatment if the medical treatment does not succeed, then surgical release of the tendon should be undertaken. It a one stitch surgery which can be done under local anesthesia with constantly good results.
Congenital trigger thumb some children are born with it. The congenital variety is usually bilateral. In these cases the treatment is surgical only. It should be done early otherwise the terminal phalanx of the child will not grow and remain small in size.

Hi, I recently completed one year of treatment for Spinal TB with 100% Adherence to medicines. During the treatment, I never had any pain which initially existed and was able to lead a very normal life like others. Once I stopped medicine, I started feeling slight pain at the same place where it all started one year back. While the pain is not as bad as it was, I am just worried about why it still exists. I was advised to go for Physio and attended IFT sessions which considerably reduced paid but I still feel a pain like a pinch between my buttocks and lower spine. Any advice will be of great help. Let me know if I should see a Neuro to ensure my nerves are in good shape after being infected with TB bacteria.

The most common reason for persisting pain might be the residual damage that tb might have caused to the spine in the form of fibrosis or scars. If you develop again symptoms like severe pain, numbness. Weakness fever night sweats weight loss you should be worried about tb again other wise it might be sequalae. Its always better to take concerned specialist advice, so no harm in taking neuro consultation.

Dear lybrate-user, mood stabiliser tablets are given for many years. Stopping them will cause a reappearance of illness. If you have started again taking tablets use some family planning method like condom to avoid pregnancy, after getting out of this episode of depression, discuss with your psychiatrist about planned pregnancy. He or she will help you to stop medication for few months, say 12th week of pregnancy and restart comparatively safe mood stabiliser after the first ultrasound of the baby at 11th week. Sometime she may ask you to take folic acid tablets also for few months of pregnancy to see that you get a normal child.

The treatment for pimples will depend on the intensity of acne and the type of acne. You will need good facewash, topical creams and oral medicines along with anti dandruff shampoo.

Hello. Start taking nutritious, healthy food and daily yoga or some kind of exercises, it will be automatically reflected on your face. Yes, the colour which is inherited from your parents/ ancestors, cannot be changed, but you can try to get glow on your face!! Daily Regime: Apart from these, you do follow CTMP, i.e., cleansing, toning, moisturising and protection (apply liberally sunscreen with SPF-30 – a thick layer of Bakson's Suncare cream may be, 20-30 minutes before going out daily and repeat it's application after every 3-4hrs.) after washing your face daily; use the products as per your skin type. This will change the skin texture and skin tone. In case you do have tanning, apply honey with a dash of lime on your face. Eat right and healthy that too on time, relieve your stress/ tensions, sleep well, stop smoking, as this restricts blood flow to your face and leaves you with a greyish complexion. lead a healthy lifestyle! Apart from this, avoid anger, anxiety, fear which increases stress hormone in the body and subsequently makes your complexion darker! (Hormonal imbalances also plays a major role). If the dull and dark skin tone is due to any underlying ailment or diseases, need to look into the matter. If you feel your skin is weak or not tight enough, that usually suggests a lack of Vitamin A. Medicine: Use homoeopathic cream - Medicynth's Whitch Hazel or Bakson’s Fairness cream daily at night. Home remedies: Apply fresh-squeezed lemon juice to your entire face and neck. Leave it on for 10 minutes and then wash it off with lukewarm water. After this, rub cucumber slices on your skin to soften and moisturize it. Do this daily or every other day. Another option is squeeze the juice of one-half lemon and mix one to two tablespoons of raw honey in it. Apply it on your face, leave it on for 15 to 20 minutes and then rinse it off. Pl take care of your eating habits. If you feel burning in stomach, then you better is to take fruits and reduce regular meals. Sometimes, simply eating less takes care of stomach trouble. A use ful way is to divide both lunch and dinner into two or three halves each, with interval of two hours between each half, so that through out the day you eat sufficiently but also avoid over loading of food on stomach and thus help to prevent acidity, indigestion etc Use parsley and jeera seeds while cooking vegetables. Please make it a point that too bitter, too sour, salty, hot, pungent, dry and burning foods are to be avoided as such foods cause distress, misery and disease. Thus deep frying foods or foods taking lot of oil as in gravy vegetables, salty foods, spicy foods, long preserved, frozen foods, junk foods like pizzas burgers chips are to be avoided. Drink two glasses of warm water on empty stomach every morning about 30 minutes before breakfast. Try it! This works amazingly! In addition, walk fast for 30 minutes every day or engage in some service every day. Please also do meditation regularly and sincerely which has been scientifically proved for overall health.

