dear sir, for suspected adrenal Tumors you can get blood and urine investigations done along with radiological scanning. Blood investigations may be related to assessment of hormonal levels like testosterone, aldosterone levels, cortisol levels and radiological scanning may be CT or MRI to know about local extent. You have to be in consultation with a physician for the same.

There are therapies besides surgery for brain tumors, after which inoperable tumor after shrinkage could be taken for surgery and will have cure.

Better to go for surgery if operable. I have seen many patients go for homeopathy, Ayurvedic or Unami medicine, some of which contain steroids which suppress the cancer for a few weeks, but does not cure it. It later comes back with a vengeance. Even metastatic at times, when it is not curable.

Causes and symptoms of Head and Neck Cancer

My name is Dr. Manish Julaha and I practice Head & Neck Oncosurgery. Today I'll tell you some facts about Head and Neck cancer how to predict them early. So in India approxiately 1.2 million cancer patients are detected every year, and out of this one thirds of the number occur in head and neck, mainly oral cavity and throat. And this is a very big number if you compare with rest of the countries because head and neck cancer they account for less than 10% in western countries and less than 4% in U.S. So what is the cause of such high number of head and neck cancers in India. It is mainly the use of tobacco and another sad fact about head and neck cancer in India is that majority of the patients, you can say more than 65% are present in stage 3 or stage 4. And we say cancer is curable if it is detected early and when patients presents to us in such late stages it effects all the paramaeters like overall survival, treatment outcome, side effects, functional outcome, quality of life everything is affected. And when patients come in early stage, the treatment is less invasive with less side effects and the functional outcome is very good and the chances of recurrence is very less.

So what can we do to detect head and neck cancer at the earliest. We can do this if we've the knowledge of the signs and symptoms which hint towards the head and neck cancer and by doing self examination. So what are the early signs and symptoms. They're mainly any ulcer in the oral cavity which dosen't heal in three weaks or any lump in the lung or oral cavity, any red or white patches in the oral cavity, or there maybe unexplained earache or nasal blockage, mainly on one side, specially with blood stained mucous discharge, ill fitting of denture because of any swelling in the gums. So, these are the early signs and symptoms. If we find any such thing we should take expert opinion. And by doing self examination we can detect head and neck cancer at the earliest. I would say all those people who're at a high risk of getting head and neck cancer they should do the self examination at least once a month. So who're the people who're at high risk of getting head and neck cancer. Most importantly those who use tobacco in any form, then elderly people above the age of 45 and people who have exposure to ionizing radiation in past, and people who have had head and neck cancer in the past they're at high risk of getting a second cancer so they should also be very careful.

Now, how to do this self examination. It is very simple, to do this you've to stand in front of the mirror and you've to feel both the lips, upper lips and lower lips, using your thumb and index finger feel both the lips and by pulling them outward you've to see the inner lining of the upper lip and lower lip and also you have to look at the upper gums and lower gums. And using your thumb and index finger pull your cheek and look at the inner lining of your cheek on both sides and look for any ulcer or lump or any red or white patches. Now just open your mouth and say "aaa" and while saying "aaa" just look at the throat and also at the roof of the mouth. Then by touching your tongue on the back of upper teeth have a look at the under surface of the tongue and the floor of mouth. Then pull your tongue outward and look at it's upper surface, by moving the sideways look at the sides also. Again look for any ulcer or lumps or any red or white patches and you have to examine your neck also. Just by standing straight palpate below the jaw bone and above the collar bones, and by moving the head backwards look for any asymmetry of the neck, if there is any fullness on one side as compared to the other side, there maybe some lump. If you find any such thing just take expert opinion.

If you have felt a lump in the breast, it is better to show it to a Breast Surgeons a Surgical oncologist. They would take a detailed history and examine you and if need be, ask for an ultrasound examination. Not all breast lumps need surgery to remove the breast. Fibrocystic changes are very common and are common cause of breast pain especially cyclical. Some times some of the cysts may enlarge causing a lot of pain, which need to be aspirated. Also they need to rule out other causes of pain and lump like trauma, abscess, granulomatous mastitis, fibroadenoma, phylloides and rarely amongst a few. My present abdomen report- Liver- appear normal Gal bladder- CBD is echo free and measures 4.5 mm approx. Pancreas- normal Spleen- 80 mm x 41 mm Kidney- both are normal size shape position and axis right kidney show cyst measuring 1.8 cm and 2.0 cm approx. Left kidney show cyst measuring 4.2 x 3.7 cms approx. There is no evidence of renal calculi on either side. Right kidney 86 mm x 36 mm Left kidney 87 mm x 43 mm Urinary bladder is normal in size shape and contour no intraluminal lesion seen. Prostate glad is enlarge in size shape and echo pattern prostate measure 37 mm x 40 mm x 42 mm. Prostate volume is 33.3 ml BLOOD REPORT Haemoglobin 10.8 gm/dl W.B.C - 6300/cu. Mm neutrophil - 63% Lymphocytes - 30% eosinophil - 06% monocyte - 01% Basophild - 00% Erythrocyte sedimentation rate 1st hour - 78 mm 2nd hour - 102 mm Average - 64. 5 mm Blood sugar - fasting 116 mg/dl. Post prandial - 174 mg/dl TAKING medicine :- 1. Cap pantocid - L - one in morning empty stomach. 2. Syp. Duphalac - 10 ml two time. 3. Tab. Flodart plus - one at nite. 4. Tab. Niftas 100 mg - two time morning and evening. 5.syp.sucral- three time. 6.cap.becosules z.7.glucobay 25 - morning. 8.glyciphage 500 mg one time. Steel feel heavy stomach not feeling hungry.

Hi Mr. lybrate-user, I appreciate you seeking help. Well, for the medical conditions described, I believe his physician has already prescribed the needful medications. However, for the appetite issue that he has currently, I would suggest that he be given small and frequent meals instead of large ones. Again the food given should suit his taste, and yet healthy/ balanced. A cup of soup, with little bit of healthy spices, given especially before his lunch/ dinner can be of help in improving his appetite levels too. Walking daily, to the extent he can safely, is recommended too. The above-mentioned is subject to his physician's concurrence lest he/ she has any reservations here. Hope this helps. I am currently taking tamdura in the night which causes to wake up thrice daily for urination. Could I have the tablet during the day.

Stop taking fluids after 7 pm in the evening. It is because of increased fluid intake after 7 pm. It is not because of medicine.

Sir actually my mother diagnosed with B- ALL and now she is admitted at cancer hospital at Jaipur under the guidance of Dr. She is in induction phase of chemotherapy (just got 15 days treatment) n in her csf report. There are some blast cell also. So I want a personal opinion from you sir. That what should I do.in such situation. In mid of treatment. please help me sir. I am also a doctor and I wan your help sir.

Blasts in CSF is not a rare finding in B ALL. Majority of them get with cured with treatment, though there is a slightly increase risk of CNS relapse in such cases. Your mother will receive little high doses of chemotherapy along with particular drugs which have a higher CNS penetration and intrathecal injections. She may require radiotherapy to brain too. CSF needs to be tested for blasts during subsequent intrathecal injection till it becomes negative.
