Hello friends,

I am Dr. Vandana Jain, practicing gynecologist at Ghaziabad Vasundhara. I am here again to talk few important tips regarding women health. Most important complain in our day to day practice is doctor why I am so much tired daily? Why I am having fatigue? Why I got lethargy all the time and even at rest? I ask a simple question to my patient first. Are you taking proper sleep? At least 7 to 9 hours sleep is required. Are you taking proper diet? Green vegetables, fruits daily, milk daily, protein daily? And are you doing physical activity? That is 20 minutes a day, 5 times a week. If you are not taking proper sleep, you are not able to eat, you are not able to exercise. And the other way round also holds true. So most important ask yourself, ask are you taking care of your body right or not?

If the answer to all these three questions are right, you are taking proper sleep, you are taking good diet, you are doing exercise, but still you are drowsy even at daytime and if you are still at first gear in the middle of the day, then you need doctor advice, doctor personal consultation. Because the doctor will give you plenty of tests to do lots of important diseases which can lead to fatigue, lethargy as a symptom. Most important disease or you can say the problem in females is Anemia and it is mostly ignored because of menstruation, because of delivery, multiple pregnancies. Ladies are always anemic. The doctor will advise you in anemia profile, will give you a test for haemoglobin, and if it is less than 11.5, then you are anemic you need iron supplementation.

Anemia is not only because of iron deficiency, it can be because of Vitamin B12 deficiency also. And this deficiency is most common in vegetarian people. So anemia profile is needed. Second is Diabetes. It is a symptomatic. You will feel only hardly in the daytime. You won’t take food even after taking food. Then you should check your blood sugar level. Third is sleep apnea. Although there it occurs in old age females, in obese females. Brain has a signal that you are not getting rid of carbon dioxide from your body in the night. Then you will wake up in the night in brief episodes and because of this you will get decreased oxygen in your body and that can lead to drowsiness and lethargy in the daytime. Sleep hack is the thing to correct this problem. Another problem is the most common is Vitamin D3 deficiency and calcium deficiency. Very common in females and very common in working ladies. They are not taking in sunlight, their bones are weak, their vitamin D is totally down to a level, to an extent that can lead to backache, lethargy, menstrual problems and so many things.

For this, your doctor will advise you Vitamin D3 level and Calcium level. Next important thing is Thyroid, Thyroid is butterfly-shaped gland at your neck and it secretes thyroid hormone. Sometimes it is hyperthyroid and sometimes hypothyroid. Most common is hypothyroid. In hypothyroid your thyroid gland will secrete less thyroid hormone for taking high protein, exogenous thyroid hormones in the form of tablets. In hypothyroid, there is lethargy, there is increased body weight, leg swelling and many problems. Another problem patient came in old age because of menopause. Menopause is the normal phase of life but still ladies ignore it. They think that it is the part of life to feel lethargic at this stage. There are so many medicines available hormonal treatment, non-hormonal treatment to decrease the side effects of menopause like night sweating, hot flashes, bone pain, backache and skin dryness, so many tablets are available to cure this. Most important thing is to feel that your body is not so old, you can live a normal life as you were earlier. Another important thing is depression.

It is very much ignored in Indian females. Most of the ladies are patients of mild depression and they think it is normal to feel sometimes lonely, having teary eyes, always unhappy, they are lethargic at times. Because there is a hormone secreted by the brain called serotonin, if it is decreased then the brain gets rob of the serotonin, then this depressive problem increases with time and even it can become moderate and even suicidal tendencies can appear. So if you are feeling not happy all day and very lethargic then also consult a psychiatrist. Another very fatal thing is congestive heart failure. It is the result of many problems and in this case the hospital admission is needed and multidisciplinary approach is needed to correct it and it’s most common symptom is breathlessness even at rest and fatigue by doing small works. So friends all these tiredness symptoms are very common and very easily and very easily neglected by us only. So it’s our duty to rectify them, correct them and to visit the doctor.

Today I am speaking about some tips about the gynaecological health of a lady. Many Indian women are ashamed of their routine gynecological checkup and even discussing about their gynecological, reproductive and sexual health. But, they should know and respect about their bodies because it is a part of their health.

Few tips to have a healthy life are as follows - Firstly, get a gynecological checkup by Pap Smear, have a breast self-examination also because this can reduce the chances of cancer infact early detection of cancer can be treated. Secondly, they should eat a balanced diet to maintain a proper weight because too thin and too fat both can lead to irregular menstrual cycles, can delay their periods can have heavy periods. For maintaining balanced weight it is very important to drink lots of fluids and drinks, have good diet- balanced healthy diet fibre rich diet. Third point is to avoid obesity. Central obesity is the most common cause of Polycystic Ovarian syndrome.

Central obesity is a very risky condition for a lady because it can increase chances of Polycystic ovarian diseases and metabolic syndrome. Metabolic syndrome can lead to diabetes, high blood pressure, lipid changes in a body later in the life and that can also increase the chances of cardiovascular problems. So daily physical activities, lifestyle modification by diet and exercise even yoga is very good. Fourth point is have a planning of the pregnancy before 30 years of age because as the age increases near about 35 of age, high chances of miscarriage are there. There are very high chances of delayed conception, infertility etc. So early pregnancy is very important.

