What do you mean by anal fissures?
An anal fissure is a little laceration in the moist tissue that lines the opening of the anus. An anal fissure might develop as a result of a trauma. For example, a person suffering from constipation (or diarrhea) can develop fissure as a result of exerting too much pressure while defecating.

Piles, fistula and fissures are the most common problems affecting the rectum and the anus. Both the rectum as well as the anus has a very rich vascular supply. There could be swelling of these blood vessels or tearing which can lead to the above conditions. Read on to know additional details about each of these conditions.

Piles: In simple words, swollen blood vessels of the anal region are called piles. It can be internal or external, depending on its position inside the rectum or outside the rectum respectively.


Chronic constipation, where the person has to strain to pass stools
Increased frequency of motion as seen in dysentery etc.
Increased abdominal pressure like in pregnancy, obesity etc.
Smoking and genetics can also lead to piles.
Symptoms: Starts off as painless bleeding, which happens in the form drops or blood spurts during bowel movement (1st stage); the swelling develops later on. They can continue to grow and may come out of anal canal while passing stool (2nd stage). Later on Pile mass comes out even on sneezing, coughing or standing for prolong time (3rd stage)


In the early stages, once identified, simple Kshar Karma (application of special medicines) is effective..

Prevention is advised if there is a family history by taking measures like consuming a fiber-rich diet and maintaining hydration, regular exercises. Sphincter exercise etc.
Fistula: When an anal gland gets infected; it may discharge pus around the anal region or inside anal canal. Anal Fistula is characterized by presence of a nodular swelling around anal orifice with on and off discharges. This condition is usually painless however some pain may be noticed in the swelling before the pus discharges. After pus discharge pain and swelling disappears to recur again after some time.


A constant wet feeling in the anal area, due to pus discharge.
This would be intermittent, and there would be periods with no discharge.
There could be a foul smell due to pus discharge.
Diagnosis and Treatment:

Identifying the exact location is important for Ksharsutra ligation or Surgery/ VAAFT.
An ultrasound scan or an MRI is usually done.
Ksharsutra ligation is a better option as compared to surgery/ VAAFT as the chances of recurrence after surgery/ VAAFT are very high.
Ksharsutra being a non surgical procedure; is more convenient and more effective as compared to surgery. This has already been proved by the scientific trials conducted at AIIMS New Delhi, PGI Chandigarh etc. by Indian Council of Medical Research in 1990.

Fissures: Fissure is a very painful condition produced due to cracks anywhere in the length of the anal canal usually at the anal verge.


Very painful and the person may not want to go to the toilet
Rarely bleeding is also seen, maybe in very small amounts

Over 90% of the fissures do not require surgery and can be easily managed by Kshar Karma sittings.
Switching to a high-fiber diet and improving hydration will help in managing these symptoms.
Topical relief can be provided by application of medicated oils and paste.
When they are recurrent, a procedure known as lateral sphincterotomy is done. However this procedure is known to have a common complication of fecal incontinence. This is usually seen due to damage to sphincter muscles during surgery or over stretching. Patient is not able to control / hold the stools.

Piles or Haemorrhoids is a disorder characterized by bleeding from anal opening during defecation. This bleeding can be in the form of drops or spurts. If untreated; mass protruding out from rectum is noticed. Initially this Pile mass gets reduced by itself but later on it progresses further so manual reduction of Pile mass becomes necessary. Later on these pile masses come out even on coughing sneezing, walking etc. Depending of their site of origin; these are divided in to internal and external Piles.

How can you tell if you have Piles?
Painless bleeding is the first sign of Piles. If it is not treated at this stage Pile mass protrudes out from Anal opening. The symptoms of piles are

Internal Piles:

In first stage, painless bleeding is the only symptom of Piles.
In second stage, pile mass protrudes out from Anus during defecation which gets reduced by itself. In late second stage; Pile mass needs to be reduced manually.

An anal fistula refers to a small infected tunnel connecting the anus to the outer skin. Your body eliminates solid waste products or feces through the anus. When any of the glands inside the anus becomes infected or blocked, an abscess may form, which in turn results in formation of anal fistula. In about 50% of the cases, abscesses develop into fistulae. What are the causes for anal fistula? Anal fistulae occur primarily due to the formation of an abscess or infection in the anus. If the abscess does not heal completely after the pus has been drained away, a fistula can develop.

