Uterine Fibroids - How They Can Be Managed?

The uterine fibroids are one of the most common gynecologic problems, with over 40-50% of women having them at some point in their life. The number and size of the fibroids, the age of onset, associated symptoms like dysfunctional bleeding and pelvic pain will determine the management of fibroid. The following is a series of management measures, starting from the most conservative to the most invasive.

1. Wait and watch: In women who are asymptomatic, it is best to watch them for the development of symptoms without any intervention. Also, in women nearing menopause, it is best to just watch the fibroids as they just shrink once menstruation ceases.

2. Medical therapy: In some women, menstrual cycles could be heavy or irregular and require hormonal replacement. They could also have occasional pain, and so may require painkillers.

  1. Tranexamic acid is a non-hormonal medication that can be used on heavy bleeding days only to ease the heavy menstrual flow.
  2. Nonsteroidal anti-inflammatory drugs (NSAIDs) are useful in pain control. Iron and vitamin supplements are also useful in managing anemia and weakness associated with heavy bleeding.
  3. Oral contraceptives and/or progestins are also used to control abnormal menstrual bleeding. They do not have any effect on fibroid size though.
  4. Progestin could also be delivered through an intrauterine device (Mirena) to relieve heavy bleeding and prevent pregnancy
  5. Gonadotropin-releasing hormone (Gn-RH) agonists: They block the production of oestrogen and progesterone, and so induce a temporary postmenopausal state. This can also be used to shrink the fibroid so it is easier to remove it surgically. This is not used for more than 3 to 6 months as they can cause hot flashes and bone loss.
  6. Newer drugs like Mifepristone and Ulipristal acetate are being used to shrink the size of fibroids.

3. Noninvasive procedure: MRI-guided focused ultrasound surgery is when the women are inside the MRI scanner and an ultrasonic transducer is used for treatment. The exact location of the fibroids are identified and sound waves are used to destroy the fibroid. Done on an outpatient basis, it is safe and effective.

4. Minimally invasive procedures: Uterine artery embolization is where the arterial supply is cut off to reduce fibroid growth. Myolysis is where laparoscopically heat or cold waves are used to lyse off the fibroids. Endometrial ablation is where the uterine lining is destroyed through high-intensity heat or cold waves.

5. Invasive procedures: Abdominal or Laparoscopic myomectomy is where the fibroids are removed through an abdominal incision or keyhole surgery. If fibroids are extensive and completed family, hysterectomy is the choice of treatment. Given their high incidence, fibroids require management based on symptoms, age, and other considerations as discussed and decided between the patient and the doctor.

Methods of Abortion for Unwanted Pregnancy

MD - Obstetrtics & Gynaecology
Gynaecologist, Mumbai
Methods of Abortion for Unwanted Pregnancy

There are two methods by which abortion can be done:

  • Medical Abortion - Non-surgical abortion in which medicines are used to induce abortion
  • Surgical Abortion - Use of transcervical procedures for terminating a pregnancy, including vacuum aspiration, and dilatation and evacuation (D and E).

Surgical or Medical Abortion for Unwanted Pregnancy

  • If pregnancy is lesser than = 7 weeks; medical method with Mifepristone with Misoprostol is favorable as the success rate is as high as 97%.
  • If pregnancy is between 7-12 weeks; surgical abortion is the only option.
  • If pregnancy is beyond 12 weeks - 20 weeks, then the woman needs to be hospitalized and will be given either Mifepristone followed by Misoprostol tablet or only Misoprostol tablets and woman will then abort like a miscarriage in the hospital. If any products are remaining, then it will be evacuated surgically.

Surgical abortion is preferred if patient desires concurrent tubal ligation or IUCD (CuT) insertion. If a woman fulfills the criteria for selecting either method i.e. before 7 weeks, final choice has to be given to the woman.

Summary of choice between Surgical or Medical abortion in India

  • Lesser than = 7 weeks - Medical method or surgical method
  • 7-12 weeks - Surgical abortion
  • 12 - 20 weeks - Medicines with or without surgical procedure in hospital

Advantages of Medical Abortion

  • Avoid surgery - Controlled by the woman and may take place at home
  • No need for admission to the hospital
  • Usually, well tolerated by women, if properly counselled and motivated
  • No need to take leave from work, as bleeding can be managed with menstrual pads even while in the office or at work.

Disadvantages of medical abortion

  • Takes time (hours to days) to complete abortion, and the timing may not be predictable. Women experience bleeding and cramping, and potentially some other side-effects (nausea, vomiting, fever, and shivering). May require more clinic visits than surgical abortion.

