Steps Involved in Knee Replacement Procedure

M. Ch. (Ortho), MS - Orthopaedics, MBBS
Orthopedist, Delhi
Steps Involved in Knee Replacement Procedure

Knee replacement is a procedure where the weight-carrying surfaces of knee joint are replaced surgically to ease the pain or any disability. People suffering from osteoarthritis, rheumatoid arthritis or psoriatic arthritis undergo knee replacement. All these conditions revolve around stiffness and painful knee. This surgery is usually performed on people aged over 50.

Surgery Types:
Knee replacement is mainly of two main types:

  1. Total knee replacement where both the sides of knee joints are replaced
  2. Partial knee replacement where only single side of the joint is replaced

Procedure: In case of partial knee replacement with minimal invasion, a smaller incision, which is 3 to 5 inches, is required. This leads to minimal tissue damage and the surgeon can work between the fibres of the quadriceps muscles. Here, an incision through the tendon is not required. This may result in less pain, recovery time is reduced, and motion is better as scar tissue formation is less.

In total knee replacement, four steps are performed:

  1. Removal of damaged cartilage surfaces, which is at the ends of the femur and tibia, with a small quantity of underlying bone.
  2. Replacement with metal components, which help as a recreated surface of the joint
  3. Incision of knee cap with a resurface made of a plastic button, which is optional based on the case
  4. Insertion of a medical grade plastic spacer amid the metal components. This creates an effortless gliding surface.

After general or spinal anaesthesia, an incision of 8-12 inches is made in the front part of the knee. Joint part which is damaged is removed from the surface of the bones. The surfaces are then formed in a way to hold a metal or plastic artificial joint. The thigh bone shin as well as knee cap is attached to the artificial joint with either cement or a special material.

After Effects of the Procedure: After the surgery, patients may stay in a hospital for three to five days. Post surgery, notable improvement can be seen after a month or later. The patient is gradually relieved from pain with the construction of new gliding surface during surgery.

There will be slow progress in the movement. In the beginning, one may walk with a support of parallel bars and then with the help of crutches, walker, or cane. After full recovery in about six weeks, people can enjoy normal activities except running or jumping.

Presently, over 90% of total knee replacements function well even after 15 years of surgery. Hence, knee problem is no problem at all!

3 Strengthening Exercises For A Frozen Shoulder

BPT, BPTh/BPT
Physiotherapist, Lucknow
3 Strengthening Exercises For A Frozen Shoulder

Frozen shoulder (also known as adhesive capsulitis) is a condition in which the shoulder is stiff, painful, and has limited motion in all directions. A person suffering from a frozen shoulder can be pretty uncomfortable.

Frozen shoulder treatment should be done immediately once it has been detected. Stretching exercises are considered as the best-frozen shoulder treatment exercises. A proper warm up becomes very necessary before any exercise.

Making the area warm by using a heating pad is one of the warm-up methods. However, a warm shower covers it all.

Physiotherapy for frozen shoulder includes various exercises that should be first confirmed by a doctor. There are a number of exercises for frozen shoulder. Some very important frozen shoulder exercises are listed below:

  1. Pendulum Stretch: To start with, you can do the pendulum stretch. This dictates that the shoulders should be in a state of relaxation, initially, following which the arm which is affected should be allowed to hang down slightly. Once this is done, the arm is to be swung in a small circle i.e. about a foot in diameter. This is to be done consistently so that there can be a gradual improvement in the condition. As there is an improvement, the size of the circle can be increased and once the shoulder can take sufficient weight again, minor weights (up to five pounds) can be used.
  2. Finger Walk: A simple exercise which can be done almost anywhere is the finger walk. What this exercise entails is that the person stands about three-fourths of an arm’s length away from a wall and then places his or her fingers on the wall itself, at around the level of the waist. The fingers are to crawl or walk up the wall till the point that it becomes a little uncomfortable. While this is just one repetition, after the fingers have been lowered, it is to be done again. In order to have the best results in combating frozen shoulder, the finger walk exercise is to be repeated about twenty times in a day.
  3. Towel Stretches: Towels do not serve a greater role apart from drying up post a bath or over the course of a visit to the spa, right? Well, this is not really true due to the fact that they can be used to even help with frozen shoulder! Indeed, towel stretches done by holding the towel with each hand across one’s back. The arm which is normal should then pull the affected arm so it is stretched. As is the case with the finger walk, about 20 repetitions a day is ideal for good results.

If one goes a step forward and follows a regime of with all these exercises, the person suffering due to frozen shoulder will be back to normal in almost no time at all!

I have a problem in back bone and the pain increase day by day so give me suggestions.

BPTh/BPT
Physiotherapist, Bangalore
I have a problem in back bone and the pain increase day by day so give me suggestions.
1) avoid those activities which increases your pain. Eg: prolong sitting, forward bending etc. 2) one should find out the cause of your pain eg: if the pain has started suddenly, it could be a muscle pull while playing or bending or long stretch sitting 3) if the pain is since long and is increasing gradually then you should go for a radiological report to diagnose the problem. Once the cause is known, treatment can be done accordingly. Mean while, focus on the following points: 1) place a cushion and support your back while sitting. 2) check on your footwears. Bad footwear leads to back pain 3) place a pillow under your knees while sleeping straight and between your both knees while sleeping side lying (this technique helps to relax your muscles) 4) use a hot water bag/ heat belt to the painful area, heat will improve the blood flow to the muscles and hence will allow them to relax. 5) start certain stretching and strengthening exercises attest twice a day. Regards!

Hi doctors, my father age-76 have suffering pain on knee and doctor suggest for knee replacement for gaping. Can you suggest other option to relief him. He is taken pill for pain and many type of balm.

Mch Orthopaedic, DNB (Orthopedics), Diploma In Orthopaedics (D. Ortho), MBBS
Orthopedist, Ghaziabad
Hi, I would like to answer your query. Your father pain is due to arthritis. For osteoarthritis, weight reduction is the main treatment along with medicines to decrease pain like nsaid's - paracetamol, aceclofenac etc, life style modifications like avoiding sitting cross legged, squatting, using indian type toilets will decrease the rate of progression of osteoarthritis. Exercises like quadriceps strengthening, swimming, straight leg raising will help in improving the muscle strength around the knee and will help in decreasing pain. Hyaluronic acid injections given into the joint (especially large joints like knee joint) in prescribed dosages will give good short term pain relief in arthritic joints ranging from 1-2 years. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

* walking regularly at slow pace without unduly straining helps as it improves blood circulation around knee and eases the joint. *you can use mustard oil for massage which is good in winter season for relieving pain. Eucalyptus oil is also good homoeopathy has a good treatment for this without adverse effects. Take bryonia 200---twice daily morning and evening and give feedback after 7 days
