Using condom can prevent pregnancy. But you need to check for the integrity of condom after its use. If it is torn then you should use i-pill asap to prevent pregnancy. If both are used together, then there is no chance of pregnancy.

Self Monitoring of Blood Glucose - SMBG Chart

Self-monitoring of blood glucose (smbg) is very crucial for those diabetics who are on insulin or on certain oral anti-diabetic drugs which can cause hypoglycemia. To achieve best glycemic control it is essential to know how your daily blood glucose levels are fluctuating in an entire day.

It is easier for a diabetes specialist to adjust drugs or to adjust insulin dose if good smbg record is maintained by the patient.

Normally we check fasting and 2 hr post prandial blood sugar levels in the lab for routine follow-up. There is a bright chance of sugar excursions in the other post meal readings which get unnoticed. Even in case of hba1c or glycated hemoglobin test we get an average result of blood glucose of last 3 months.

In practice, I have seen several patients who had to fast and post prandial blood glucose lab tests in the normal range but hba1c levels below 7 is not achieved because they might have high blood sugars in other post meals which have got unnoticed.

To achieve the best blood glycemic control it is important to control blood sugar levels within normal limits during the entire day and not just fasting or post breakfast levels.

Interpretation of smbg chart:

In the above image of smbg chart, I have mentioned 7 point charting system where you can fill your blood glucometer readings which you take on several spots in an entire day.

Seven points includes:


    Fasting or before breakfast reading
    2 hrs after breakfast
    Before lunch
    2 hrs after lunch
    Before dinner
    2 hrs after dinner or bedtime
    At mid night 3.00 am


This seven point smbg charting approximately covers entire day glucose fluctuations to some extent (of course not as accurate as cgms).

I have also mentioned a comment column in the above smbg chart where you can note down change in food or change in insulin dose or any other changes that you have made which probably caused unusual readings like for example if hypoglycemia occurs at before lunch reading and you have skipped breakfast or had very little snack at breakfast you can mention that.

You do not need to check daily all the 7 points, you can check any one or 2 points randomly depending upon your sugar control.

More uncontrolled blood sugars require more frequent smbg glucometer monitoring.

Monitoring of blood glucose at home in case of diabetic pregnancy or gestational diabetes is also important where frequent monitoring and smbg charting of glucometer readings helps a lot to the patient.

I hope this article will help you to control your blood sugar levels more efficiently.

If you find that some of the post-meal or pre-meal readings are high or low, please consult your diabetologist with smbg chart to adjust your medication or insulin dosage.

Is it safe to have sex during Pregnancy?
Most parents-to-be worry about this, but if you have a normal pregnancy, you can keep making love.
If you have a healthy pregnancy, there’s no reason why you and your partner can not have sex. You would not hurt your baby by making love.
There are, however, some circumstances where you may have to be careful about having sex.

If you’ve had any bleeding or spotting in early pregnancy, your doctor may advise you not to have sex until you reach 14 weeks. Your doctor may say it is best not to have sex for the rest of your pregnancy if you have:
- a history of cervical weakness
- a low-lying placenta (placenta praevia)
- had heavy bleeding
- had a vaginal infection
It is advisable to consult your gynecologist if you have a doubt ask your gynecologist. Your doctor is the best person to advise in such matters.

To support the healthy growth of their baby, pregnant or breastfeeding women need to consume around 400 additional calories per day in order to meet their own and baby's energy needs.

During pregnancy or while breastfeeding your baby, it is important to eat a variety of healthy foods. The essential nutrients listed below are needed to help you and your baby's health. They're found in fresh fruits and vegetables, whole grains, nuts, beans, dairy products, and lean meats.

Calcium

Calcium helps build strong bones and teeth and plays an important role in the healthy functioning of the circulatory, muscular, and nervous systems. Pregnant and breastfeeding women should get 1,000 mg of calcium a day. Healthy sources of calcium include low-fat dairy products, spinach, calcium-fortified orange juice and cereals.

Carbohydrates

Eating carbohydrates helps provide energy to support the growth and development of a baby both during pregnancy and breastfeeding. The best sources of carbohydrates are whole grains, fruits, and vegetables, which also are good sources of fiber.

Fiber

Fiber is a nutrient that can help patients with constipation commonly associated with pregnancy. Whole grains (like whole-wheat bread, whole-grain cereals, and brown rice) and fruits, vegetables, and legumes (beans, split peas, and lentils) are rich sources of fiber.

