Mebeverine

Prescription vs.OTC: Prescription by Doctor required

Mebeverine helps treat symptoms of intestinal disorders and irritable bowel problems, such as spastic colitis, spastic constipation, colon irritation and mucous colitis. The drug belong to the group of medicines called anti-spasmodics. It helps relax intestinal muscles, which in turn reduces pain and irritability.

Mebeverine can only be prescribed to children who are above the age of 10 years. The drug can only be prescribed to children below the age of 10 if absolutely necessary.

Although Mebeverine acts as an over-the counter drug, it is always best that you take it after consulting your medical provider. This is primarily because you may experience certain side effects or even an allergic reaction in some cases, once you start taking Mebeverine. Some common side effects of Mebeverine are problems with breathing and swelling of the hands and face. An allergic reaction to Mebeverine can result in hives, skin rashes and irritation. Seek immediate medical help in case.

You are allowed to drives as well as consume alcohol while taking this medicine.

The drug is meant for oral consumption and should be taken about half an hour before a meal. Regular use of Mebeverine makes it more effective.

Small frequent meals to be chewed slowly, not hastily without talking. Avoid junk, fried,chillies, Chinese food. Raise the head end of the cot by 6 to 8 inches. Atleast 2.5 hrs gap between DINNER and sleep. Walk and exercise DAILY. Avoid caffeinated drinks. Need councelling for stress management. Need full details for prescription. Advise consultation.

I am 20 Male. I have been having a weird feeling in my testicles from last few days. It is sometimes leading to pain. I think it's mostly a muscle or vein related as I get a light sensation when I pull/push my muscle there. I went to an urologist. He told me these are common for my age and examined my testicles. He applied pressure there and asked me if I am having a burning sensation which I had a very slight one. He told me it looks fine and mostly it will go away and gave me a tablet called signoflam and told me to use when ever I get pain. Should I go for further diagnosis or use the tablet? I have been under stress too for last few weeks. Could that have caused the problem?

