Terbutaline

Prescription vs.OTC: Prescription by Doctor required

Being a bronchodilator, Terbutaline, works by relaxing the muscles around the bronchioles in the lungs and opening them up. This helps air to pass in and out easily of the lungs. It is useful to treat spasms or contractions of the bronchioles, seen in asthma, bronchitis and emphysema.

It is not to be used if you are allergic to any of its ingredients or to a sympathomimetic amine like epinephrine and albuterol. Let your doctor know if you are pregnant, planning to be pregnant or are breastfeeding. While beta blockers may decrease its effectiveness, insulin’s effectiveness is decreased if taken along with Terbutaline.

Terbutaline is usually in the form of an injection given at the doctor’s office itself. If you are using it at home, make sure you learn the correct procedure from your doctor first. Do not reuse syringes or needles. If you miss a dosage and it is nearly time for the next, skip it. Do not take two dosages one after another.

Headache, nausea, nervousness, drowsiness, dizziness are all common side effects of Terbutaline that may be seen.

What is recurrent miscarriage?

If you have three or more miscarriages in a row, doctors call it recurrent miscarriage. If you have experienced recurrent miscarriage, your GP or midwife will refer you to a gynaecologist. Your gynaecologist will try to identify the reason for your losses.

Having miscarriage after miscarriage may leave you feeling utterly drained of hope. At times, it may be hard to keep trusting in the future. This experience affects every aspect of a woman’s life from her mental and emotional health to her physical health and social well-being. 

If you can, try to draw comfort from the fact that most women who experience recurrent losses do go on to have a baby. This is especially the case if tests can find no reason for the losses. Six out of 10 women who have had three miscarriages will go on to have a baby in their next pregnancy.

Treatment of Recurrent Pregnancy Loss

Treatment for anatomic abnormalities of the uterus involves surgical restoration through removal of local lesions such as fibroids, scar tissue and endometrial polyps or timely insertion of a cervical cerclage (a stitch placed around the neck of the weakened cervix) or the excision of a uterine septum when indicated.

A thin endometrial lining has been shown to correlate with compromised pregnancy outcome. Often times this will be associated with reduced resistance to blood flow to the endometrium. Such decreased blood flow to the uterus can be improved through treatment with sildenafil (Viagra), Terbutaline and possibly aspirin.

Sildenafil (Viagra) Therapy Viagra has been used successfully to increase uterine blood flow. However, to be effective it must be administered starting as soon as the period stops up until the day of ovulation and it must be administered vaginally (not orally). Viagra in the form of vaginal suppositories given in the dosage of 25 mg four times a day has been shown to increase uterine blood flow as well as thickness of the uterine lining. To date, we have seen significant improvement of the thickness of the uterine lining in about 70% of women treated. Successful pregnancy resulted in 42% of women who responded to the Viagra. It should be remembered that most of these women had previously experienced repeated IVF failures

Terbutaline this is a medication that relaxes the muscle in the uterine wall and so permits improved hormone delivery to the endometrium. The use of Terbutaline will often cause an increase in heart rate. It should not be prescribed to women who have irregular heart beats (arrhythmias), and women who have decreased cardiac reserve.

Aspirin this is an antiprostaglandin that improves blood flow to the endometrium. It is administered at a dosage of 81mg orally, daily from the beginning of the cycle until ovulation.

Selective Immunotherapy Using Intralipid, heparin, aspirin and corticosteroid

Many causes of pregnancy loss or failure can be treated with immunotherapy comprising combinations of aspirin and heparin and corticosteroids (dexamethasone or prednisone) and Intralipid (IL) to regulate increased level of Natural Killer Cell Activation (NKa). Achievement of optimal success with Intralipid/corticosteroid therapy requires that the treatment be initiated well before ovulation takes place (about 7-14 days prior to anticipated implantation). Given the fact that only 10-15% of natural cycles (with or without the use of insemination and/or fertility drugs) will result in a pregnancy, it follows that repeated administration of Intralipid will be required in most cases before a pregnancy will occur. IVF achieves pregnancy rates that are often 2-3 times higher. This often makes IVF a treatment of choice in cases of immunologic recurrent pregnancy loss.

Role of IVF

Preimplantation genetic diagnosis (PGD) a procedure whereby the embryo can be tested for genetic or structural chromosomal abnormalities requires the use of IVF to select the best embryo(s) for transfer to the uterus. In cases of structural chromosomal (translocations) egg or sperm donation is often another option worth considering.

In those cases where due to intractable anatomical or alloimmune dysfunction IVF repeatedly is unsuccessful or is not an option, Gestational Surrogacy might represent the only recourse other than adoption.
If a couple with Recurrent Pregnancy Loss is open to all of the diagnostic and treatment options referred to above, a live birth rate of 70% – 80% is ultimately achievable.

Types of Diabetic Neuropathy-
Diabetic neuropathy can be classified as Peripheral, Autonomic, Proximal, or Focal. Each affects different parts of the body in various ways...

Peripheral neuropathy, the most common type of diabetic neuropathy, causes pain or loss of feeling in the toes, feet, legs, and hands.

Autonomic neuropathy affects the nerves that control involuntary body functions such as digestion, bowel and bladder function, sexual response, and perspiration. It can also affect the nerves that serve the heart and control blood pressure, as well as nerves in the lungs and eyes. Autonomic neuropathy can also cause hypoglycemia unawareness, a condition in which people no longer experience the warning symptoms of low blood glucose levels.

