Fissure in Ano, also known as Anal Fissure, is basically a small longitudinal cut in skin that lines the opening of the anus. Fissures cause bleeding and severe pain in the bowel movements. These are very common in the general public, but often confused with other pain. A fissure can happen at any point in your lifetime and can have equal general distribution.

Types of Fissure in Ano:

Acute Fissure: Fissure in Ano less than 6 weeks of duration is known as Acute Fissure.

● Chronic Fissure: Anal Fissure with more than 6 weeks of duration is known as Chronic Fissure.

Causes of Fissures in Ano:

● Anal fissures are basically caused by stretching of the anal muscles beyond its capability.

● In adults, it is caused by the passing of large stools, constipation or by prolonged diarrhea. ● In older adults it is caused by the less flow of blood to that area.

● It is also caused by sexually transmitted infections (syphilis, chlamydia, herpes) and it results in fissures.

● Anal sex can also be the cause of fissures.

Symptoms of Fissures in Ano:

● Tear in the skin around the anus

● Small lump of skin, or skin tag next to the tear

● Pain during bowel movement

● Stools with blood or pus

Itching and burning in the anal area

Preventions During Fissures in Ano:

● One should drink enough water.

● One should have high fibre diet so that one does not suffer from constipation.

● Be careful and hygienic after anal defecation.

● In case of suspected fissure or any kind of pain, use a lubricating ointment.

● Sit in lukewarm water

Treatment for Fissures in Ano

Surgical treatments are not recommended, but there are some other measures that can be taken and are listed as follows:

● Some prescribed medication to lower the severity of pain.

Nitroglycerin ointment may help heal the fissure.

● Having a rich fiber diet as a lifestyle change.

● Drink enough water as that helps in softening the stool.

● Sometimes Stool softeners may be required to soften the bowel movement.

When Anal Fissure does not heal with all of the above listed things, then a surgery may be required:

● Lower most part of the internal anal sphincter is divided laterally. With this acute fissure heals of its own. In chronic cases one may require to undergo fissurectomy.

● The surgery decreases the anal pain and spasm, allowing the fissure to heal.

● If a sentinel pile is present, it is removed to heal the fissure.

It is important to note that any kind of treatment requires 6-10 weeks to recover. However, the pain may disappear just after 2-4 days after the surgery. One must consult a doctor before going for any treatment. This surgery can also be performed with LASER,where there is less trauma and early recovery is posssible.

Anal fistula
An anal fistula is a chronic inflamed, abnormal communication track between the anal canal and the outer skin of the anus. Commonly this disease develops after spontaneous bursting or operation of an abscess located in this area. It then remains open and discharges pus, even after the abscess has healed.

Cause- anal gland infection is the commonest cause of Fistula In-Ano. Other reasons are tuberculosis, trauma or after operation in and around the anal canal.

Symptoms of Fistula In-Ano-

  • discharge of pus/blood from the perianal region.
  • Pain around the anus.
  • Fever.
  • Backache.

Treatment- medicine.
By surgery - fistulectomy, lift technique, craft technique, seton
Ksharsutra is a medicated thread (chemical seton) prepared by using plant-based ingredients. The active ingredients are coated repeatedly on the medical thread. This ksharsutra control infection by microbicidal action.
Due to caustic action, it destroys and removes unhealthy tissue and promotes healing of the fistulous track.

What is Fistula in Ano?

A fistula-in-ano is an abnormal tract with an external opening in the perianal area that is communicating with the rectum or anal canal by an identifiable internal opening. Most fistulas are thought to arise as a result of cryptoglandular infection with resultantperirectal abscess.

Treatment of anal fistulas depends on their location and complexity. When considering surgery, the ultimate goal is to protect the sphincter muscle and prevent recurrence of an anal fistula

LIFT (ligation of internal fistula tract) technique is yet another approach in treating a fistula-in-ano.

The Technique

LIFT procedure is based on secure closure of the internal opening and removal of infected cryptoglandular tissue through the intersphincteric approach. The LIFT technique is often performed for complex or deep fistulas. The procedure allows surgeon to access the fistula between the sphincter muscles and avoid cutting them.

I want to discuss on anal fistula medication. If there is a patient of fistula asks you to recommend some medication please tell him clearly that there is no any medication needed other than Kshar Sutra ligation in fistulous tract. No need of any medicine after ligation before or after because if you give any medicine like antibiotics or even homoeopathic medicine if not consulted properly it will bock the track of fistula and it will cause worsening of fistula because it will make more tracs in fistula.

Fistula in ano or bhagandar is very much recurrent disease means, d first thing is when a person have anal fistula then it itself some times look like it is completely gone by time, but suddenly after few days, weeks, months or years it appears with all it's features. And a person with few medications like antibiotics or homoeopathic medications, if case is not taken properly, it again behaves the same. It's very recurrent even after surgeries. Now what I have learned in my practice and in my institutions, the kshar sutra method of ligation of fistula, completely cures the fistula in ano. And also it doesn't harm the rings of anus, so that it doesn't cause any incontinence.