If periods have occurred after intercourse then there is no chance of pregnancy. Delayed periods may be due to some other reason. Consult nearby.

First of all check your posture when you are lifting any material like heavy bucket. And try to lift the heavy bucket without bending your back. You may lift the bucket after bending your knees only. Fever is not considered medically significant until body temperature is above 100.4 F (38 C). Fever serves as one of the body's natural defenses against bacteria and viruses which cannot live at a higher temperatures..Also, the body's defense mechanisms seem to work more efficiently at a higher temperature -Fevers of 104 F (40 C) or more may be dangerous and demand immediate treatment The following medications may be used at home for mild fever : -Acetaminophen (dolo-650 mg) twice or thrice can be used to lower a fever . Aspirin should not be used in children during a viral illness. -Wear comfortable loose fitting clothes and donot overdress. Overdressing can cause the temperature to rise further. -Tepid water (85 F [30 C]) baths may help bring down a fever -The body should be cooled with a wet sponge. -Fever can cause dehydration.Drink plenty of fluids.Have citrus fruits, particularly Vitamin C rich ones like mausami,Amla juice etc. -Take light and easily digestible home cooked meals .avoid outside food -Wash your hands frequently with soap and warm water especially before meals, or after returning from a public places or using public transport. -Further therapy depends on the cause of fever like urinary infection/typhoid or malaria/dengue infection and the accompanying symptoms that need to be investigated and treated accordingly