Other factors responsible for the occurrence of anal fistulae are-

Crohn s disease- a chronic condition where the digestive system becomes inflamed.
Diverticulitis- an inflammation of the small pouches that develop along the intestinal walls.
Hidradenitis suppurativa- a chronic painful skin condition which causes scarring and abscesses on the skin. Apart from these, people with HIV or infected with Tuberculosis disease also develop anal fistulae. A tear in the anal canal is called anal fissure. When the skin breaks, it can cause pain and extreme discomfort. Some amount of bleeding during defecation may also be experienced. When the anal aperture is over-strained, it results in a crack leading to fissure. In women, delivery trauma can lead to anal fissure. Ulcer in the anal canal or sexually transmitted diseases too can cause the anal fissure. In children, it is commonly due to injury by passing hard stool. Fissure is usually seen at the anal verge. This being a highly sensitive area; any scratch or tear in this area gives excruciating pain, burning, itching, irritation like symptoms. There may be spasm in the anal region (which is a protective mechanism) and the patients are scared of passing stools. Sometimes patient avoids eating food so as to avoid going to toilet. Usually the pain subsides in an hour or so. However in some conditions where there is underlying inflammation and infection; the pain may persist whole day. Recurrent anal fissure which has not been treated properly give rise to an extra skin growth known as sentinel tag.

Ayurvedic remedies: In the patient suffering from Anal fissure; it is very important to regulate the bowel habits properly. Some mild laxatives may be used to soften the stool. Hot sitz bath and use of some soothing ointments, coconut oil, ghee etc. are helpful as they avoid the irritation. Ayurveda can completely cure the people suffering from anal fissures. The treatment duration differs from one individual to another. This is also based on the severity of the disease. In acute anal fissure; the allopathic doctors usually recommend surgery. Piles can be really painful and embarrassing but fairly common at the same time. it should not be taken as a social stigma as with awareness and proper treatment, the condition can be managed and treated well. If overlooked then it may lead to many complications which are difficult to manage.

In Ayurveda, a complete cure to haemorrhoids is possible, so seeking medical attention can be of great help to relieve you from this condition. Piles, also known as Haemorrhoids is a condition in which veins at the lower end of rectum get inflamed and enlarged. When pressure on the rectum and anus increases, it causes added pressure to these veins, resulting in formation of haemorrhoids. They may burst and cause rectal bleeding. Haemorrhoids are most commonly found in middle-aged and old people while also in people who suffer from regular constipation.

Types of Piles -

Haemorrhoids are of two kinds, internal and external based on their location. Internal haemorrhoids are located in the inner lining of the anal tract. These veins within the rectum are concealed and cannot be felt so they are painless but when ruptured can lead to bleeding. External haemorrhoids are located on the outer surface of the rectum and can be seen, felt and when inflamed become painful. These are the more troubling type of piles.

Piles are caused mostly by constipation, but other causes may include untreated gastric conditions such as dysentery, sedentary lifestyle, obesity, genetic constitution and weight of increasing uterus during pregnancy.
If you are experiencing bleeding in anal region during defecation without any pain; you are suffering from 1st stage of Piles. If problem is not treated at this stage; Pile masses start progressing.
In starting, Pile mass come out during passing stool and go inside by itself (2nd stage). Later on these pile masses need to be reduced manually. If still Piles is not treated; Pile mass comes out again even after manual reduction while coughing, sneezing etc. (3rd stage). While piles may be an embarrassing issue to talk about, it should not be ignored either. It can be easily dealt with simple ways, and in most cases, all it takes is some lifestyle changes to get rid of the problem.
However, if left untreated, there could lead to severe complications. Some of them have been listed below.

Bleeding: In patients who have chronic bleeding, it could lead to chronic blood loss. Patient can develop anaemia because of this regular bleeding.
Mass protruding out from anal opening: If Piles are not treated at 1st stage; it can lead to development of pile mass which protrudes out from anal opening while passing stool and go inside.
