Hello. Ask her to maintain a right standing and sitting posture, keep your back always straight, avoid carrying weight on back. Hold or carry weight divided equally in both the hands or shoulders (to avoid over straining on one particular hand or shoulder). At her age, low calcium in bones also leads to backache. Exercise: Practice Shavasana or Corpse Pose as it looks like a dead body. If practiced correctly and with full efforts, can relax every body part, relaxing not only the body but also the mind: Spread the legs one to two feet apart, the toes are turned outwards, the heels facing each other, a comfortable distance apart. Bring the arms a little away from the body, palms turned upward. Relax the neck and allow it to turn to the side if it is more comfortable. Close the eyes and focus the attention on the body, breathing normally. Management: -Stretching or activities that place additional strain on the back are discouraged. -Sleeping with a pillow between the knees while lying on one side may increase comfort. -Lying on your back with a pillow under your knees would also relieves pain and no pillow under your head. -Some people seem to benefit from the use of ice or heat. Take care, do not use a heating pad on ‘high’ or place ice directly on the skin. Medication: Give her homoeopathic medicines - Arnica 200 and Rhus Tox. 200 - both of them thrice daily for a month. Also, apply Schwabe's Topi-Arnica and Topi MP Gel on the affected area in circular motions gently thrice a day. Apart from that give her calcium tablets also - Calcarea Phos. 6x/ thrice a day for 3-4months, then discontinue in between.

Lybrate-user, madam, looking at her height and weight it is clear that she has (bmi 51.94kg/m2) morbid obesity, which is the main reason for her problems with joints and joint pain. Unless the weight is reduced joint pain will not go away. She may have to look the option of bariatric surgery, which can help in reduction weight.

Hi thanks for your query. If you have only localized shoulder pain, then you have to rule out any injury as a cause for it. There are many causes of shoulder pain but we have to look out for a cause as treatment is dependent on it. If there is no swelling/fever etc, then I think intensive physical therapy with a short course of anti-inflammatory medications can be tried, if it does not improve in next 2-3 weeks, then we will have to evaluate to find out a cause and treat accordingly. Do not hesitate to contact me if you need any further assistance. Thanks & regards Dr Akshay Kumar Saxena

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

Hello, thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are many causes of pain in lower back region. It can be due to faulty posture, related to weak muscles, issues with intervertebral disc, narrowing of spinal canal, any bony instability besides many other, so accurate diagnosis is the key to proper management of the condition. As you have an acl injury your stability to the knee joint will be reduced. I would firstly suggest you to wear a knee cap as you walk or climb stairs. Secondly I will list down a few important exercises which you should do daily morning and evening. If you do these exercises you will develop good muscle strength that will support and hold your knee from further damages. Secondly you will improvise on joint proprioception (joint sense) which is important as the knee has to understand various movements. You should also aim for good knee movements and build up the tone of muscles. Let me list the exercises that you could do for the knee. > press your knee down on a towel for about 10 to 15 seconds. You should be in the lying down position. Hold and then release your knee. > press your heel down on a towel again in the lying down position. You must hold the heel in the pressed position for 10 to 15 seconds. > lie down and raise your leg to about 45 degrees and hold for 15 seconds. This will build up quadriceps strength. > start bending your knee in the lying down position and in the sitting position. You must be aware of sitting on a high seat so that your knee will bend much more. > you could use a theraband which has a little resistance so that you can bend your knee with some resistance. > squeeze a towel inbetween your kness in the lying down position. Kindly follow all these exercises so that you will gain good strength to the knee and you attain good stability to the knee so that you can get back to walking jogging and playing once again. As far as possible please dio these exercises so that you could also avoid surgery. Please get back to me after two weeks.

You will have to elaborate your problem as to a detailed discussion of your back pain which means duration of pain and mode of onset. Hi lybrate user, first there is no abnormal growth of disc rather compression of disc and waking of posterior annulus causing disc herniation. I believe surgery done was probably laminectomy or micro discectomy. In 60% of patients undergoing open spine surgery gets pain after 2/3 years not because surgery was not done properly but because of manipulation of dural membrane which causes adhesion & then this condition is labelled as failed back surgey syndrome (fbss). Treatment of fbss depends on current symtoms and mri findings. If there is no acute or recurrent herniation then something called adhesinolysis (percutaneous neuroplasty) can be done to break all the adhesions. If there is recurrent herniation then it can be removed by endoscopic spine surgery without manipulating dural membrane, if above mentioned procedures not possible due to severity of degeneration or migration of disc or ligament flavum hypertrophy then spinal cord implant or intrathecal pump can be placed for pain. Hope it helps.