Folic acid

Folic acid helps the healthy development of a baby's brain and spinal cord. It's also needed to make red blood cells and white blood cells. Women who get 400 micrograms (0.4 milligrams) of folic acid daily prior to conception and during early pregnancy can reduce the risk that their baby will be born with a neural tube defect. Good sources of folic acid include fortified cereals, leafy green vegetables, beans, nuts and citrus fruits,

Healthy Fats

Healthy fats (unsaturated fats) are used to fuel a baby's growth and development. They are especially important for the development of the brain and nervous system. Healthy fats are found in olive oil, peanut oil, canola oil, avocados, and salmon. While fat is necessary for any healthy diet, it's important to limit fat intake to 30% or less of your daily calorie intake.

Iodine

Iodine helps the body's thyroid gland make hormones that help with growth and brain development. Not getting enough iodine during pregnancy can put a baby at risk for thyroid problems, cognitive delays, and even mental retardation. It is recommended that pregnant or lactating women use iodized salt in their cooking and eat foods high in iodine, like seafood and dairy products.

Iron

Eating a diet rich in iron and taking a daily iron supplement while pregnant or breastfeeding helps prevent iron-deficiency anemia. Women who don't get enough iron may feel exhausted and are highly prone to infections. Good dietary sources of iron include lean meats, fortified cereals, leafy green vegetables and legumes like beans, split peas, and lentils.

Protein

Protein helps build a baby's muscles, bones, and other tissues, especially in the second and third trimesters of pregnancy. The recommended protein intake during the second half of pregnancy and while breastfeeding is 71 grams daily. Healthy sources of protein include lean meat, peanut butter, eggs, poultry, fish, beans,and tofu.

Vitamin A

Vitamin A helps develop a baby's heart, eyes, and immune system. Prenatal vitamins should not contain more than 1,500 micrograms (5,000 IU) of vitamin A and pregnant women should not take vitamin A supplements. Both too little and too much vitamin A can harm a developing fetus. Good sources of vitamin A include milk, dark leafy greens, orange fruits, and vegetables like carrots and sweet potatoes.

Vitamin B6

Vitamin B6 helps form a baby's red blood cells; breaking down protein, fat, and carbohydrates; and is needed for normal brain development and function. Rich sources of vitamin B6 include poultry, fortified cereals, fish, whole grains, and bananas.

Vitamin B12

Vitamin B12 plays an important role in the formation of a baby's red blood cells, as well as brain development and function. Vitamin B12 is only found in animal products like meat and eggs, so it's important to speak to the Gynae about taking a B12 supplement during your pregnancy and while breastfeeding as well if you're a vegetarian. Rich sources of vitamin B12 include lean meats, poultry, and fish, and low-fat milk.

Vitamin C

Vitamin C plays an important role in tissue growth and repair, and in bone and tooth development. Vitamin C also helps the body absorb iron. Good sources of vitamin C include citrus fruits, fortified fruit juices, broccoli, and tomatoes.

Vitamin D

Vitamin D is helpful in the body's absorption of calcium for building healthy bones and teeth. Good sources of vitamin D include fortified low-fat or fat-free milk, egg yolks, fortified orange juice and salmon.

For an expecting mother, nothing is more important than the safety and well-being of her unborn child. Read on to find out the 5 things pregnant women must avoid at all costs in order to have a safe pregnancy.