I am a Gastroenterologist and would be answering your query. As per you query answers are- Firstly for Irritable bowel syndrome. Irritable bowel syndrome is a common intestinal disorder. There is some problem in the functional ability of the gut that leads to the symptoms of irritable bowel syndrome. The structure of the intestines is normal, only functional abnormality occurs in this condition. So frankly speaking (IBS) represents a functional disorder of gastrointestinal tract without the presence of an anatomic defect. Recent studies by French scientists have indicated that the most important mechanisms include visceral sensitivity, abnormal gut motility and autonomous nervous system dysfunction. The interactions between these three mechanisms make bowel's function susceptible to many exogenous and endogenous factors like gastrointestinal flora, feeding and psychosocial factors. Further data indicate that according to the above mechanisms, the influence of genetic factors and polymorphisms of human DNA in the development of IBS is equally important. So in lay mans language till date we don't know the exact cause of this problem. So the million dollar question is how to repair a system when you don't know which part of it is faulty (Some says it can be brain other syas it can be hormone anxiety age etc etc). Some patients have attacks of diarrhea and bloating and some may have constipation. Other symptoms that may be present in irritable bowel syndrome include nausea, vomiting, headache, loss of appetite, muscular pains, heartburn, and weakness. Some patients have occasional symptoms whereas others may have symptoms for a long period of time. So as seen above every patient is different. Intensity of symptoms in every patient is different as so is the response to medicines and life style change. Every drug is not effective in every patient so we have to try different therapies till we find the drug which suits best our patient. Dicyclomine 20 mg four times a day is to heavy a dose for 80 year old women. Dicyclomine ios an anticholinergic drug. An anticholinergic agent is a substance that blocks the neurotransmitter acetylcholine in the central and the peripheral nervous system. Anticholinergics inhibit parasympathetic nerve impulses by selectively blocking the binding of the neurotransmitter acetylcholine to its receptor in nerve cells (In simple words it messes up with both your peripheral and central nervous system) Mebeverine is an antispasmodic medicine and is very useful in IBS and is well tolerated by patient with minimal side effects. Mebeverine works on certain muscles in the wall of your intestines, causing them to relax. Clidinium bromide is an anticholinergic (specifically a muscarinic antagonist) drug. It may help symptoms of frequent stools. Chlordiazepoxide works by increasing the action of GABA, a chemical messenger which suppresses the abnormal and excessive activity of the nerve cells in the brain. Can be used in combination with above mentioned medicines. Another OTC medication is Bismuth sub-salicylate. It belongs to a class of medications called antacids and adsorbents marketed by name of Pepto Bismol. Doses up to 1gram can be taken daily to provide symptomatic relief. Loperamide can be used but should be avoided. Diphenoxylate and Atropine combination (Lomotil)-Atropine/diphenoxylate is an antidiarrheal and anticholinergic combination. For bloating and gas take Tab Nexium 40 mg twice daily. Take a probiotic daily with food as proper digestion of food will prevent excess gas formation. Food pattern recommended is- Eat simple and natural home cooked food. If possible try detox diet (Raw vegans) for atleast 3 month. Take normal food in lunch while in supper take uncooked raw natural form of food. This can play wonders. Avoid taking excessive fibres. A total fiber intake of about 20-30 grram per day would be good for you. Any thing in excess is good for nothing. No pizza, Burger,Red meat, and dairies for few weeks. No alcohol or smoking. Take natural food rich in probiotics like Yoghurt, Kimchi,etc. This doesnot mean that you have to take them as much as you can. Follow your body increase their uptake gradually. Drink lots of juices and fresh water daily. Avoid over spicy and heavy food. If possible go for a detox diet but again don't force your body rather let your body adopt gradually to things. Anal fissure and fistula- An anal fissure is a small tear or crack in the lining of the anus so in lay mans language its an internal wound. Certain treatments can promote healing and help relieve discomfort, including stool softeners (you already have soft stools) and topical pain relievers. If an anal fissure doesn’t improve with these treatments, surgery may be required. Salts adviced for trial before going for surgery are- A broad spectrum antibiotic like Cefuroxime to prevent bacterial infection of site. Combination of trypsinogen with serratiopeptidase (Wound healer). Dicofenac as an anti inflammatory. Ointment containing broad spectrum antibiotic to be applied locally over fissures. Vitamin C (wound healer). Folic acid and zinc supplement. However initially an anoscopy should be done by a Rectal surgeon to see the extent of fissure. Best surgical option is Video-assisted anal fistula treatment (VAAFT) is a novel minimally invasive and sphincter-saving technique for treating complex fistulas. As per the standard data of NCBI (National centre of biotechnological information) a facility funded by American govt data's are-"From May 2006 to May 2011, 136 patients were operated using VAAFT. Ninety-eight patients were followed up for a minimum of 6 months. 1) No major complications occurred. In most cases, both short-term and long-term postoperative pain was acceptable. 2) Primary healing was achieved in 72 patients (73.5%) within 2–3 months of the operation. 3) Sixty-two patients were followed up for more than 1 year. The percentage of the patients healed after 1 year was 87.1%. Hermorrhoids- 1) Sitz bath-Sit in about 3 inches of warm water for 15 minutes or so, several times a day. This helps reduce swelling in the area and relaxes your clenching sphincter muscle. 2) A 1% hydrocortisone cream on the skin outside the anus can relieve itching and pain. 3) With an electric probe, a laser beam, or an infrared light, a tiny burn will be made to remove tissue and painlessly seal the end of the hemorrhoid, causing it to close off and shrink. 4Rubber band ligation is a procedure in which the hemorrhoid is tied off at its base with rubber bands, cutting off the blood flow to the hemorrhoid. Hello everyone, I am Dr Archana Prabhu. I am an Obstetrician and gynaecologist, practicing in MUmbai. Today I will like to discuss about Thyroid Problems.

What is Thyroid?

Thyroid is a small endrocrine gland situated in front of the sound box, in the middle of the neck. 

Why is it so important?

The importance of thyroid gland is that it controls our metabolism. It controls the heart rate, skin thickness, texture. It controls our brain, mood, nervous systems, hair and so many other things are controlled by Thyroid. Any thing goes wrong with thyroid. If secretion becomes more, we call it as hyperthyroidism or if secretion become less, we call it as hypothyroidism. THere are problems, out of these problems, 75% of the problems are Hypothyroidism. THat means the secretion through thyroid glands are sub normal. So, I would like to discuss Hypothyroidism.

What happens in Hypothyroidism and how do we come to know that we are suffering from this problem?

Usual problems are;

What happens when patient's thyroid levels are not under control or they are Hypothyroid during pregnancy?

In this case, th baby at its very early stage are exposed to the level of hypothyroidism. There are chances of abortion. In case the pregnancy continues, the baby may develop many complications like, at birth, this baby might be having growth issues i.e, growth retardation.

