The likelihood of tetanus is greatest following deep, dirty puncture wounds where there is little bleeding and an absence of oxygen. In this case no need of tet vac in my opinion.

If you took the tetanus vaccine, no need to take it again but if you took only preformed antibodies (anti tetanus serum), you should take a shot now.

Hey don't worry too much. Tetanus bacteria is present in soil/animal faeces etc. Clean surfaces/ hands don't harbor the bacteria. Moreover, it only spreads through cuts/ wounds. It can't penetrate intact skin. So you won't get tetanus that easily. Try to prevent yourself from washing hands repeatedly even if you feel they are dirty. You will gradually be able to curb the habit.

important method of preventing tetanus is cleaning out the wound as thoroughly as possible. The wound can be washed with clean water, and soap can be used to clean the area around the wound.

Using too much kenalog-40 is not likely to cause serious problems. However, long term use of high doses can lead to thinning skin, easy bruising, changes in body fat (especially in your face, neck, back, and waist), increased acne or facial hair, menstrual problems, impotence, or loss of interest in sex

No need to take it, it is not a road traffic accident or it has not occurred outside then there is no need of tetanus.
