Can be gerd. Get an upper gi endoscopy done which r/o stricture and also gerd. Pain on exersion then angina is a possibility. Go one by one to make proper diagnosis.

Since you already have chest pain with a positive TMT and are taking medications then you must have already got evaluated by a cardiologist, who also must have suggested that you these medicine lifestyle precautions and that should get a angiography done to find out the level of blockage in your heart arteries. Apply Hot Fomentation twice daily.
Avoid bending in front.
Postural Correction- Sit Tall, Walk Tall.
Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Smoking itself is a risk factor for hypertension.
Your overall blood pressure is on the higher side. Your medications need to be increased and adjusted.

It would be better to take him after getting him checked by a doctor. Keep monitoring his blood pressure and breathing. Don't give him sorbitrate carelessly, it can lower the bp.