  • This baby may have jaundice.
  • This baby may develop short stretcher
  • Poor growth
  • Cardiac issues

The very important part here is that mental development of the baby gets affacted by thyroid level. It has been seen when the baby is exposed to low secretion thyroid level in the early months, they develop mental retardation or low IQ.

I dont think that any of the mother would liek to have this kind of baby, that too because of the small thing which she could have checked and be treated and could easily escape from such a complications. Now, they are certain problems which can go really wrong with the mother also if she has hypothyroidism and the levels are not under control. LIke the blood pressure, she may get into preeclampsia. The mother who is suffering from Hypothyroidism are at risk of developing some kind of cardiac problems.

So, friends the important point here is to check thyroid level in your early month of pregnancy. There are levels which are T3, T4 and TSH. Check thema nd if they are shwoing any kind of abnormality, the tablet is started immediately. It is a very small tablet can be taken anytime empty stomach. Throughout the pregnnacy, every 4-6 weeks we have to keep checking thyroid levels and according to pregnancy month, the levels of thyroid are different for the three trimesters.

We have to keep the levels in that particular level. So the dose adjustment and regular checkup are really important. THis can control teh complications and can save the baby and the mother.

NSTRUCTIONS FOR PEOPLE WITH ACUTE LOW BACK PAIN

  • The simple rule is that if bending forward has been the cause of over stretching, bending backward should rectify this problem and reduce any resultant distortion. You must restore the lordosis slowly and with caution, never quickly or with jerky movements. You must allow some time for the distorted joint to regain its normal shape and position: a sudden or violent movement may retard this process, increase the strain in and around the affected joint, and thus result in an increase of low back pain .
  • Remember, when commencing the exercises, some increase of mid line low back pain can be expected. Some exercises will only be effective when you actually move into the pain while exercising. You should feel some pain when doing these exercises, but you should never have a lasting increase in pain which remains present the following day.When in acute pain, you must, apart from exercising, make certain adjustments in your daily activities. These adjustments forma very important aspect of self-treatment. If you do not follow the instructions given below, you will unnecessarily delay the healing process. This is entirely your responsibility.Maintain your lumbar lordosis at all times. 
  • Slouched sitting and bending forward as in touching the toes will only increase the pressure in the joints, stretch and weaken the supporting structures,and lead to further damage in the low back. If you slouch you will have discomfort and pain. 

Good posture is the key to spinal comfort. Sit as little as possible, and then for short periods only. 

  • If you must sit, choose a firm, high chair with a straight back, make sure that you have an adequate lordosis, and use a lumbar roll to support the low back. Avoid sitting on a low, soft couch with the legs straight out in front as in sitting up in bed or in the bath; both these situations force you to lose the lordosis. When getting up from the sitting position, you should attempt to maintain the lordosis: move to the front of the seat, stand up by straightening the legs and avoid bending forward at the waist.
  • Drive the car as little as possible; it is better to be a passenger than to drive yourself. If you must drive, your seat should be far enough back from the steering wheel to allow you to drive with your arms relatively straight. 
  • With the arms straight, your upper body is held back and you are prevented from slouching; this allows full benefit to be obtained from the lumbar roll, which should always be used when driving. Avoid activities which require bending forward or stooping. Many activities can be modified adequately to enable maintenance of the lordosis. It is possible to maintain the correct posture when vacuuming in the standing position; it is also possible to maintain a correct lordosis by going down on all fours when gardening, making beds, etc.
  • If you have acute low back pain, you should not lift at all. If you must lift, you should avoid objects that are awkward to handle or heavier than 30 pounds (15 kg). At all times you must use the correct lifting technique.
  •  If you are uncomfortable at night, you may benefit from a supportive roll around your waist. For most people, it is recommended that the mattress should not be too hard but well supported by a firm base.
  •  If your bed sags, slats or a sheet of plywood between mattress and base will straighten it, or else you can have the mattress placed on the floor.When getting up from lying, you should keep your back in lordosis: tum on one side, draw both knees up, drop the feet over the edge of the bed, raise yourself to the sitting position by pushing your upper body up with your hands, and avoid bending forward at the waist. Stand up from sitting as described before.
  • Avoid coughing and sneezing while you are sitting or bending forward. You should stand and bend backward if you are forced to cough or sneeze.Avoid those positions and movements which initially caused your problems. You must allow some time for healing to take place.