Proximal neuropathy causes pain in the thighs, hips, arms, or buttocks and leads to weakness in the legs and hands, resulting in difficulty in walking, standing, picking up objects, buttoning your clothes, etc.

Focal neuropathy results in the sudden weakness of one nerve or a group of nerves, causing muscle weakness or pain. Any nerve in the body can be affected.

How Diabetes Causes Damage to the Nervous System?
There are several factors that are likely to contribute to nerve damage through diabetes...

  • high blood glucose, a condition associated with diabetes, causes chemical changes in nerves. These changes impair the nerves' ability to transmit signals. 
  • high glucose levels affect many metabolic pathways in the nerves, leading to an accumulation of a sugar called sorbitol and depletion of a substance called myoinositol. These changes are the mechanism that causes nerve damage. Nitric oxide dilates blood vessels. In a person with diabetes, low levels of nitric oxide may lead to constriction of blood vessels supplying the nerve, contributing to nerve damage.
  • presence of mechanical injury like carpal tunnel syndrome in a diabetic patient worsens its symptoms and prognosis
  • inherited traits increase susceptibility to nerve disease
  • lifestyle factors, such as smoking or alcohol use

Symptoms:

  1. Numbness, burning sensations, tingling, or pain in the toes, feet, legs, hands, arms, and fingers
  2. Either hypersensitivity to touch or insensitivity, even to hot and cold temperatures
  3. Weakness in muscles and loss of reflexes
  4. Indigestion, nausea, or vomiting
  5. Diarrhea or constipation
  6. Dizziness or faintness due to a drop in blood pressure after standing or sitting up
  7. Problems with urination
  8. Changes in gait and balance
  9. Injuries that are taking longer to heal and are more prone to infections

Prevent Diabetic Nerve Damage-
Keeping your blood sugar levels in your target range, set with your doctor, may help prevent nerve damage from ever developing. The best way to do this is by checking your blood sugar and adjusting your treatment. Vision, hearing, and speech are an important part of your child's life. When an infant is born, his or her eyesight is immature. The infant later develops the ability to focus. Hearing appears early in fetal development and is necessary for proper progression of speech and language. Monitoring your child's ability to see, hear, and speak is an important part of the health of your growing child.

The American Academy of Ophthalmology, the American Optometric Association, and the American Academy of Pediatrics (AAP) have recommended the following vision screening stages:

  • Newborn. All newborns are examined in the nursery for eye infections and other eye disorders, such as glaucoma.

  • 6 months. Visual screening of infants should be performed during the well-baby visits, particularly checking for how the eyes work together.

  • 3 to 4 years. Formal visual acuity tests and the complete eye exam should be performed.

  • 5 years and older. Annual visual screening tests and eye exams should be performed.

Children develop speech, language, and hearing skills at different ages. However, hearing loss can lead to delays in your child's ability to make sounds, learn to speak, and communicate. The AAP recommends hearing screening for all newborns before they leave the hospital. Talk with your child's healthcare provider if you're concerned about your child's hearing or speech, or if you notice any of the following:

  • No response to sound at any age

  • Infant doesn't move or jump when a loud sound is made

  • No babbling by the time the infant is 9 months old

  • No words spoken by age 18 to 24 months

  • Doesn't follow simple commands by age 2

  • Poor voice quality at any age

A cataract is defined as a clouding of lens in the eye where your vision gets blurred. A cataract affects the eyes, when light that passes through the lens prevents a clearly formed image from reaching your retina. The disease is very common and usually develops as your eyes age or due to any injury caused to the tissues that cover your eye's lens.

Types of cataracts:

  1. Senile CataractThis is the commonest of all. It is age-related clouding of the lens. It can affect the near or distance vision and can also cause glare and change in glasses power.
  2. Secondary Cataract: It can be developed after surgery for other eye problems like glaucoma and other health conditions such as diabetes.
  3. Traumatic Cataract: It can develop many years later after an injury caused to your eye.
  4. Congenital Cataract: As the term explains, the disease may be inborn or some children might develop it at a later stage which often affects both eyes. 
  5. Radiation Cataract: It can form after you are exposed to some form of radiation.

A cataract surgery involves the extraction or cleaning of the cloudy lens, which is then replaced by a clear artificial lens.

Why does it happen? 

There are lens that lies behind our iris and pupil which act much like a camera lens. It helps concentrate light onto the retina at the back of our eye to form a sharply defined image. Besides, the lens also helps our eyes to adjust focus and allows us to see things clearly both far away and up close. The lens is composed of protein and water where the protein is arranged in a manner to keep the lens clear thereby letting light pass through it. 

However, as we grow old, some of the protein starts to become thick and cloud a small lens area. This is known as a cataract. With the passage of time, it may inflate and cover more of the lens, making it difficult for us to see.

Besides, there are other causes of cataract such as smoking, addiction of alcohol, prolonged sunlight exposure, to name a few. 

When should you opt for a cataract surgery?

Believe it or not, but till date no eye drop or medication has proven to reverse or prevent the formation of a cataract. If a cataract is affecting your nearsightedness or alteration in your prescription, then new prescription eyeglasses may help to better your blurred vision. However, the only treatment for a cataract is the surgical removal of your natural lens. And, most eye doctors recommend this surgery only when the problem becomes severe and starts hampering your day-to-day activities, such as studying or driving at night.