With proper management of Diet one can control diabetes but cannot remove it altogether. To help manage your diabetes, your meals need to be: • Regular and spread evenly throughout the day • Lower in fat, particularly saturated fat • If you take insulin or diabetes tablets, you may need to have between meal snacks Matching the amount of food you eat with the amount you burn up each day is important. Not putting too much fuel in your body (keeping food intake to moderate serves) is vital to getting the right balance. Along with healthy eating, regular physical activity can help you to manage your blood glucose levels, reduce your blood fats (cholesterol and triglycerides) and maintain a healthy weight. . It is important to recognise that one diet does not fit everyone with diabetes. So my slogan for all diabetics is 'One Diet does Not Fit All'. So every individual diabetic should consult a dietician to plan a way according to individual body,tastes and requirements.Only then can you control your blood sugar levels. Fat Fats have the highest energy (kilocalorie) content of all foods. Eating too much fat can make you put on weight, which may make it more difficult to manage blood glucose levels. While it is important to try and reduce fat in your diet, especially if you are trying to lose weight, some fat is good for health. Saturated fat It is important to limit saturated fat because it raises your LDL (‘bad’) cholesterol levels. Saturated fat is found in animal foods like fatty meat, milk, butter and cheese. Vegetable fats that are saturated include palm oil (found in solid cooking fats, snack foods or convenience foods) and coconut products such as copha, coconut milk or cream. To reduce saturated fat: • Choose reduced or low-fat milk, yoghurt, ice-cream and custard • Choose lean meat and trim any fat off before cooking • Remove the skin from chicken (where possible, before cooking) • Avoid using butter, cream, sour cream, coconut milk, coconut cream and hard cooking margarines • Limit the amount of cheese you eat and try reduced-fat and low-fat varieties • Limit pastries, cakes, puddings, chocolate and cream biscuits to special occasions • Limit pre-packaged biscuits, packet snacks, cakes, frozen and convenience meals • Limit the use of processed deli meats ( chicken loaf, salami etc) and sausages • Avoid fried takeaway foods such as chips, fried chicken and battered fish and choose BBQ chicken (without the skin) and grilled fish instead • Avoid pies, sausage rolls and pastries • Rather than creamy sauces or dressings, choose those that are based on tomato, soy or other low fat ingredients. As some tomato and soy sauces can be high in salt, choose low-salt varieties or make them yourself without any added salt. • Limit creamy style soups. Polyunsaturated & monounsaturated fats Eating small amounts of polyunsaturated and monounsaturated fats can help ensure you get the essential fatty acids and vitamins your body needs. Polyunsaturated fats include: • Polyunsaturated margarines (check the label for the word ‘polyunsaturated’) • Sunflower, safflower, soybean, corn, cottonseed, grapeseed and sesame oils • The fat found in oily fish such as herring, mackerel, sardine, salmon and tuna. Monounsaturated fats include: • Canola® and olive oils • Some margarines • Avocado. Seeds, nuts, nut spreads and peanut oil contain a combination of polyunsaturated and monounsaturated fat. Ideas for enjoying healthy fats • Stir-fry meat and vegetables in a little canola® oil (or oil spray) with garlic or chilli • Dress a salad or steamed vegetables with a little olive oil and lemon juice or vinegar • Sprinkle sesame seeds on steamed vegetables • Use linseed bread and spread a little canola margarine • Snack on a handful of unsalted nuts, or add some to a stir-fry or salad • Spread avocado on sandwiches and toast, or add to a salad • Eat more fish (at least three times a week) because it contains a special type of fat (omega-3) that is good for your heart. • Do more dry roasting, grilling, microwaving and stir-frying in a non-stick pan • Avoid deep fried, battered and crumbed foods Carbohydrate I recommend to eat a moderate amount of carbohydrates for all Diabetics means, on a plate, approx, 1/4 of the plate should be carbohydrate foods. Carbohydrate foods are the best energy source for your body. When they are digested they break down to form glucose in the bloodstream. Insulin takes the glucose out of the blood and deposits it into the muscles, liver and other cells in the body where it is used to provide energy. A regular carbohydrate intake is required to provide our body and brain with instant energy. Most foods contain carbohydrate and also provide us with fibre, vitamins and minerals. Very low carbohydrate diets are not recommended for people with diabetes. If you eat regular meals and spread your carbohydrate foods evenly throughout the day, you will help maintain your energy levels without causing large rises in your blood glucose levels. If you take insulin or diabetes tablets, you may need to have between meal snacks. Discuss this with your doctor or dietician. All carbohydrate foods are digested to produce glucose but they do so at different rates – some slow, some fast. The glycemic index or GI is a way of describing how a carbohydrate containing food affects blood glucose levels. The type of carbohydrate you eat is very important as some can cause higher blood glucose after eating. The best combination is to eat moderate amounts of carbohydrate and include high fibre foods that also have a low GI. Sugar A healthy eating plan for diabetes can include some sugar. However, it is important to consider the nutritional value of the foods you eat. In general, foods with added sugars should be consumed sparingly (manufacturers sometimes use fruit juice or other sources of sugar to avoid using table sugar). In particular, high energy foods such as sweets, lollies and standard soft drinks should not be consumed on a regular basis. Some sugar may also be used in cooking and many recipes can be modified to use less than the amount stated or substituted with an alternative sweetener. Select recipes that are low in fat (particularly saturated fat) and contain some fibre. Alternative sweeteners The use of intense sweeteners by people with diabetes is preferable to use of natural sugars. Protein Choose protein foods that are also low in fat. This will help to reduce the amount of saturated fat you eat. Protein foods include lean meat, poultry without the skin, seafood, eggs (not fried), unsalted nuts, soy products such as tofu and pulses (dried beans and lentils). Herbs, condiments & drinks You can use these following foods nd add flavour and variety to your meals. • Herbs, spices, garlic, chilli, lemon juice, vinegar and other seasonings. • Products labelled ‘low calorie’ (e.g. low calorie/diet soft drinks, low calorie jelly). • Tea, coffee, herbal tea, water, soda water, plain mineral water. • Limit your alcohol intake if you choose to drink

Dear Bhawan, It is your your hormones which have made you fat and keeping you fat. Unless we make correct diagnosis and and create a diet and lifestyle plan to correct that hormonal abnormalities you will not be able to lose weight for long term. Most diet fails because they are low calorie and low fat. Instead of correcting metabolism, they make you feel low energy or makes you feel excessively hungry. If you are doing regular exercise, it will increase your fitness but it does not reduce weight much. I am sure you did not gain this much weight suddenly but over a period of time. To achieve weightloss - think long term. Weight loss achieved over short term, always comes back. Try this general approach (see below) for 4 weeks. If this works then continue. If you have tried this in past or if it does not work, then approach us for weightloss package of 6 months by lifestyle modification - combination of nutritional plan, stress management, exercise prescription for your current fitness level and sleep improvement, (if required). 1. Do not eat anything sugary or sweet. Zero sweetness in diet. 2. Eat plenty of salad in lunch and dinner along with other foods. 3. Do not drink any liquid calorie like cola or fruit juices. 4. Continue to do exercise every day, don't quit. It will help you in million other ways... My best wishes.

yes.. that yellow spot can b wash off.. for that you may be required the treatment called the bleaching of teeth aur may be requred other treatment of the tooth whitening for that you should visit ther dentist