Advantages of Surgical Abortion

  • Quick procedure
  • Complete abortion easily verified by evaluation of aspirated products of conception
  • Takes place in a hospital so Sterilization or placement of an intrauterine device (IUD) may be performed at the same time as the procedure. It can be performed under local anaesthesia if woman is properly motivated and counselled

Disadvantages of surgical abortion

  • Requires instrumentation of the uterus
  • Small risk of uterine or cervical injury
  • Timing of abortion controlled by the doctor and hospital
  • Anaesthesia needs to be administered. Most doctors perform this procedure under short general anaesthesia. A woman needs to be in the hospital for at least 6 hours. Effect of sedation will be there throughout the day.

Vaginal Bleeding Between Periods - 5 Ways To Prevent It!

Vaginal Bleeding Between Periods - 5 Ways To Prevent It!

Many women experience unusual vaginal bleeding between periods at some point in their lives. It is also known as metrorrhagia. Vaginal bleeding is thought to be abnormal in the following cases:
1. When your menstrual period is not expected
2. When your menstrual flow is lighter or heavier than normal
3. At unexpected stages in life. For example, when you are pregnant or after you have attained menopause.

Bleeding during periods is not a normal condition. Usually, the duration of the cycle is about 21 – 35 days. Abnormal vaginal bleeding has numerous conceivable causes. Independently, it doesn't show a genuine condition.

  1. Mid-cycle bleeding can be caused due to ovulation.
  2. Two hormones that regulate the menstrual cycle are progesterone or oestrogen. A condition of hormonal imbalance which is known as Polycystic ovary syndrome (PCOs) causes interference with normal ovulation. This results in problems in thyroid glands as well as causes dysfunctional ovaries.
  3. Medications such as birth control pills are capable of causing abnormal bleeding, especially with the irregular intake of such medications.
  4. Infections or inflammation caused in the pelvic regions such as uterus, vagina, cervix, ovaries or fallopian tubes are capable of causing abnormal bleeding. STIs (Sexually Transmitted Diseases) and PIDs (Pelvic inflammatory disease) are often the cause of such a condition.
  5. Although Cancer is not one of the primary causes, cancer in organs such as the uterus, vagina, cervix and ovaries are capable of causing abnormal bleeding during periods.

How it can prevented

  1. Maintaining a healthy diet. Women who are either underweight or overweight tend to have more problems with abnormal bleeding
  2. Relaxation practices should be followed so that stress is reduced. Stress can cause abnormal bleeding.
  3. Non-steroidal anti-inflammatory drugs (NSAID) should be consumed to reduce menstrual bleeding. Examples are ibuprofen or naproxen.
  4. In case of consumption, birth control pills should be taken as per the prescription only and regularly at the same time.
  5. Hormone therapy (using external hormones in a medical treatment) can be used in order to regulate menstrual cycles, stabilize the endometrium which is the lining of the uterus as well as to rectify hormonal imbalances. 5 Strange Reasons Behind Vaginal Bleeding!

5 Strange Reasons Behind Vaginal Bleeding!

Unexpected or abnormal vaginal bleeding usually refers to the kind of bleeding that occurs any time outside of the normal time of menstruation. Also termed as spotting, inter-menstrual bleeding or metrorrhagia, bleeding between periods always calls for extra measures to be taken against it, and is a matter of serious concern.

The primary causes for unexpected vaginal bleeding generally are:

  1. An imbalance of progesterone and estrogen levels triggered by a variety of causes like thyroid gland problems, dysfunctional ovaries and irregular doses of birth control pills, which may eventually lead to spotting. However, one must take note of the fact that hormonal contraceptives, in many cases, may cause abnormal bleeding for the first few months, after which it always subsides.
  2. Noncancerous growth such as uterine fibroids are also potential causes of spotting or abnormal bleeding.
  3. Miscarriages, ectopic pregnancy, and other kinds of complications involved during childbirth can cause abnormal vaginal bleeding.
  4. Infection of reproductive organs caused due to intercourse, vaginal douching, pelvic inflammatory disease, or STDs (sexually transmitted diseases) may lead to bleeding and inflammation.
  5. Cancer of the vagina, uterus, cervix, or ovaries, albeit extremely rare in nature, may cause abnormal bleeding of the vagina.

While in most cases, this type of bleeding is naturally corrected, some women might need to undergo treatments when the case is severe. Overlooking an otherwise minor issue might thrust you in the face of life-threatening circumstances, if it develops into a case of cancer, infection, or any other type of disorder.

‘Prevention’ of vaginal bleeding, as such, is indeed a narrow possibility and the measures will almost always vary since the factors that cause intermenstrual bleeding aren’t the same in all cases. However, what your doctor will always advise you in this regard will be to ensure that your diet is balanced, your weight under control and your lifestyle is essentially healthy. If you happen to be on birth control medications, be so while adhering to medical instructions. Resorting to taking aspirin doses often might also influence the incidence of such abnormal conditions. If your pain still persists after you experience unexpected bleeding, you must consult your doctor without any further delay.