When you are young, have a pre-conceptional check up because it can be beneficial to you when you are having a baby and maintaining a balanced diet before pregnancy. Fifth is to have Kegel exercises daily. Kegel's exercises basically maintain the tone of your pelvic floor because of pelvic floor is not toned there can be increased chances of incontinence, urinary bladder problem and uterovaginal prolapse in later life. As uterovaginal prolapse is not treated by medicine only surgery is required, it is also a very necessary thing.

The last point is practice safe sex because if you don't do this, there can be a lot of sexually transmitted diseases. As 20 common STDs can occur and can lead to infertility, ectopic pregnancy, period problem later in the life. Like HIV (Human Immunodeficiency Virus ) can lead to AIDS and it can increases the chances that your body is not able to fight against infection as the immunity of the body is decreased. Second is HPV (Human Papillomavirus). It is also very strong virus that can increase chances of Warts which can increase chances of cervical cancer in later life.

Gonorrhea, chlamydia all infections are sexually transmitted and if you are facing white discharge then a routine checkup by doctor is very much necessary. I wish, a lady should respect their gynaecological and sexual health, to have a healthy life. Thank you! The last point is take care of your vulva and vagina, avoid frequent washings with rough soap and solutions, avoid wet/tight garments, wear breathable garments because it can increase chances of infections. Hello friends, I am Dr Vandana Jain I am practising gynaecologist at Ghaziabad. Today I am going to speak on cervical cancer and its prevention by vaccination. Cervical cancer is the cancer of cervix and cervix is the opening of the uterus and it prevents infection entering into the uterus. Cervical cancer is the most common cancer in India and in world by it accounts for 5 Lac newer cases diagnosed per year and that accounts for 2.5 Lac deaths per year worldwide, in India it accounts for 1,32,000 cases per year and 74,000 deaths per year. This is the most common cancer even the more common than breast cancer in India need accounts for a most common cause of death in females in India. What causes cervical cancer, cervical cancer is caused by human papillomavirus and it is a virus which is usually sexually transmitted but sometimes it can infect a body without any sexual contact. There are about 100 type of strains of HPV out of those hundred strains there are 30 strains which are sexually transmitted and out of those 30 there are 15 strains of HPV which are oncogenic, oncogenic means cancer-causing virus, this virus remains in a body for a long, for once to years and it takes about 10 to 20 years to change a normal cervix into a cancer cervix and in a lifetime of a lady about 50 to 80% females are infected by these viruses. Mostly these viruses going into spontaneous remission they will cure by itself but in some cases, it progresses into cancer. So when there is a vaccine we can prevent cancer from occurring and causing death. There are two types of cancer cervix adenocarcinoma and squamous carcinoma, adenocarcinoma is more lethal, more difficult to diagnose and it is mostly in young females. Pap smear is the type of screening test which can detect early changes of cancer in cervix or invasive carcinoma but remember Pap smear is a screening test only it can't detect infection by HPV and also this vaccination is to prevent infection by HPV and the age group for this vaccination is from 9 years to 45 years. Surrogacy is a practice where a woman bears and give birth to a child for the couple with the intention of handing over the child to the intended infertile couple after the birth.

What is Altruistic surrogacy?

Altruistic surrogacy is one in which no charge, expenses, fees, any incentive has taken by the doctor or the surrogate mother. Now in the newer bill, that is the surrogacy bill, which is finally implemented on 24th.aug, 2016, the government intenf=ds to ban surrogacy for any commercial purposes. And hence commercialization of surrogacy has been banned and only Altruistic surrogacy has been allowed.

Now, what other conditions are there so that surrogacy can be implanted? So when surrogacy will be allowed?

Surrogacy is allowed only when the couple is infertile for a long period of time. The age of female should be 25-50 years of age and the age of male should be 25-55 years of age. And if they both or either of them is suffering from infertility. And it is only for altruistic surrogacy and not for commercial surrogacy. No person other than a closed relative can be a surrogate. No woman should act a surrogate more than once in her life.

There are the cases in which after birth, the couple has an abundant born child because of his disability. So now there will not be any amendment of the born child because of surrogacy. The couple do not have any surviving child either biological or by adoption. It should be their first child by surrogacy.

Surrogacy is not for children producing for sale. It should not be a baby factory which can be used. In past, there was illegal trafficking also. Only the clinics which are registered under the act can conduct activities and procedure related to surrogacy. So surrogacy clinic must maintain their records atleast for 25 years.

Violation of any other provision shall be punishable with the imprisonment of 5 years and the fine of up to 10 lakhs. An amendment of the surrogate child is there, then the imprisonment will be for 10 years and a fine of up to 10 lakhs.

A National surrogacy board or a state surrogacy board will take care and keep the watch on the clinics which are registered under this act.

It is a real challenge to squeeze in the right diet, exercise and care both to the mother and the child and focus should be on regular antenatal visits to the doctor. A few things which pregnant women should focus on are;

  • Eat well
  • Exercise well.

  • Do not smoke
  • Do not lift heavy objects