  1. Abdominal x-rays: Abdominal x-rays during pregnancy are a big no. This kind of x-ray exposes the abdomen of the mother, and the baby as a consequence, to high levels of radiation, which can cause changes in the baby's rapidly growing cells. As a result, the baby's chances of suffering from birth defects or certain cancers, such as leukaemia later on in life, increase rapidly. It is best to avoid x-rays until absolutely necessary. X-rays of area other than abdomen if considered essential by your doctor can be performed after wearing an abdominal lead shield.
  2. Alcohol: All types of alcohol must be avoided during pregnancy. Consumption of alcohol during these 9 months has been associated with several birth defects such as poor growth, learning disabilities and mental retardation. In worst case scenarios, excessive alcohol consumption during pregnancy can lead to what is known as foetal alcohol syndrome or fas, which severely affects the mental development and physical growth of the unborn child, particularly of the face and skull. Also, you must quit smoking.
  3. Illicit or recreational drugs: Drugs like marijuana, cocaine and other recreational drugs are again absolutely forbidden in pregnancy. These result in birth defects in the unborn child, poor growth and even withdrawal symptoms in the newborn after birth.
  4. Medicines: As the metabolism of a growing baby is very different from that of an adult, it is very important to be sure of the medicines that you take while pregnant. This is especially true in the first trimester or the first 3 months of pregnancy when the foetus is most vulnerable. Certain medicines are deemed safe for the baby, while some are potentially harmful. Hence one must make sure to consult your doctor before taking any medicine when pregnant.
  5. Stress: Stress can trigger various health problems, which can be bad for both the baby and the mother. Studies have revealed the emotional environment of the mother is experienced by the baby as well. In fact, a particular study has shown that any kind of stress, whether it be work related or stress caused by strain in relationships, can affect the mental development of the child. I am a 21 year old female. I had unprotected sex on November 23rd. It was my first time. My periods date for December was 12th. I have very irregular menstrual cycle. I noticed some brownish and pinkish discharge on the 9th 10th and 11th of December. I got my periods on January 6th. I was very worried that I might be pregnant and I took many pregnancy test kit each time it showed negative. I had sex again on January 26th. My boyfriend used condom and we were not able to have sex properly that day. He later on went to the washroom and removed his ejaculate. I got my periods on the 15th of February. The blood flow was less and I usually have stomach pain during periods this time I didn't feel it and I also noticed some small clots during periods in January and February. I had normal bleeding in January for about 5 days. This month it's the third day and it's like my periods has almost stopped. I took pregnancy test on 13th February in the evening around 5 it showed a clear negative. I was going for aerobics I left from December and my sleeping timings have changed and even my eating timings have changed I skip breakfast almost everyday. I feel like I have put on weight especially near the stomach and arms. I have no symptoms of pregnancy but is it possible that I am pregnant? My boyfriend tells he is sure he didn't ejaculate inside me.

A urine pregnancy test has a sensitivity of more than 95%. So if it's negative then there is almost no chance that you are pregnant. But to be very sure. You could go for blood investigations and consult a gynecologist.

Most probably there is nothing to worry. If you still have doubt, pls get urine pregnancy test done.

Last time I took i-pill then my period was came early and now its late what should I do sir please suggest me.
Delay in periods after consuming I-pill is quite common. Wait for a week and if you do not get your periods then take pregnancy test.

My wife is 39 years old and we have a child through IVF, three years back. But had some complications like High blood pressure and we were forced to have premature delivery through cesarean in he 8th month of pregnancy. My wife is insisting on second child. Please suggest.

Please note that if there was high blood pressure in first pregnancy then risk of getting it again in second pregnancy increases. Your wife can get pregnant again, however she needs to be monitored well for her blood pressure through out the pregnancy & there is a possibility that she may deliver prematurely again.

Dengue - Common Symptoms To Watch Out For!!

With the change of season, we all have to be geared up for the increase in the number of mosquitoes in and around our homes, and among these is one such mosquito, which can be fatal for your life. The bite of the ‘Aedes aegypti’ mosquito causes Dengue. This mosquito survives and breeds in tropical and sub-tropical areas. It is a waterborne disease and causes extreme pain, discomfort and in some cases even death. Each year 390 million people are inflicted with dengue out of which 96 million develop serious illnesses and are required to be hospitalized.

Causes:
1. As mentioned above it is caused by the bite of the ‘Aedes aegypti’ mosquito.
2. When a mosquito bites a person who already has dengue, it becomes infected and it can be transmitted to any healthy person this mosquito bites next.
3. If you already have dengue and are infected again then you have a higher chance of developing dengue hemorrhagic fever or a dengue shock which can even cause death.
4. Living near a stagnant water body, or accumulating water in open containers, staying in the vicinity of clogged drains and open windows can also expose you to the threat of Dengue.

Symptoms:
The symptoms can last to about 10 days after one gets infected. The symptoms are as follows:
1. Excruciating pain behind the eyes
2. Severe headache
3. Sudden high fever.
4. Vomiting
5. Fatigue
6. Skin rash which generally appears two to five days after you get bitten.
7. Nose bleeding, easy bruising and bleeding gums.
8. Severe muscle and joint pain.

These symptoms can be mistaken for high fever or viral infection, so it is important to get tested as soon as you start experiencing these symptoms. If dengue is not treated in time, it can give rise to dengue hemorrhagic fever. In this case, you can suffer from high fever, nose bleeding (damage of blood vessels), failure of the circulatory system and enlargement of liver.

You can also prevent dengue by avoiding storing water in containers, keeping your surroundings clean, wearing full body clothing, using mosquito repellants, sleeping under a mosquito net and setting mosquito traps. Dengue is very devastating and can cause death in worse cases as in dengue shock or dengue hemorrhagic fever. Dengue is now an epidemic in many countries. People affected with dengue need to drink a lot of fluids and take plenty of rest. They can also reduce the fever by having a paracetamol. If you wish to discuss about any specific problem, you can consult a General Physician.