Our anal system is an important part of our whole body. Any kind of problem in and around it can affect the overall health of our body. A fissure in the ano is a common problem which affects many people but there are some important information you should know about it.

What is Fissure In Ano?

An anal fissure or fissure in ano is a small physical tear or scut around the lining of the anus. This tear in the skin around this area causes significant pain and bleeding which becomes unbearable in most cases. At times the tear can be deep enough to expose the muscle below. Due to this physical tear and muscle exposure, the pain is severe which can be treated none the less.

Even though a fissure in the ano is not a very serious situation and can be treated easily but it can be responsible for some sleepless nights because of severe pain. This can be seen in individuals of almost all ages especially in children or infants. ● These can be seen in women when they are have given a child birth.

● In many cases people suffering from HIV or Syphilis can also develop these fissures.

क्षार-सूत्र चिकित्सा क्या है? तथा विभिन्न प्रकार के गुदागत रोगो की चिकित्सा में क्षार सूत्र चिकित्सा क्यों उपयोगी सिद्ध हो रही हैं? 

  • क्षार-सूत्र विभिन्न प्रकार की आयुर्वेद औषधियो से निर्मित medicated thread होता हैं यह एक प्राचीन आयुर्वेद चिकित्सा प्रक्रिया हैं जिसे स्व. Dr. P. J. Deshpande (1964 b. H. U.) नें एक नवीन रूप में विभिन्न प्रकार की शोध प्रक्रियाओं के द्वारा विकसित किया तथा भगन्दर आदि गुदागत रोगो की चिकित्सा में इसकी उपयोगिता को सिद्ध किया
  • क्षार-सूत्र चिकित्सा विधि एक लघु आपरेशन (minor surgery) हैं जिसमें रोगी को बाद में कुछ घंटो के लिये भर्ती रखा जाता है तथा दूसरे दिन से अपने कार्यो को सुचारु रूप से शुरू कर सकता है यदि किसी रोगी को बेहोशी या स्पाइनल एनेस्थेसिया (under anesthetic) दिया जाता है तो 24 घंटे भर्ती रखना आवश्यक होता है
  • क्षार-सूत्र चिकित्सा विधि एक ऐसी प्रक्रिया है जिसमें इस विधि से सही हुए रोगो के दोबारा (recurrence) होने की संभावना बहुत कम हैं
  • इस दौरान लगभग 8000 गुदागत opd रोगियो का परीक्षण-जांच-निदान-चिकित्सा की तथा लगभग 300 रोगियो का क्षार-सूत्र चिकित्सा विधि द्वारा आपरेशन किया है
  • इस दौरान मैंने गुरू जी के सानिध्य में जाना कि इस विधि का परिणाम 98.5 प्रतिशत सफल हैं
  • इस दौरान मैंने 200 multiple opening fistula in ano रोगियो पर अध्ययन किया तथा इनमें से 21 रोगियो पर thesis तैयार किया जो पहले 2-3 बार सर्जरी करवा चुके थें लेकिन फिर भी वापस fistula in ano. Recurrence हो गया था और उनमें अच्छे परिणाम प्राप्त किये और उस तकनीक का नाम - bypass kshar-sutra aplication in high anal/anorectal fistula in ano. रखा

अन्य विधियो की तुलना में क्षार सूत्र चिकित्सा विधि द्वारा ईलाज के फायदे:-
1. आयुर्वेद क्षार-सूत्र चिकित्सा अन्य ईलाज व आपरेशन की तुलना में एक सरल प्रक्रिया है, जिसमें बिना चीरा -फाडी के औषधि युक्त धागे से धीरे धीरे कटिंग होती हैं तथा साथ ही साथ जख्म भी भरता जाता हैं
2. इस विधि से सही हुए रोगो के दोबारा होने की संभावना बहुत कम है
3. High type fistula, multiple fistula in ano में सर्जरी में गुदा की वल्लियां कटने का खतरा रहता है जिससे रोगी का लैट्रिन पर कंट्रोल समाप्त होने का खतरा रहता हैं जबकि क्षार-सूत्र चिकित्सा विधि में फिस्टूला tract धीरे-धीरे एक नियन्त्रित गति से कटता हैं
4. बनारस हिन्दू विश्वविद्यालय, aiims, icmr, Dr. P. J. Deshpande तथा मेरे गुरूवर के द्वारा किये गये शोध कार्यो तथा क्लीनिकल ट्रायल्स से यह सिद्ध हो चुका है कि क्षार-सूत्र चिकित्सा विधि गुदागत रोगो के ईलाज में सर्जरी से अधिक प्रभावी और स्थायी ईलाज हैं!
