Looking at the details I suggest you should shift to long acting basal analogue insulin like injection basalog, start with 6 units once a day and increase the dose based on daily blood sugar levels. You will have to do self monitoring of sugar levels. One of the oral medications namely metformin with glimepiride to be continued.

I have see n the details given. However, only the drugs and their dosage is mentioned. But there is no information on fasting, PP glucose levels, HbA1c%, lipid profile, weight, height, extent of daily exercise and dietary details. Plus it is also necessary to know serum creatinine and urea as you are 61 years old. Addition of metformin will restrict your insulin dose.You can stop glimipride in glycomet Gp 3/850 instead you can use glycomet 1000mg twice a day.You can stop jalra also because it is effective in early years of disease.
